MISSION CACHE improves health by eliminating health disparities that affect racial, ethnic, and other atrisk populations through collaborative partnerships. VALUES Diversity: We are representative of the community with regard to race, ethnicity, gender, class, age, language, etc. Collaboration: We work together and with other community organizations to achieve our mission, vision, and goals. Accountability: We honor our commitments to one another, to members, to the organization, and to the community and supporters.

2 ORGANIZATION CACHE is a nonprofit corporation headquartered in Charlotte, North Carolina. It has a governing board and four Councils, each of which has task forces and work groups: Access to Healthcare Cultural and Linguistic Competence Quality Improvement Research and Science ACTION PLAN ORGANIZATIONAL Complete steps for organization structure and operation Internal policies and procedures Team building Hire staff and accounting, legal, and other professionals Secure a home Develop organization identity package Funding to sustain organization, programs, and projects

3 ACCESS COUNCIL Connect people to healthcare services and healthcare services to people Health and Wellness Education Screening programs Community Resource Book for Seniors Surveys for providers Empower community based organizations to better attend to the basic healthcare needs of their constituents Encourage development of Parish and Congregational Nurse Ministries Train Lay Health Advisors Foster an environment of trust and cooperation between established institutional providers and community based organizations Educate health service providers about health disparities and how their practice patterns impact the problems Assist in educating providers about the North Carolina Best Care Project Distribute information about new innovations in health care Publish professional papers on health disparities Engage community based organizations on the process of improving the health of their constituencies Promote participation in CACHE Promote participation in Parish Nurse Certification Programs Send communications to neighborhood groups and key gathering places Provide a workshop on community capacity planning Advocate for public policy changes that advance health equity and access Monitor state and local legislation Comment on proposed legislation Encourage servant leadership projects in the community high school, college, and graduate school students

4 CULTURAL AND LINGUISTIC COMPETENCE COUNCIL Increase the number of students of color (African American, Hispanic, Latino/Latina, Asian American, and Native American) who choose healthcare as a field of study. Identify health care administrators and others committed to introducing students of color to healthcare professionals (i.e., who are already working with youth) Partner with identified others to develop a week long institute to encourage students of color to consider healthcare as a profession Partner with identified others to be speakers on issues of health disparities to students of color Empower the CACHE board and members to increase interpersonal effectiveness in addressing issues of cultural and linguistic competence at personal, interpersonal, institutional, and cultural levels. Conduct a cultural and linguistic strategic competence workshop Conduct a quarterly cultural and linguistic strategic competence session Review the cultural and linguistic competence plan, annually. Offer cultural and linguistic competence workshops to patients (including children and youth), community groups, and healthcare providers to help improve the quality of health care. Provide at least four workshops, each year, to healthcare providers who primarily work in communities of color Partner with AHEC, Mecklenburg County Health Department, and identified others to conduct workshops Advocate for state and local policies that address issues of cultural and linguistic competence in health care. Identify state and local entities that distribute funds for health care disparities and determine whether policies are in place to address cultural and linguistic competence Develop alliances with other advocates for cultural and linguistic competence in health care Develop and/or advocate for policies that insure that those receiving funding for health care and health care disparities have policies in place that address cultural and linguistic competence skill issues

5 QUALITY IMPROVEMENT COUNCIL Serve as key regional leaders and partners in community based initiatives and efforts to reduce disparities in the quality of healthcare, locally and nationally. Identify the problems related to quality and healthcare disparities. Host Town Hall meetings Identify key concerns, ideas, and initiatives Sponsor education and outreach programs Develop measuring tools to gauge how we are doing. Define and formulate benchmarks for quality of care for minority and underserved populations. Research and analysis of needs and gaps Measure available data for respective diseases, conditions, and issue areas Identify cost effective, efficacious, and evidence based strategies Identify system wide barriers (provider and consumer side) to providing high quality health care and create/ find workable solutions Create community initiatives based on research outcomes 2006 Minority Health Symposium (focus: diabetes) Develop outreach and education projects that promote creative thinking and skill development RESEARCH AND SCIENCE COUNCIL Develop an interdisciplinary academic research partnership in health disparities Seek external funding to support infrastructure development Develop directory of health disparities research in our region Identify mechanisms to foster awareness of health disparities education and training Promote education and training about health disparities Collaborate with national and local Urban League
