INTRODUCTION Apex Companies, LLC was authorized to proceed with an asbestos survey and inspection of the City of Largo PSTA Building located at th Street North in Largo, Florida. The purpose of the inspection was as a pre-demolition survey for the identification of suspect Asbestos-Containing Materials (ACM), as required by the U.S. Environmental Protection Agency through the National Emissions Standards for Hazardous Air Pollutants (NESHAP), and by the State of Florida. The survey was performed of the facility as specified by Mr. Tom Thibault (Collman & Karsky Architects). II. DEFINITIONS AND ABBREVIATIONS The following definitions and abbreviations are appropriate as they pertain to this report. Asbestos: The asbestiform varieties of serpentine (chrysotile), riebeckite (crocidolite), cummingtonite-grunite, amphibole, anthophyllite, and actinolite-tremolite. NESHAP: National Emission Standards for Hazardous Air Pollutants, 40 CRF 61, Subpart M. ACM: Asbestos-Containing Materials. Any material that contains in excess of 1% of the asbestiform minerals, by volume. PLM: Polarized Light Microscopy. The recommended analytical method for determining the presence and content of asbestiform minerals in a material. Friable: A material that when dry, can be crumbled, pulverized, or reduced to powder through hand pressure. O&M Program: Operations and Maintenance Program. An O&M Program defines special work practices and notifications to minimize exposure of all building occupants to asbestos fibers. The work practices include: (1) maintaining ACM in good condition; (2) ensuring proper cleanup of asbestos fibers previously released; (3) preventing further release of asbestos fibers; and (4) monitoring the condition of ACM. Homogeneous Sampling Area (HSA): An area of material that is similar in color, texture, age, etc. Category I Nonfriable Asbestos Containing Material: Asbestos-containing packings, gaskets, resilient floor covering, and asphalt roofing products determined to be ACM. 1
5 City of Largo PSTA Building Pre-Demolition Asbestos Survey th Street North Apex Project #: Largo, Pinellas County, Florida May 17, 2013 Category II Nonfriable Asbestos Containing Material: Any material, excluding Category I nonfriable ACM, that has been found to be ACM and when dry cannot be crumbled, pulverized, or reduced to powder by hand pressure. Regulated Asbestos Containing Material (RACM): means a.) Friable asbestos material, b.) Category I nonfriable ACM that has become friable, c.) Category I Nonfriable ACM that will be or has been subject to sanding, grinding, cutting, or abrading or d.) Category II nonfriable ACM that has a high probability of becoming or has become crumbled, pulverized, or reduced to powder by the forces expected to act on the material in the course of demolition. III. SCOPE OF WORK The Scope of Work for this project was defined and agreed upon as follows: Visual inspection of building materials which are suspected of potentially being asbestos-containing materials. Collection of bulk samples of suspect friable and non-friable ACM. Analysis of bulk samples by Polarized Light Microscopy (PLM). Identification of locations of asbestos-containing materials. Preparation and submittal of Report of Findings, including recommendation for further actions if warranted. Inspection/survey performed by Accredited Asbestos Inspector under the direction and review of a Florida Licensed Asbestos Consultant, as required by Florida Statute (FS) 469. IV. SITE INSPECTION The subject survey area consists of the City of Largo PSTA Building located at th Street North in Largo, Florida. The survey was performed of the entire facility as specified by Mr. Tom Thibault (Collman & Karsky Architects). The commercial building contains approximately 3,904 square feet and was originally constructed in The last tenant to occupy the building was the City of Largo PSTA. The one-story building consists of a lobby area, several offices, restrooms and break room. 2
6 City of Largo PSTA Building Pre-Demolition Asbestos Survey th Street North Apex Project #: Largo, Pinellas County, Florida May 17, 2013 On May 7, 2013, an Accredited Asbestos Inspector (Jeffrey A. Borgmeyer) performed a visual inspection of the specified areas, and identified Homogeneous Areas of building materials that were suspected of containing asbestos. A total of thirteen (13) Homogeneous Areas were identified in various locations during the performance of the inspection. SAMPLING ACTIVITIES On May 7, 2013, the Inspector collected bulk samples from several locations in the established Homogeneous Sampling Areas identified as potentially containing asbestos. A Homogeneous Sampling Area is defined as an area of material of the same type, texture, and color, and installed or applied in the same general time period. A total of forty-one (41) bulk samples from random locations, were collected from suspect materials. These samples were collected in labeled, air-tight containers and sent to an asbestos analytical laboratory, accompanied by a properly completed Chain-of-Custody form, for analysis using Polarized Light Microscopy (PLM), a recommended method of asbestos identification for bulk samples. Samples were not collected from materials that were readily identifiable as non-asbestos (ie: wood, fiberglass, foam rubber, etc.). The identified Homogeneous Sampling Areas and materials are as follows: 3

ANALYTICAL RESULTS The bulk samples were submitted to Arrowhead Technologies, LLC on May 8, 2013 for analysis using Polarized Light Microscopy (PLM). A total of two (2) analyzed samples yielded a positive result for asbestos. The Environmental Protection Agency NESHAP (40CFR Part 61) states that "if the asbestos content is less than ten percent as determined by a method other than Point counting by Polarized Light Microscopy (PLM), verify the asbestos content by Point Counting by PLM." Analysis yielded the following results: 4

