1 To: All Planholders From: Tormod Hellwig, llc Addendum Date: 1 April 2016 DENTAL CLINIC REMODEL for PORT GAMBLE S'KLALLAM TRIBE ADDENDUM #3 This Addendum shall hereby be and become a part of the Contract Documents. The following clarifications, amendments, additions, revisions, changes, and modifications change the original Contract Documents only in the amount and to the extent hereinafter specified in this Addendum. All other terms, specifications, and conditions remain the same. The bidder is responsible for transmitting this information to all affected subcontractors and suppliers prior to the opening of bids. Bidder shall acknowledge receipt of this Addendum in his proposal or bid. CHANGES AND CLARIFICATIONS TO SPECIFICATIONS ITEM NO. 1: SECTION BID PROPOSAL Replace: the Bid Proposal Form with the one attached showing the correct time for completion. ITEM NO. 2: SECTION SUPPLEMENTARY GENERAL CONDITIONS ARTICLE 11 INSURANCE AND BONDS Add: Add to the following: Conditions for bonds: Payment and performance bonds for 100% of the Contract Award Amount shall be furnished for the Work, using the Payment Bond and Performance Bond form published by and available from the American Institute of Architects (AIA) form A312. Prior to execution of a Change Order that, cumulatively with previous Change Orders, increases the Contract Award Amount by 15% or more, the Contractor shall provide either new payment and performance bonds for the revised Contract Sum, or riders to the existing payment and performance bonds increasing the amount of the bonds. ITEM NO. 3: Add: SECTION EXISTING HAZARDOUS MATERIAL INFORMATION PART 1 - GENERAL 1.01 EXISTING HAZARDOUS MATERIAL INFORMATION A. This document with its referenced attachments is part of the Procurement and Contracting Requirements for Project. They provide Owner's information for Bidder's convenience and are intended to supplement rather than serve in lieu of Bidder's own investigations. They are made available for Bidder's convenience and information, but are not a warranty of existing conditions. This Document and its attachments are not part of the Contract Documents. An Asbestos and lead Survey for the Dental Clinic was contracted by the Owner and was performed by Jose Technologies, PO Box 191, Keyport WA 98345, (360) The resultant study, dated March 30, 2016, consisting of 3 pages is attached. END OF SECTION ITEM NO. 4: SECTION ALUMINUM-FRAMED ENTRANCES AND STOREFRONTS Delete: 3.3 FIELD QUALITY CONTROL 1. Air Infiltration: Areas shall be tested for air leakage of 1.5 times the rate specified for laboratory testing under Part 1 "Performance Requirements" Article, but not more than 0.09 cfm/sq. ft., of fixed wall area when tested according to ASTM E 783 at a minimum static-air-pressure difference of 6.24 lbf/sq. ft. 2. Water Penetration: Areas shall be tested according to ASTM E 1105 at a minimum uniform and cyclic static-air-pressure difference of 0.67 times the static-air-pressure difference specified for laboratory testing under Part 1 "Performance Requirements" Article, but not less than 4.18 lbf/sq. ft., and shall not evidence water penetration. B. Repair or remove work where test results and inspections indicate that it does not comply with specified requirements. C. Additional testing and inspecting, at Contractor's expense, will be performed to determine compliance of replaced or additional work with specified requirements. I was asked by Joe to do both an Asbestos and Lead test in the wing of the medical building at the Tribal Center on Little Boston Rd. I took one lead sample in the storage room midway through the clinic. This sample was to determine if there was any lead in the paint of the center. The sample was sent to the EMSL Labs in Seattle and their report is attached to this report. I also checked three different places for Asbestos in the tile floor and the ceiling tiles. The first floor sample was taken in the dental treatment, in the hall way ceiling tile and in the corner of the storage area in the center of the building. These three samples I also sent to the ESML lab in Seattle for analysis. I received the lab reports that I will attach and they show no asbestos or lead in the facility. With the different areas tested I am confident that the Dental Clinic is free of both Asbestos and Lead. If you have any questions please feel free to give me a call.
