DPARTMNT The Pharmacy services operate over 4 sites within Alfred Health and involve in training interns, undergraduate and overseas research students. Pharmacists are essential members of all clinical teams and have a critical role in patient management. Their understanding of Pharmaceutics, Pharmacology and Medicine Management ensures Alfred Health patients receive the best pharmacy care possible. POSITION SUMMARY The Alfred Health Pharmacy Technician, Clinical Support Services Pharmacists at ward level in order to; provides technical support to Clinical Upon completion of accreditation, a Pharmacy Technician, Clinical Support Services will competently demonstrate and contribute to the management and handling of medicines at ward level to ensure prescriptions and requisitions orders required are received and processed with minimal delays. Reduce wasted medicines through regular monitoring of Imprest lists, Non Imprest supplies, returned medication and appropriate re-use of patient s own medication. Order Non Imprest items as per Alfred Health procedure and undertake a transcription assessment and reaccreditation in this area thereafter to maintain competency. Process returned medications for re-use in pharmacy as per Alfred Health procedure. Assess patient s own medicines for re-use during hospital stay as needed. Carry out Drugs of Dependence (DD) ordering and delivery as per Alfred Health procedure. Co-ordinate the review of Imprest lists and Non Imprest Supplies with CH2 staff and Clinical Pharmacists. Provide financial information on Imprest values and returned medications at agreed intervals using Crystal Reports. Review and contribute to the development of Standard Operating Procedures. Maintain own competence as a Pharmacy Technician, Clinical Support Services by undertaking an Alfred Health re-accreditation assessment every 2 years. DIRCT RPORTS (if applicable): Daily reporting to the Senior Supervisory Technician, Clinical Support Services. Ad hoc reporting to Senior Clinical Pharmacist in area of work. KY RSPONSIBILITIS Working Relationships & Communication Work closely with CH2 staff and monitor imprest levels at agreed intervals in conjunction with the Senior Clinical Pharmacist. Work closely with the Clinical Pharmacist to ensure non-imprest supplies are made in a timely way and in accordance with specific monitoring, instructions or legal requirements. Provide an effective handover to colleagues prior to Annual Leave or Accrued Days Off to minimise disruption to service. Provide information to other health professionals within limits of job role and refer appropriately to the Clinical Pharmacist. Work closely with the Senior Supervisory Technician, Clinical Support Services, to continuously improve the efficiency and effectiveness of the clinical support service. Meet regularly with the clinical teams and contribute to the development and improvement of Clinical Support Services. Liaise with nursing staff on medicine supply and storage issues. Work independently at ward level by prioritising own work and referring to the pharmacist by pager or telephone when needed. Refer to nursing staff as appropriate Make best use of patients own medications as per local policy by liaising with the clinical pharmacist Participate in data collection for audit, quality improvement or research purposes. Collect medication history information as needed from Nursing Homes, GP Surgeries and/or Community Pharmacies. Order aseptic products as directed by the Senior Pharmacist. Safety Bring to the attention of the Clinical Pharmacist any supply problems to avoid missed dose Feedback dispensing errors identified at ward level to staff involved in a constructive manner to support reflection and learning. Assess medicines returned from the ward as suitable for re-use in pharmacy, in accordance with Alfred Health returns procedure. Monitor drug storage conditions on ward and feedback to Senior Pharmacist if there are any concerns or problems. Attend Medicine Safety Walkarounds and complete any actions for as a result of the Walkaround, as deemed appropriate by the Med Safety pharmacist, Senior Pharmacist or Nurse Unit Manager. Medication Supply Order and dispense Non Imprest items Dispense and co-ordinate the supply of discharge prescriptions to ensure they always meet the required time for patients on the ward. Produce Medication Charts, including Foreign Language Charts and variable dosage charts as requested. Prepare and deliver urgent prescriptions or stock as required Order and deliver DDs Assist with the supply and administrative procedures for SAS and Non Formulary medications Personal and People Development Contribute to own annual Performance Management Review. Identify own gaps in training and/or education and bring to the attention of the Senior Supervisory Technician, Clinical Support Services. Assist the Senior Supervisory Technician, Clinical Support Services with training and supervision of new pharmacy technicians. Participate in ongoing learning and development as a Pharmacy Technician. Participate in clinical team meetings and clinical learning and development programmes. Attend and contribute to the Society of Hospital Pharmacists of Australia (SHPA) seminars and conferences. Maintain SHPA membership. Train members of own ward based team and dispensary staff to ensure a consistent and efficient dispensing process with no unnecessary delays Alfred Health Ward Based Pharmacy Technician Page 3 of 7
Reporting and Quality Assurance Generation and analysis of reports on medication usage and collation e.g. tracking of high costs drugs, Imprest and Non-Imprest supplies, returned medication. Data collection as required for quality initiatives, audits, KPI generation and research. nsure records are maintained as per local standard operating procedures. nsure all documentation filing is kept up to date and archived as necessary, following legal requirements for the retention of pharmaceutical and other records. Actively contribute to the review and development of procedures and services. Prepare financial, activity reports and audit results as necessary. Present information to colleagues and other health professionals as needed. Health and Safety xcellent physical fitness and good general health to walk regularly between wards. Ability to apply good moving and handling technique. Adhere to Alfred Health procedures for the safe storage and disposal of medicines, and other hazardous substances stored and supplied from the pharmacy. Minimise hazards in the working area and report any identifiable hazards to the Senior Pharmacy Technician, Clinical Pharmacist or Nurse in charge of the ward. nsure all medicines are stored under appropriate conditions e.g. fridge items, and that all medicines are rotated and expired stock disposed of appropriately. Future Development As this is a continually expanding role for Alfred Health, the post holder will need to be flexible and adaptable to meet the changing needs of service, including working hours. The post holder may be required to undertake training to carry out the following tasks; Assist with patient medication counselling during hospital stay and on discharge Assist with the Medicines Reconciliation process Assist with accuracy checking of inpatient and discharge medication following Alfred Health procedures. Assist with monitoring prescribing of specific medicines use e.g. high cost or non formulary medicines. Assist with the identification and recording of allergy and sensitivity status. Assist with Therapeutic Drug Monitoring. General Assist the Ward Based Senior Technician in general administrative responsibilities Represent the Pharmacy Department when required to attend communication meetings outside and inside the Hospital. Participate in weekend and public holiday services according to departmental roster. Actively participate in the Alfred Health Performance Management Program. Maintain and document ongoing CPD at a level applicable to the post. Use of ipharmacy, UList, Powerchart and other information systems as needed at Alfred Health.
