3 our people and our passion in every project 1.0 Introduction The New York State Office of General Services (NYSOGS) retained Bergmann Associates (Bergmann) to conduct a Limited Asbestos and Lead Survey in Building 44 of the Willard Drug Treatment Center (Willard DTC) located in Willard, New York. The building was occupied and accessible at the time of this survey, which was performed to facilitate bathroom renovations throughout. 2.0 Asbestos Survey and Sampling Methodology One site visit to collect bulk samples and to estimate quantities of suspect ACM was conducted by Bergmann personnel on September 11, The site visit and bulk sampling were performed by Michael Carpenter, a New York State Department of Labor certified Asbestos Inspector. Bergmann personnel conducted the Asbestos Survey in accordance with applicable Federal and State regulations. These regulations include: National Emission Standards for Hazardous Air Pollutants (NESHAPS). New York State Department of Labor Industrial Code Rule 56 (ICR-56) New York State Department of Health Environmental Laboratory Approval Program (ELAP) National Voluntary Laboratory Accreditation Program (NVLAP) Occupational and Health Administration (OSHA) Regulation 29 CFR and 29 CFR Samples were collected in a manner to minimize damage to the surrounding area, to minimize potential release of material and in a fashion to ensure the safety of the collection personnel, building occupants and any building visitors. The Bergmann Asbestos License, laboratory certifications and certifications of the persons that conducted the bulk sampling are provided in Appendix A. A figure of the sample locations is provided in Appendix B. Procedures for bulk sampling and measurements included: Sample collection area was cleaned of any debris or non-acm material and was wetted with amended water as needed. A minimal amount of material was collected and placed into discrete plastic sample bags. Sample containers were labeled with a site-specific code that reflected location and sample number. The representative bulk samples of suspect materials were collected for laboratory analysis. The samples from the September 2013 survey were transported via Chain-of-Custody protocol to AmeriSci, a New York State Department of Health Environmental Laboratory Approval Limited Asbestos & Lead Survey 1 October 2013 Building 44, Willard DTC

4 our people and our passion in every project Program (NYSDOH-ELAP) and National Voluntary Accreditation Program (NVLAP) certified asbestos testing laboratory. Copies of the Chain-of-Custody forms are provided in Appendix C of this report. 3.0 ACM Laboratory Analysis and Results All samples, including both friable and non-friable, were initially analyzed for the presence of asbestos via Polarized Light Microscopy (PLM). For friable material and material determined by gravimetric analysis to be greater than 1% asbestos no further analysis was required. PLM analysis is typically sufficient on samples of friable insulation, pipe wrap, spray-on fire proofing, drywall and plaster. Some samples were determined to be non-friable organically bound (NOB) material. The results on representative samples of NOB material that were determined to be non-acm via initial PLM analysis were confirmed via Transmission Electron Microscopy (TEM). The TEM analysis was performed in accordance with NYSDOH-ELAP regulations to confirm the presence or absence of asbestos from NOB material. TEM confirmation is typical of materials such as electrical wiring, vinyl floor tile, adhesive mastics and roofing materials including flashing, caulk, roofing tar and asphalt shingles. All laboratory analysis was performed in accordance with NVLAP and NYSDOH ELAP regulations. A copy of the laboratory analysis on the bulk samples is provided in Appendix C of this report. 4.0 Materials Sampled & Analyzed The following table identifies the material sampled which was determined to be ACM: Table 1 - Materials Sampled & Analyzed Sample ID Material Sampled Asbestos? The asbestos-containing pipe insulation is present in the north wing of the basement resting on top of piping as it routs through the narrow passage. 5 our people and our passion in every project 5.0 Lead Based Paint Evaluation Concurrent with the September 2013 bulk sampling of suspect ACM, Bergmann personnel inspected painted surfaces for suspected lead-based paint. A bulk sample of the typical interior painted surfaces was collected for laboratory analysis to determine the presence or absence of lead-based paint. The paint chip sample, chain-of-custody form and laboratory analysis for lead are provided in Appendix C. The location of the representative bulk paint chip sample is depicted on the sample location plan. The sample was transported via Chain-of-Custody protocol to AmeriSci, a USEPA Lead-Based Paint Activities Certification and New York State Department of Health Environmental Laboratory Approval Program (NYSDOH-ELAP) certified lead testing laboratory for analysis. The Federal Department of Housing and Urban Development (HUD) has established guidelines for the identification of lead based paint. Paint containing a concentration equal to or greater than 0.5% by weight (5,000 parts per million or 5,000 ppm) is to be considered lead-based paint. Determination of lead in solid wastes at a hazardous waste level is dependent upon concentrations in a Toxicity Characteristic Leaching Procedure (TCLP) liquid extract. A TCLP Lead concentration at or exceeding 5.0 milligrams per liter (5.0 mg/l) is considered to be a hazardous waste under Federal and State hazardous waste regulations. The laboratory analyses performed on the representative paint chip sample collected in September 2013 did indicate the presence of lead-based paint. Table 2 - Lead Based Paint Results Location Item Material % by Weight Condition Interior Typical Interior Paint % Fair Worker protection for compliance with applicable OSHA regulations for exposure to lead are required when lead based paint is identified or suspected. This may require appropriate training, use of respirators, personal protective equipment and medical clearance and monitoring. 6.0 Limitations This asbestos and lead survey investigated the presence of accessible and/or exposed suspect ACMs and lead based paint materials. Bergmann did not perform destructive testing within interior cavities, spaces that may exist in inaccessible spaces. Additional suspect materials may remain hidden in walls, hidden in or under the roofing system, in columns or within decking. Should suspect material be identified, the material should be sampled and analyze to confirm or deny the presence of asbestos. Limited Asbestos & Lead Survey 3 October 2013 Building 44, Willard DTC

6 our people and our passion in every project 7.0 Asbestos Containing Materials Report Notification Requirements In accordance with New York State Asbestos Regulations under 12 NYCRR Part 56 (Industrial Code Rule 56) Subpart (g), one copy of the results of the building/structure asbestos survey shall be immediately transmitted by the building/structure owner as follows: The completed asbestos survey for controlled demolition (as per Subpart ) or pre-demolition asbestos projects shall be submitted to the appropriate Asbestos Control Bureau District office. The nearest office is the Syracuse District office: 450 South Salina Street, Syracuse, NY 13202, phone - (315) fax - (315) One copy of the completed ACM survey shall be sent by the owner or their agent to the local government entity charged with issuing a permit for demolition, renovation, remodeling or repair work under applicable State or local laws. The completed asbestos survey shall be kept at the construction site throughout the duration of the demolition, renovation, remodeling or repair work.
