INSTRUCTIONS TO PROPOSERS 1. SCOPE OF WORK Project Description: Coffeyville Community College (CCC) is issuing this Request for Proposals (RFP) soliciting proposals from qualified firms to provide all services for the complete abatement and disposal of Asbestos Containing Materials >1% in various buildings, as outlined in the report dated January 7, 2016 by Paradigm Group, LLC. A summary of the survey findings in total and by building are below. Regulated Asbestos Materials Summary (>1% Asbestos) Friable Approximately 34,000 sf of wall texture w/overspray located in building A Approximately 28,400 sf of ceiling texture w/overspray in 7 buildings Approximately 6 sf of asbestos paper insulation in building M Approximately 61 sf of asbestos linoleum in building A Approximately 420 sf of asbestos drywall/joint compound in building F Category I Non Friable Building G has exterior asbestos caulking. Approximately 375 sf of floor tile & mastic in building A Approximately 310 sf of mastic in buildings A & D Approximately 2,200 sf of floor tile only in building C Category II Non Friable Approximately 7,750 sf of wall and ceiling plaster located in building M Approximately 2,450 sf of transite siding located on building M Approximately 20 windows with asbestos window glaze on building M The letters corresponding to the description of the buildings below are shown on the attached map for location reference. A. 413 W. 9 th Raven Apartments Two story, 10,600 sq. ft., 16 unit apartment building built in 1962. This building will be demolished. Asbestos Material: Friable Approximately 34,000 sf of wall texture. Approximately 10,500 sf of ceiling texture. Approximately 61 sf of asbestos linoleum. Category I Non Friable Approximately 375 sf of floor tile & mastic. Approximately 125 sf of mastic under other flooring.

B. 510 W. 10 th Raven Hall Units 1, 2, 3, 16, 17, 18 Two story building 3 first floor units with 4 bedrooms, a commons area and a common bathroom built in 1972. 3 second floor units with 4 bedrooms and a bathroom built in 1991. This building will be demolished. Asbestos Material: Friable Approximately 3,300 sf of ceiling texture throughout 1st floor. C. 509 W. 9 th Raven Hall Units 4, 5, 6, 19, 20, 21 Two story building 3 first floor units with 4 bedrooms, a commons area and a common bathroom built in 1972. 3 second floor units with 4 bedrooms and a bathroom built in 1991. This building will be demolished. Asbestos Material: Friable Approximately 3,300 sf of ceiling texture. Category I Non Friable Approximately 2,200 sf of floor tile only on 1st floor. D. 507 W. 9 th Raven Hall Units 7, 8, 9, 22, 23, 24 Two story building 3 first floor units with 4 bedrooms, a commons area and a common bathroom built in 1972. 3 second floor units with 4 bedrooms and a bathroom built in 1991. This building will be demolished. Asbestos Material: Friable Approximately 3,300 sf of ceiling texture throughout 1st floor. Category I Non Friable Approximately 185 sf of floor tile mastic located on the 2nd floor. E. 505 W. 9 th Raven Hall Units 10, 11, 12, 25, 26, 27 Two story building 3 first floor units with 4 bedrooms, a commons area and a common bathroom built in 1972. 3 second floor units with 4 bedrooms and a bathroom built in 1991. This building will be demolished. Asbestos Material: Friable Approximately 3,300 sf of ceiling texture throughout 1st floor.

