To develop effective and assertive treatment strategies for those people who have a serious mental illness, which is complicated by co-existing substance misuse. To improve integration between mental health and drug and alcohol services and achieve positive client outcomes. To train and support mental health and drug and alcohol professionals/support workers in dual diagnosis issues. To develop more flexible and tailored treatment approaches. Provide specialist mental health/alcohol and drug assessment and treatment services to referred clients using appropriate assessment and treatment procedures. Clients are prioritised and actioned appropriately and in a timely manner. Assessment and treatment of co-existing alcohol and drug issues and mental health disorders. Implementation of Harm Reduction model of treatment. Maintenance of a caseload of clients (as agreed with Dual Diagnosis Manager and Clinical Team Leader). Provision of ongoing assertive case support and A&D management of clients in consultation and conjunction with other services. Management/follow-up plans for clients are developed, actioned, monitored and evaluated. Clients are seen as often as clinically indicated. Clients management is regularly discussed and reviewed with clinical team. Appropriate recommendations and/or referral to others are actioned to ensure the best outcome for the client. Clients are supported in developing their own resource systems to meet their needs. Family/whanau and caregivers are encouraged to participate with client s treatment with client s consent. Provision of group facilitation or co-facilitation as appropriate and requested. Client care is documented and care is provided in accordance with service protocols and clinical pathways. Statistical information collected and recorded as per organisational and unit requirements. All clinical file audit requirements are met. Effective systems and processes are set up and maintained which enable liaison and consultation to occur. Relationships are proactively and effectively established with mental health and alcohol and drug providers that foster mutually cooperative collaboration and consultation. An up to date knowledge base of community resources and support services is maintained. Active participation in liaison and consultation with all services that currently and/or could potentially utilise the Dual Diagnosis Service. Provision of consultation/peer support to mental health and alcohol and drug professionals, as appropriate. Active involvement in updating and dissemination of relevant educational materials. All WDHB and CADS Clinical Policies, Protocols, Standard Operating Procedures, Practice Guidelines and Practice Standards are upheld. Adherers to the requirements of a competency based practicing certificate and engages in the WDHB Professional Development and Recognition Programme (PDRP) for Registered Nurses. Completion of all orientation and mandatory training requirements. All CADS Performance Development System requirements are fully met and accurate records are maintained. Supervision is undertaken from an identified Supervisor in accordance with CADS policy. Involvement in approved research and evaluation activities and projects as negotiated via the manager. Involvement in quality initiatives and continuous improvement. Educational and/or training courses, which ensure that the incumbent remains up to date in the fields of youth, mental health, drug and alcohol disorders and treatments are attended in consultation with the Manager and the Clinical Team Leader. PERSON SPECIFICATION POSITION TITLE: DUAL DIAGNOSIS CLINICIAN NURSE- CMDHB Focus Minimum Preferred Qualification NZ Registered Psychiatric or Comprehensive Nurse with Current Practicing Certificate. Relevant postgraduate health related tertiary qualification. Experience Reasonable period of experiences working in either the mental health or drug and alcohol fields. The ability to work both independently and as a member of a multidisciplinary team. Good communication and negotiating skills. Ability to develop and maintain effective networks. Ability to offer training and education to other health professionals and related groups. Demonstrate willingness and a commitment to understanding the implications for Maori health that are implicit in the Treaty of Waitangi.
