1 Patient Portal How to Begin: If you provided an address when you registered as a patient, you can expect an after your discharge that will be sent directly from CPSI, our Electronic Health Record software vendor, from the address: donotreply@cpsi.com. The initial invitation will provide step-by-step instructions to set up your personal Username & Password for the Patient Portal. If you have already set up access to your Patient Portal and simply need to locate the website to log in, you can visit our website at click the Patients & Visitors menu, and then choose Patient Portal. Alternatively, you can type the Patient Portal website address directly into your internet browser address field: Accessing the Patient Portal 1. After selecting the hyperlink in the , you will be taken to the New User Registration Screen. You must enter your first and last name exactly how it was registered during your visit, your date of birth, and create a user name and password. You must select the "I verify." checkbox and click the REGISTER button: Enter first and last name, date of birth and create a user name and password. 2 2. You will then be prompted to set up security questions and must answer ALL of them. These will be used in the event of forgetting your password. Once the security questions are answered, click Submit: You can choose each Security Question by clicking the drop down arrow. You must answer all 3 Security Questions. Submit when finished. 3. You will be taken to a Patient Portal confirmation screen. From this screen, you may select Main Menu: Main Menu to proceed.

3 4. From the Main Menu, you can select the action you would like to take, either View Clinical Information or Pay your bill: View Clinical Information or Pay your bill. 6 To Download the Summary of Care: 1. To download the Summary of Care, click the Download link under the patient name: the Download link to download the Summary of Care. 2. A dialog box will appear to Open, Save or Save As. Choose the appropriate action to Open or Save the file. 3. After the action has been selected, the Patient Summary will be saved as an HTML Document. The Patient Summary will display once selected: the HTML document to display the downloaded Summary of Care.