F. 503 W. 9 th Raven Hall Units 34, 35, 36 Single story building 3 units with 4 bedrooms, a commons area and a common bathroom built in 1983. This building will not be demolished. Asbestos Material: Friable Approximately 420 sf of asbestos drywall & joint compound. G. 501 W. 9 th Raven Hall Units 37, 38, 39 Single story building 3 units with 4 bedrooms, a commons area and a common bathroom built in 1983. This building will not be demolished. Asbestos Material: Category I Non Friable Building has exterior asbestos caulking Gray. H. 502 W. 10 th Raven Hall Units 50, 51, 52 Single story building 3 units with 4 bedrooms, a commons area and a common bathroom built in 1983. This building will be demolished. Asbestos Material: No asbestos materials were identified in the samples taken. I. 504 W. 10 th Raven Hall Units 31, 32, 33 Single story building 3 units with 4 bedrooms, a commons area and a common bathroom built in 1983. This building will be demolished. Asbestos Material: No asbestos was identified in the samples taken. J. 506 W. 10 th Raven Hall Units 13, 14, 15, 28, 29, 30 Two story building 3 first floor units with 4 bedrooms, a commons area and a common bathroom built in 1972. 3 second floor units with 4 bedrooms and a bathroom built in 1991. This building will be demolished. Asbestos Material: Friable Approximately 3,300 sf of ceiling texture throughout 1st floor.

K. 508 W. 10 th Raven Hall Manger s Apartment/Laundry Single story building The apartment at the West end of the building was built in 1972 and does contain a basement. The laundry room was added to the East end of the building in 1983. There is no basement under this portion of the building. This building will be demolished. Asbestos Material: Friable Approximately 1,400 sf of ceiling texture. L. 501 W 10 th Student Union The East end of the Student Union building was constructed in 1969. We are seeking testing of what appears to be sprayed insulation on the underside overhang leading to the basement. Asbestos Material: Exterior fireproofing on concrete at the lower level North side of the Student Union tested negative for asbestos. M. 520 W. 9 th Vacant House Two Story, 1,672 sq. ft. built in 1920 Detached garage, 440 sq. ft. built in 1950 These buildings will be demolished. Asbestos Material: Friable Approximately 6 sf of asbestos paper insulation on a 2nd floor wall heater Category II Non Friable Approximately 7,750 sf of wall and ceiling plaster Approximately 2,450 sf of transite siding Approximately 20 windows with asbestos window glaze Each contractor is responsible to determine for themselves the accuracy of the volume estimates of asbestos containing material in the report. In order to expedite the demolition process to immediately follow remediation, CCC requests required documentation of the asbestos remediation be sent to the Kansas Department of Health and Environment as each building is complete. CCC also requests the buildings be remediated in the following order: Building: J, K, B, C, D, E, F, G, A, M CCC will not recycle any of the concrete/structure materials from those buildings to be demolished. CCC will provide access to electricity and water for this project.

The selected contractor will be responsible for the safety and security of the work site and must follow all Federal and/or State laws and regulations and provide documentation of proper disposal of the asbestos containing materials to CCC. 2. SCHEDULE Issue Request for Proposals (RFP) 1/14/16 Deadline for receipt of written questions and requests for addenda 1/26/16 CCC responds to questions and/or addenda issued 1/29/16 Proposals Due 2:00 pm Central Time 2/5/16 Tentative Notice to Proceed 2/16/16 3. GENERAL PROPOSAL CONDITIONS A. The firm, organization or company must be a licensed asbestos abatement contractor as per the requirements of the State of Kansas. B. Responses to the RFP shall be made according to the instructions contained herein. Failure to adhere to instructions may be cause for rejection of any proposal. C. Proposers understand and agree that submission of a proposal will constitute acknowledgment and acceptance of, and willingness to comply with, all the terms, conditions, and criteria contained in this RFP, except as otherwise specified in the proposal. Any and all parts of the submitted proposal may become part of any subsequent contract between the selected proposer and CCC. D. False, misleading, incomplete, or unresponsive statements in connection with a proposal may be sufficient cause for rejection of the proposal. The evaluation and determination of the fulfillment of the above requirement will be CCC s responsibility, and its judgment shall be final. E. Proposals shall provide a straightforward, concise delineation of the proposer s capability to satisfy the requirements of the RFP. Each proposal shall be submitted in the requested format and provide all pertinent information, including but not limited to information relating to capability, experience, financial resources, and other information as specified in Section 5 and otherwise required in this RFP. Each proposal shall be signed in ink by a duly authorized officer of the company.

