1 JOB DESCRIPTION POST: DIRECTORATE: REPORTS TO: ACCOUNTABLE TO: INITIAL LOCATION: HOURS: Specialty Doctor in Emergency Medicine Medicine Clinical Director in Emergency Medicine Associate Medical Director Lagan Valley Hospital, Downe Hospital and The Ulster Hospital Full Time JOB SUMMARY/MAIN PURPOSE: PROFILE OF SOUTH EASTERN HEALTH & SOCIAL CARE TRUST The South Eastern and Social Care Trust was established on 1 April 2007 and is one of the 5 Trusts in total within Northern Ireland. The new Trust is an amalgamation of the former Ulster Community & Hospitals Trust and the Downe and Lisburn Trust. The Trust is an integrated organisation, incorporating acute hospital services, community health and social services and services a population of approximately 440,000. THE ULSTER HOSPITAL is part of the South Eastern Health & Social Care Trust and provides care in the following specialties: Emergency Medicine General Medicine Coronary Care General Surgery Otorhinolaryngology Obstetrics/Gynaecology Geriatric Medicine Medical/Surgical Paediatrics Pathology Renal Service Fracture Service Ophthalmology Intensive Care Anaesthesiology Dermatology Psychiatry
2 Plastic and Maxillofacial Surgery Radiology There is a Cardiac Arrest Team based in the Cardiology Unit. The early management of major trauma is the responsibility of the Emergency Department, with anaesthetic, surgical and orthopaedic back up on site. THE LAGAN VALLEY HOSPITAL is primarily an elective general hospital within South Eastern Trust, situated on a spacious site in the centre of Lisburn providing acute medicine and elective surgical services to a resident population of approximately 180,000, which is increasing at a rate of about 1.5% year on year. This is approximately 25% of the total population of the Eastern Health and Social Services Board. The Emergency Department is currently open from 08:00 hours until hours seven days a week. THE DOWNE HOSPITAL is a small district general hospital situated on the edge of Downpatrick. The newly built hospital has a complement of 49 beds and provides inpatient and outpatient services in a range of specialties including General Medicine, Geriatric and Rehabilitation Medicine and Coronary Care. The hospital has a suite of diagnostic facilities including CT and has a busy Day Procedure Unit. The hospital also has an Emergency Department co-located with the GP Out of Hours Service. A new model for emergency care has just been approved by the Minister in which the Emergency Department will accept new patients presenting to the Department from 08:00 hours until hours seven days a week. The General Practice Out-of-Hours Scheme (GPOOH) will accept patients from 22:00 hours until 08:00 am. Patients who have attended prior to hours and are booked on the Emergency Department system will continue to be processed by ED medical staff. ED medical staff will remain in the Department until 12 midnight to facilitate the treatment of these patients. Emergency Department Nursing staff will be present on a 24 hour basis. There are close relationships with Community Services, including Social Services, Step-down facilities and Community Hospitals. There are two other Emergency Departments in the greater Belfast area, within nine miles of The Ulster Hospital site. There are close working relationships with the staff of these other departments. Specialist Registrars rotate to these and other NI Emergency Departments, for their training.
3 EMERGENCY DEPARTMENT, ULSTER HOSPITAL (a) A 24-hour 'on call' Radiology service is available in the hospital, including ultrasound and CT. There is an MRI scanning unit. The department has its own X-ray Unit. Emergency Physician sonography is practised within the department. (b) Patient attendances and notes are recorded on a computerised system. This has 4-hour target functionality on the system. There is the facility to utilise these records for statistical and demographical analysis. Current attendances are in the region of 80,000 new patients per year and the department expects this figure to grow. (c) The Emergency Department has a Rapid Assessment and Treatment Unit (RATU). Initial Medical Assessment has led to a streamlining of investigations and earlier discharge of ambulatory patients from the Emergency Department. Three pediatric cubicles are included in the RATU. (d) Whilst the bulk of admissions to the hospital are through the Emergency Department, some admissions are through the Medical Assessment Unit, and a limited number by direct General Practitioner admission. The Emergency Department medical staff can admit directly to the hospital, without prior recourse to in-house medical staff. This is for the majority of admissions, excluding the sub-specialties. There is a system of streaming of attendances into minor and major cases within the department. ROLES & RESPONSIBILITIES The post holder will be required to fulfil the following responsibilities. The Post holder will work 10 programmed activities per week. Post Holders wishing to gain entry to the Specialist Register for Emergency Medicine via article 14 will be reasonably supported via the job planning and appraisal processes. Regularly perform clinical diagnosis, treatment and care for patients, both new and re-attendances. To assist in the teaching and supervision of undergraduate medical students and postgraduate medical staff. To assist in the provision of care in the event of a major incident. The post holder will be expected to take part in continuing professional development within the guidelines set by the General Medical Council and the College of Emergency Medicine. To facilitate this, protected office time will be provided. To participate in regular audit. Research is encouraged, and it is expected that this will take place within the overall Research and Development Strategy of the Trust. Funding for research will require appropriate bidding. The Trust is committed to providing safe and effective care for patients. To ensure this, there is an agreed procedure for medical staff that enables them to report, quickly and confidentially, concerns about the conduct, performance or health of medical colleagues, (Chief Medical Officer,
