2 INVITATION TO BID The Town of Sherman, on behalf of the Sherman Public School system, invites quotations for removal of approximately 1327 square feet of asbestos-containing mastic and associated floor covering from The Sherman School, 2 Route 37 East, Sherman, Connecticut. 3 INSTRUCTIONS TO BIDDERS 1. CONDITIONS A. No mandatory pre-proposal meeting but you should visit the site. Call the school at (860) to arrange for a visit. B. Bid bond is not required. C. Proposals are due by 12:00 noon on Tuesday, June 24, Sealed proposals must be delivered to Andrea O Connor, First Selectman, Town of Sherman, 9 Route 39 North, P.O. Box 39, Sherman, CT D. Proposals must include: lump sum cost. statement of assurance that the bidder will comply with the project schedule. E. Do not include Federal Excise Taxes nor State of Connecticut Sales Taxes. The Sherman School and the Town of Sherman are exempt. 2. AWARD OF CONTRACT A. The Owner reserves the right to award the contract to the bidder offering the best value in the interest of the Owner, but not necessarily award the contract to the lowest dollar amount. B. The Owner reserves the right to reject any or all proposals and to waive defects or informalities in any bid id it is deemed to be in the best interests of the Owner. C. All completed work and material must be inspected and approved by the Town of Sherman for approval of payment. 3. ADDITIONAL CONDITIONS A. The execution of a contract binds the Contractor to all applicable State labor laws and regulations. All such standards, laws, and regulations shall be binding to the same extent as if they were copied at length herein. B. Each Contractor shall be subject to compliance with all applicable federal, state, and local statutes, ordinances, and regulations relating to discrimination in employment. It shall be the responsibility of the Contractor to be familiar with and knowledgeable about these laws. Instructions to Bidders page 1 of 1

4 CONTRACT CONDITIONS 1. INSURANCE A. The successful bidder must provide the Town of Sherman with Certificates of Insurance providing coverage with an insurance carrier licensed to write insurance in the State of Connecticut. B. The following insurance policies and coverage are required: General Liability $1,000,000 Automobile Liability $1,000,000/$500,000 Workers' Compensation Statutory/$100,000 Owner's Protective Liability $1,000,000 C. The Town of Sherman and the Sherman Public School system must be named as an additional insureds on all policies except Worker s Compensation. D. Liability coverage must include work with asbestos. 2. QUALIFICATIONS A. The Contractor must be licensed in the State of Connecticut as an asbestos abatement contractor. B. All workers must be certified by the State of Connecticut. At least one worker on site must be certified at the Supervisor level. 3. WORK SCHEDULE A. The Contractor shall start work no earlier than Saturday, August 2, All work must be completed no later than Friday, August 15, The work area must be ready for reoccupancy air testing no later than 12:00 noon, Thursday, August 14, Access to the school will be provided as needed. Visual inspection and TEM clearance air sampling will be performed by a licensed asbestos project monitor from Brooks Environmental Consulting. 4. COORDINATION OF WORK A. Work schedules must be coordinated through and approved by the Owner. B. The Owner is the Town of Sherman, 9 Route 39 North, P.O. 5 ABATEMENT SPECIFICATIONS 1. SCOPE OF WORK A. Contractors are to supply price quotations for removal and disposal of approximately 1327 square feet black mastic and yellow adhesive and associated blue pliable linoleum with gray backing from the lobby, hallway, and vestibule as shown on sketch 1. Quotations are to be LUMP SUM AMOUNTS which include all costs for insurance, abatement work, fees and permits, disposal. and documentation. Removal of mastic and adhesive shall be by shot blast and surface grinder methods. Solvent will not be allowed. B. Contractors are expected to inspect the site to determine the exact scope of work and the prevailing conditions. 2. WORK PRACTICES A. Meet or exceed the requirements of the Connecticut Department of Public Health contained in RCSA 19a-332a-1 through 19a-332a-23 and those of OSHA contained in 29 CFR which include: 1. Code conforming negative pressure containment system. 2. Contiguous worker decontamination unit. 3. Approved waste disposal site. B. Perform work in the following order: 1. Construction of containment system by Contractor. 2. Inspection by Owner s Consultant. 3. Abatement work by Contractor. 4. Work area clean-up by Contractor. 5. Visual inspection by Owner s Consultant. 6. Air sampling by Owner s Consultant. 7. Final clean-up by Contractor. 8. Disposal of waste by Contractor. C. The Owner s asbestos project designer will obtain an Alternative Work Practices approval for use of a single layer of 4-mil polyethylene on the walls. 3. SUBMITTALS A. Certificate of Insurance should be sent to the Owner. All other submittals should be sent to the Owner s Consultant. B. The following are required before work commences: 1. Certification of insurance. 2. Copy of Contractor s license. 3. Copy of each worker s and each supervisor s Connecticut certificate, current refresher training certificate, most recent medical exam, and most recent respirator fit test.
