Instructions for Registering on MyHealth In order to register, you (patient) will need to enter personal identification information contained in the MyHealth Patient Web Portal letter provided to you by their clinic. If you do not have this information you will need to contact your appropriate clinic to obtain this information. Creating your Secure MyHealth Account 1. Log onto the https://securemail.ht.msu.edu/portal website. 2. Select your clinic. If your clinic currently offers web portal access, it will appear as an active link in the web box labeled Select Your Clinic along the left side of the screen on the main page. Click on the desired clinic. For this example we will select the Internal Medicine clinic. 3. Register. Click the Register link at the upper left of the page. 4. Review the MyHealth Website Use: Terms and Conditions screen. To complete the registration process, all users must accept the Terms and Agreement. To accept the Terms and Conditions, click the I accept button within the page. If you do not accept the Terms and Conditions you will not be able to register. 5. Complete all of the fields on this page (see detailed instructions for each field below screen shot). ACCOUNT INFORMATION User ID - enter the desired name you wish to use into this field using one continuous string of letters and/or numbers (no spaces are allowed). This will be your MyHealth account name. First Name: Enter your first name in this field. Prepared by Michigan State University EMR Page 2 of 6

Last Name: Enter your last name in this field. Email Address enter your personal e-mail address in this field. If you do not have an email address you will need to get one from an online website such as www.yahoo.com or www.hotmail.com. All users are required to enter an email address in this field. Note when registering children, each child will need their own e-mail address. If a child does not have an e-mail account, parents can use the following format firstname.lastname@nomail.com. Alternate Notification Email Address - Message notifications will be sent to this e-mail address instead of the e-mail above. This field is where a parent would enter their e-mail address so all notification messages will be directed to this registered e-mail account. Password Information New Password enter the password to be used for your secure email account. ALL PASSWORDS MUST BE A COMBINATION OF LETTERS AND NUMBERS AND BE BETWEEN 6 AND 15 CHARACTERS LONG Re-enter Password re-enter your password in this field for confirmation. Secret Question 1 and Secret Question 2. Note: These questions are used in case you forget your password or when resetting your password. Select a secret question from the Drop Down list. For enhanced security purposes you must have 2 secret questions. Both your secret questions must be a minimum of 4 characters. They are not case sensitive. 6. Print a copy of this page and store in a secure place for future reference before you click the Save button at the bottom of the window. You can print by going to File>Print or clicking the from the action buttons on Internet Explorer. Note: Once you click the Save button, you cannot go back to this page to print. DO NOT hit the BACK button of your Browser window during this process. Doing so will compromise your web portal account. 7. Click the Save button at the bottom of the window 8. Patient Verification Once you click Save you will be directed to the Patient Verification page where you will be informed that you have successfully created a new website account. You will now need to attach your account to your specific clinic. To do so, click the I have been seen here by a physician before button. Click the Next button to continue. 9. Identity Verification Enter your personal identification information exactly as it appears on your Registration Instructions: MyHealth Patient Web Portal letter that was provided to you by your clinic. You must complete each field listed. This information must match our system exactly, or it can not verify your identity. Click the Verify button after completing this page. Note: Parents registering their children s account within the Okemos Pediatric clinic will also enter a PIN number field. Please pay particular attention to the month/date/year format for entering birth date. 10.Once your identity has been verified, the following message, will display. You have completed the registration process and may 1) log out, 2) complete a questionnaire, or 3) send messages depending on the services available by your clinic. The services available by your clinic are listed in the clinic toolbar. In the example below, the Internal Medicine toolbar is displayed. The Internal Medicine clinic participates in all the services highlighted with the red bar. receive an error message indicating your account was not created. Please retry beginning at #1 or contact the EMR office at 517.353.0817 or contact the EMR office at 517.353.0817 and give them the information on the Error Message screen. ADDENDUM If you are seen as a patient in more than one clinic, before you will be able to send messages or complete patient questionnaires you will need to verify your identity for that clinic (i.e. surgery, cardiology). Note: Only those clinics listed in the Select Your Clinic box are currently participating in MyHealth. Additional clinics will be added over time. To verify your identity for another clinic 1. Go back to Step 1 of this document. 2. At Step 2 select the desired clinic. 3. Complete the remaining steps of registration and identity validation process just as you did for the first clinic.
