Nurses will be asked to help patients manage chronic illnesses Nurses must now use complex information management systems to improve the quality and effectiveness of care. Nurses are integral members of multi-disciplinary care teams

2013 Highlights Conducted our first ever Prevention Challenge Campaign aimed at promoting workplace wellness. The winner was Allen & Gerritsen a Boston-based PR firm for their challenge Get Your A in G Updated and expanded our website and membership portal for an improved user experience. Increased advocacy efforts, including launch of our first ever Virtual Lobby Day which resulted in 661 letters send to legislators! Expanded our patient and professional education offerings - launched our online Heart Healthy Toolbox and offered several webinars and more than 24 regional live programs.

As a Hispanic nurse, she is passionate about educating women in her community regarding CVD risk factors and prevention utilizing literacy appropriate materials and health communication.

PCNA Heart Failure Prevention Award This award recognizes a registered or advanced practice nurse who has demonstrated an innovative, successful model of primary or secondary preventive care for patients with heart failure. The best practice model or research program should be culturally appropriate and relevant to a middle aged or geriatric population.

Preventing avoidable readmissions for patients with heart failure is a national focus for quality outcomes.
