JOB DESCRIPTION Title of Post: Cardiac Pathway Nurse / MINAP Nurse Band/Grade: Band 6 Location: Professionally Accountable: Reports to: Hours: The Ulster Hospital and Trust Wide Clinical Manager Medical Specialties Clinical Manager Medical Specialties Full-Time 37.5 hours per week Job Purpose: To submit robust timely data to the MINAP data set on all patients that present to the Trust with acute coronary syndromes. This will include The Downe Hospital and Lagan Valley Hospital. To work as part of the cardiology team to carry out a programme of ongoing data collection and audits within Cardiology Services in Northern Ireland from first access to service to patient discharge. To work in conjunction with Trust / Cardiac Network / HSCB in the initiation and participation of audit and research in order to ensure the development of evidence based pathways of care in relation to diagnosis and management of patients with cardiac disease. To work in conjunction with the Trust management team to ensure the strategic aims of the Trust / Cardiac Network / HSCB are achieved and the standards of the service meet the performance indicators and raise awareness of areas of concern. 1. DUTIES AND RESPONSIBILITIES To submit timely high quality data to the MINAP data set for all patients presenting to the Trust with troponin positive chest pain. To ensure the maintenance of a high standard of data capture and information through continual evaluation and analysis of activity and outcomes. Page 1 of 9

Discussed emerging information with individual clinical teams responsible for the care in order to critically evaluate the appropriateness on an individual and Trust basis Sharing of good practices throughout the Trust area. Assessing and analysis any gaps in the clinical pathway from first access to services to discharge from hospital across all components including primary care, NIAS, A&E and Cardiology Departments. Systems may also vary across legacy Trusts. Work with the clinical teams responsible for care and help facilitate the subsequent service improvement required as the result of the analysis. Planning the data collection across the Legacy hospitals and coordinate with the various clinical teams / professionals involved in the care, including those working in other organisations. To assisting in the collation and analysis of other identified data at local level in order to ensure the completeness and the clinical quality of the information prior to sharing with other agencies/trusts. To work with clinical teams to ensure that chest pain patient assessment and treatment is developed in line with the current guidelines and standards. To complete relevant documentation and maintain accurate database for patients seen. Undertakes surveys or audits necessary to own work and participate in R & D and clinical trials appropriate to the role. To establish and maintain communication with people across the Trust sites, NIAS and Network to ensure appropriateness and timely provision of care. To communicate project objectives, effecting change and Improvements in care based on project findings to managerial level at all Legacy Hospitals. To prepare, present and deliver review reports at all levels internal and external to the Trusts. To identify areas of service redesign / improvement based on audit findings and discuss with relevant personnel both internal and external to the Trust as appropriate. To contribute to the development of the improvement programme policies and practices working with multiprofessional team members and other improvement programme teams and agencies, as required. Identifying data requirements and introducing systems to ensure collection of robust and clinically correct data is available for audit purposes. To organise and provide training programmes in accordance with the identified needs for the department as required. To assist with the in-house training of nursing staff with regard to specific aspects of cardiology patient pathways. 2. CLINICAL GOVERNANCE Promote best practice in health, safety and security. Contribute to quality improvement. Promote people s equality, diversity and rights. Participate in personal and departmental clinical audit. To maintain the Myocardial Infarction National Audit Program (MINAP) and the BCIS databases. 3. PROFESSIONAL To maintain a professional attitude at all times developing the role of the nurse in the best interest of the patient and profession in accordance with Nursing & Midwifery Council. To be aware of personal and professional development. To attend courses and conferences as recommended and approved by the Cardiology Services Manager. To be conversant and comply with the Trust policies and regulations and attend mandatory lectures as instructed. Environmental Cleaning Strategy The Trust s Environmental Cleaning Strategy, recognises the key principle that Cleanliness Matters. It is everyone s responsibility, not just the cleaners. Whilst there are staff employed by the Trust who are responsible for cleaning services, all staff employed by the Trust have a responsibility to ensure a clean, comfortable and safe environment for patients, clients, residents, visitors, staff and members of the general public. Infection Prevention & Control All Staff should co-operate fully with regard to Trust policies and procedures relating to infection prevention and control. All staff should be aware of the Trust s Infection Prevention & Control strategy and their local reduction of infection plan and know their role, in keeping with key principle Infection Prevention and Control is everyone s business. Staff, in delivery of all care must:- -Wash their hands thoroughly between each patient contact. - Be compliant with Standard Infection Control Precautions, Hand Hygiene and decontamination and other relevant infection prevention and control measures. - Be aware of the Infection Control guidance within the Trust s Infection Control Manual and ensure they obtain mandatory Infection prevention control training or other specific infection control related training as required. Hygiene & Prevention of Infection You will at all times be required to observe hygiene/infection prevention & control standards and notices in operation throughout the Trust. Hygiene and control of - infection are key elements in delivery of safe, quality services and you will, in certain jobs, be required to undertake an appropriate training course at the expense of the Trust.