4. SUBMISSION OF PROPOSALS A. Proposals in response to this RFP shall be considered received at the time actually received by the addressee. Written proposals must be received by CCC at its office address listed in paragraph A of this section by 2:00 pm, Central Time, February 5, 2016. C. Proposals received after this specified date and time shall be considered late and shall not be considered for evaluation. D. Each proposal shall be in the form specified in this RFP, and in a sealed envelope with outside markings stating: CCC Asbestos Abatement Proposal. 5. REQUIRED ELEMENTS OF PROPOSALS A. Cover Letter. Each proposer shall submit a short cover letter including the name and address of the organization submitting the proposal; and the name, address and telephone number of the contact person who will be authorized to make representations for the organization. B. Experience. Qualifying Experience. Brief history of the firm including any fields of expertise, previous experience with jobs of similar scope. List a minimum of three (3) recent asbestos abatement projects similar in scope (if possible). Litigation. Provide a list of any litigation in which the Firm, Managing Principal and/or Abatement Professional(s) is named a party. C. Proposed Work Plan/Schedule. Include completion date D. Certification and/or Licenses. Proposer must possess and submit with this bid all valid certification and/or licenses required by federal and state laws at the time of submittal and for the length of the project. E. Certificate of Liability Insurance. Proposer must submit proof of liability insurance in an amount of at least $1M. If proposer is awarded a contract for this project, a certificate of liability insurance listing CCC as an additional insured will be required. F. Bid Submittal Form. (Included in this RFP)

6. CLARIFICATION AND ADDENDA PROCESS A. A proposer may submit to CCC a written request for an interpretation or clarification of, or an addenda to, this RFP. Any such request must be received by CCC no later than 5:00 pm, Central Time, January 26, 2016. The request must be emailed to morris.jeff@coffeyville.edu. The subject line of all emails should be CCC Asbestos Abatement RFP. Proposers should specifically note CCC prefers questions be submitted by email. B. CCC will review and prepare a written response to each request made by a proposer pursuant to this section. CCC written determination will be emailed by January 29, 2016, unless circumstances require a later date. C. CCC reserves the right to make modifications or addenda to this RFP. If CCC determines it is appropriate to revise any portion of this RFP, either at the request of a proposer or upon CCC s own initiative, CCC will issue, and make available to all prospective proposers via CCC s website at www.coffeyville.edu. D. CCC is not bound by any oral interpretations, clarifications, or changes made to this RFP by any CCC employee or consultant. Any clarification or change to the RFP must be provided in writing pursuant to this section. 7. COST OF PROPOSAL Any costs incurred by proposers in responding to this RFP shall be the proposer s sole expense and will not be reimbursed by CCC. 8. CANCELLATION OF REQUEST FOR PROPOSALS CCC reserves the right in its discretion to cancel this RFP in whole or in part. 9. PROPOSAL REJECTION CCC reserves the right to accept or reject any and all proposals submitted in response to the RFP, or refuse to enter into any contract resulting from any proposal submitted, without expense to CCC. 10. PROPOSAL WITHDRAWAL Bids will be evaluated promptly after opening and a bid tabulation summary will be provided upon request. Bid results will not be given over the telephone. No bid may be withdrawn for a period of thirty (30) calendar days after 2/5/16. 11. AWARD Award of bid shall be made to the lowest responsible bidder meeting the specifications set forth herein. In addition to the quoted price, the following criteria may be used in the award: past experience, and service provided by the proposer, quality of work, completion date, and other factors deemed relevant by CCC.

Mr. Morris: Per your request, we have completed an asbestos survey of 13 structures located at Coffeyville Community College. This survey is required for identification of asbestos containing materials in buildings that are to be renovated or demolished. Samples were taken of all accessible suspect asbestos containing materials throughout the facility.
