Summary of role 1. Integrated Nursing Team The Health Care Assistant will be required to work under the direction of the Senior Practitioner Nurse Specialist or Senior Staff Nurse Nurse working within the area of Community Nursing The Health Care Assistant will participate, as part of the Community Nursing team, in providing care to both chronically ill and acutely ill patients and those requiring a rehabilitative programme. Provide support to nursing staff in the administrative duties/it skills associated with Community Nursing. He/she will accept responsibility for delegated care as allocated within competencies achieved through QCF in Care/Clinical Health Care Skills. 2. Treatment Room Services The Health Care Assistant will be required to work under the direction of the registered nurse within a general practice setting. The Health Care Assistant will participate, as part of the Treatment Room Nursing team, in providing care and facilitating investigations / treatments to ambulant primary care patients. Provide support to nursing staff in the administrative duties/it skills associated with Treatment Room Services. He/she will accept responsibility for delegated care as allocated within competencies achieved through QCF in Care/Clinical Health Care Skills Hour Home Nursing Care Scheme The Health Care Assistant will be required to work under the direction of the Coordinator for the 24 Hour Home Nursing Care Scheme. The Health Care Assistant will accept responsibility for delegated care as allocated within competencies achieved through QCF in Care/Clinical Health Care Skills. Provide support to nursing staff in the administrative duties/it skills associated with Nursing Home care scheme. The post holders will be part of a team providing 12 or 24-hour care, 7 days a week, in shift patterns, to an individual client in his/her own home. The post holders may also be assigned to any other scheme that may occur throughout the Trust area. This may involve participation in the provision of 12 or 24-hour care to other clients in similar schemes in the Trust area. Should the scheme be brought to an end, the post holder will be re-deployed to a suitable alternative post. will accept responsibility for delegated care as allocated within competencies achieved through QCF in Care/Clinical Health Care Skills.

3 Applicable to all posts The Health Care Assistant will work within the supervision/support framework provided by the nursing team under the NMC Code of Professional Conduct June 2002 Ref. 4:6 This states that the registered nurse may delegate care delivery to others who are not registered nurses. Such delegation must not compromise existing care, but must be directed to meeting the needs and servicing the interests of patients and clients. The registered nurse remains accountable for the appropriateness of the delegation, for ensuring that the person who does the work is able to do it and that adequate supervision or support is provided. Main Responsibilities The Health Care Assistant will assist the line manager with appropriate delegated duties either in patient s own home or other Community/Treatment Room setting. These duties will include:- Carry out clinical procedures within the areas of personal care, including simple dressings, catheter, stoma, and continence care etc. Carry out clinical observation and test procedures such as blood pressure, respiration, height, weight. Carry out capillary and venepuncture sampling, obtain swabs and sputum samples and urine testing. Carry out continence reviews in keeping with Trust policy. Administration of patients medication such as eye/ear drops, topical and oral medications and others as delegated and appropriate to the role of Heath Care Assistant. Assist patients towards independence and gives practical advice to ensure immediate well being of patients. Obtains patients consent for all procedures. Receives patient into clinical area and prepares patient for procedure as required/appropriate. Acts as chaperone and assists with procedures where required. Maintains respect for patient s property and privacy and ensures patient s personal requirements are kept near at hand. Ensures confidentiality regarding information about patients and nursing department in general. Reports to line manager any changes in the patient s general condition and changes in care arrangements as appropriate. Works within and maintains their level of competency. Be aware of, and contribute to, efficient, effective use of nursing supplies, equipment and resources. Cleans and decontaminates equipment as appropriate. Actively participates in the retrieval of equipment Maintains and controls stock of pharmacy and other materials and ensures proper and safe storage of same. Works in co-operation with other team members.

4 Home Nursing Care Team as above plus Implement nursing care plans as agreed Acquire necessary training in all aspects of the individual clients nursing care. Accept responsibility for any nursing procedures delegated to the post holder after appropriate training and assessment of competency by Coordinator for the Home Nursing Care Scheme or other appropriately qualified nurse. Participate in competency assessments as conducted by Coordinator for the Home Nursing Care Scheme or other appropriately qualified nurse. Maintain patients records and participate in written and verbal handover arrangements as required. Follow the contingency arrangements as set in place within the home care nursing care scheme. Assist patients to maintain optimal function. General Responsibilities Employees of the Trust will be required to promote and support the mission and vision of the service for which they are responsible and: - At all times provide a caring service and to treat those with whom they come into contact in a courteous and respectful manner Carry out their duties and responsibilities in a manner which assures patient and client safety Comply with all instructions in regard to Infection Prevention and Control Demonstrate their commitment by their regular attendance and the efficient completion of all tasks allocated to them Comply with the Trust s No Smoking Policy Carry out their duties and responsibilities in compliance with health and safety policy and statutory regulations Adhere to equal opportunities policy throughout the course of their employment Ensure the ongoing confidence of the public in service provision Comply with the HPSS Code of Conduct Other information Records Management All employees of the Trust are legally responsible for all records held, created or used as part of their business within the Trust including patient/client, corporate and administrative records whether paper-based or electronic and also including s. All such records are public records and are accessible to the general public, with limited exceptions, under the Freedom of Information Act 2000, the Environmental Information Regulations 2004 and the Data Protection ct Employees are required to be conversant with the Trust s policy and procedures on records management and to seek advice if in any doubt.

5 Pay and Terms and Conditions Agenda for Change pay and terms and conditions will apply. Salary 16,271-19,268 per annum (pro rata for part-time staff ) Hours Various depending on post. Medical Appointment will be subject to a successful pre-employment health assessment. Holidays 27 days per annum, 29 days after 5 years service, 33 days after 10 years service and 10 statutory and public holidays (pro rata for part-time) Pension The post holder can participate in the Health and Personal Social Care Pension Probationary Period You will serve a probationary period of normally six months. Waiting List A Trustwide waiting list will be created for permanent/temporary, full/part posts that may arise within the next 9 months Pay Protection Legacy Trust Staff who apply for a promotion after 1 April 2011 should be aware that they will loose their legacy protection arrangements and will be entitled to pay protection in accordance with circular HSS (JNF)(1) This circular will also apply to legacy staff who are currently in receipt of protection and who move post on promotion Closing Date Completed application forms should be returned to Human Resources, Bush

Ability to maintain hand written records. Willing to undertake any relevant units at QCF in Care/ Clinical Health Care Skills to fulfil roles and responsibilities appropriate to post. Failure to achieve these units within the agreed time scale will result in termination of employment. Confirmation of employment will be upon receipt of Certificate of Award.