HAZARD ASSESSMENT The method used for the determination of Hazard Potential is through the use of the "Hazard Ranking Decision Tree" as outlined by the U.S. Environmental Protection Agency (USEPA). The ranking of hazard potential is based on the current condition of the material, and the potential for future disturbance based on ( a ) Potential for contact, ( b ) Influence of vibration, and ( c ) Potential for air erosion. Each material is then ranked on a scale of 1-7, with 7 being the highest hazard. Nonfriable materials are assigned a Hazard Ranking of zero (0). VIII. RESPONSE ACTION RECOMMENDATIONS HSA: 4 MATERIAL: Roof Pitch Tar HAZARD RANKING: 0 NESHAP CATEGORY: Category I Nonfriable (unregulated) RESPONSE ACTION: Asbestos containing roof pitch tar material was established to be present at the seam connecting the roof of the structure with the covered walkway that extends to the south. The pitch pan material was observed to be in good condition at the time of the inspection. Due to the material s condition, it is categorized as Category I Nonfriable ACM, and is thus unregulated in its current condition. The identified materials may remain in place during demolition, providing that wet demolition methods are employed 5

HSA: 6 MATERIAL: Mirror Mastic HAZARD RANKING: Nonfriable NESHAP CATEGORY: Category II Nonfriable RESPONSE ACTION: The mirror mastic located behind the mirrors within the bathrooms has been found to be asbestos containing. The material was observed to be in good condition at the time of the inspection. This material is categorized as Category II Nonfriable ACM, and is thus unregulated in its current condition. This material must be removed prior to the demolition process by a licensed asbestos contractor pursuant to Florida Statute 469. IX. CONCLUSIONS AND RECOMMENDATIONS On May 7, 2013, an asbestos survey was performed of the former City of Largo PSTA Building located at th Street North in Largo, Florida. Two (2) analyzed samples yielded positive results for asbestos content. The following materials were found to be asbestos containing: 1. Roof Pitch Tar 2. Mirror Mastic The roofing material is categorized as a Category I Nonfriable ACM, and is thus unregulated in its current condition. The identified asbestos containing material may remain in place during demolition, providing that wet demolition methods are employed and all mastic material is removed for proper offsite disposal in a landfill which accepts asbestos waste. The mirror mastic found within the building is categorized by NESHAP as Category II Nonfriable ACM, and as such is unregulated in its current condition. The material must be removed prior to demolition by a licensed asbestos abatement contractor pursuant to Florida Statute 469. According to the November 20, 1990 revisions to the National Emission Standards for Hazardous Air Pollutants (NESHAP); "Regulated Asbestos Containing Materials (RACM) 6
10 City of Largo PSTA Building Pre-Demolition Asbestos Survey th Street North Apex Project #: Largo, Pinellas County, Florida May 17, 2013 need not be removed before demolition if: (I) It is Category I Nonfriable ACM that is not in poor condition and is not friable, (ii) it is on a facility component that is encased in concrete or other similarly hard material and is adequately wet whenever exposed during demolition, (iii) it was not accessible for testing and was, therefore, not discovered until after demolition began and as a result of the demolition cannot be safely removed...(iv) they are a Category II nonfriable ACM and the probability is low that the materials will become crumbled, pulverized, or reduced to powder during demolition." Further, Category I Nonfriable ACM as defined includes; "asphalt roofing products, and resilient floor covering (including vinyl floor tile)." The EPA also commented on the fact that some nonfriable materials may remain during the demolition process by stating materials such as resilient floor coverings, packings, and gaskets would rarely, if ever need to be removed because, even when broken or damaged, they would not release significant amounts of asbestos fibers. Based upon the findings, the EPA states that the floor tile may remain in place. However, Pinellas County requires removal of the floor covering prior to demolition. Some precautions must be taken before, during and after the demolition due to the ACM, including: 1. Proper notification must be filed with the appropriate regulatory authority. 2. The landfill must be notified of the presence of nonfriable asbestos containing materials and must accept the material for disposal. 3. The building debris must be kept wet at all times during demolition, and loading. 4. The debris must be completely removed from the site immediately upon demolition. 5. A demolition notification must be submitted to the local regulatory authorities. Please be advised that a copy of this inspection report must be on site during the demolition of the building, in the event of a regulatory inspection. 7

STATEMENT OF LABORATORY ACCREDITATION The samples were analyzed in general accordance with the procedures outlined in the Method for the Determination of Asbestos in Bulk Building Materials, EPA/600/R-93/116 or the U.S. Environmental Protection Agency method, under AHERA, for the analysis of asbestos in building materials by polarized light microscopy. The results of each bulk sample relate only to the material tested and the results shall not be used to claim product endorsement by NVLAP or any agency of the U.S. Government. Floor tile and other resinously bound materials, when analyzed by the EPA method, may yield false negative results because of limitations in separating closely bound fibers and in detecting fibers of small length and diameter. When a definitive result is required, Arrowhead recommends utilizing alternative methods of identification, including Transmission Electron Microscopy.