5 1996). All medical staff practising in the Trust should ensure that they are familiar with the procedure and apply it. Records Management All employees of the Trust are legally responsible for all records held, created or used as part of their business within the Trust including patient/client, corporate and administrative records whether paper based or electronic and also including s. All such records are public records and are accessible to the general public, with limited exceptions, under the Freedom of Information Act 2000, the Environmental Information Regulations 2004 and the Data Protection Act Employees are required to be conversant with the Trust s policy and procedures on records management and to seek advice if in doubt. Environmental Cleaning Strategy The Trust s Environmental Cleaning Strategy, recognises the key principle that Cleanliness Matters. It is everyone s responsibility, not just the cleaners. Whilst there are staff employed by the Trust who are responsible for cleaning services, all staff employed by the Trust have a responsibility to ensure a clean, comfortable and safe environment for patients, clients, residents, visitors, staff and members of the general public. Infection Prevention & Control All Staff should co-operate fully with regard to Trust policies and procedures relating to infection prevention and control. All staff should be aware of the Trust s Infection Prevention & Control strategy and their local reduction of infection plan and know their role, in keeping with key principle Infection Prevention and Control is everyone s business. Staff, in delivery of all care must:- - Wash their hands thoroughly between each patient contact. - Be compliant with Standard Infection Control Precautions, Hand Hygiene and decontamination and other relevant infection prevention and control measures.
8 - Be aware of the Infection Control guidance within the Trust s Infection Control Manual and ensure they obtain mandatory Infection prevention control training or other specific infection control related training as required. Hygiene & Prevention of Infection You will at all times be required to observe hygiene/infection prevention & control standards and notices in operation throughout the Trust. Hygiene and control of -infection are key elements in delivery of safe, quality services and you will, in certain jobs, be required to undertake an appropriate training course at the expense of the Trust. Personal Public involvement Staff members are expected to involve patients, clients and their families in developing, planning and delivering our services in a meaningful and effective way, as part of the Trust s Personal Public Involvement (PPI) Strategy. Location Please note that it is a standard condition that all Trust staff may be required to work at any location within the Trust s area, as needs of the service demand. REMUNERATION The current whole-time salary scale for this post will be that applicable to Specialty Doctors under the National Terms and Conditions of Service.
9 CUSTOMARY HOLIDAYS Full Time Specialty Doctors will receive 10 Public Holidays and 2 Statutory Holidays or days in lieu thereof. The two statutory days may, by local agreement, be converted to a period of annual leave. LEAVE ARRANGEMENTS Doctors who have completed a minimum of two years service in the specialty doctor grade and/or in equivalent grades or who had an entitlement to six weeks annual leave a year or more in their immediately previous appointments shall be entitled to annual leave at the rate of six weeks a year. All other doctors shall be entitled to leave at the rate of five weeks a year and six weeks thereafter. GENERAL MEDICAL COUNCIL REGISTRATION The appointee will be required to maintain full registration with the General Medical Council. MEDICAL FITNESS The appointment is subject to the completion of a satisfactory medical examination. PENSION There is an Occupational Pension scheme in operation. This appointment is superannuable and, unless you opt out of the scheme, superannuation contributions will be deducted from your pay. MEDICAL DEFENCE Employees are normally covered by the HPSS and Community Health Service Indemnity against claims for medical negligence. However, certain circumstances may not be covered by the Indemnity, especially where a separate fee is received. The Department of Health, Social services & Public Safety (DHSS&PS) therefore advises that membership of a medical defence organisation is maintained.
10 NATIONALITY If successful there will be a requirement to produce documentary evidence of a legal entitlement to live and work in the United Kingdom. This documentation can be a P45, payslip, P60, National Insurance card, a birth certificate confirming birth in the UK or republic of Ireland or a passport confirming EEA nationality or which shows otherwise an entitlement to live and work in the UK. Failure to produce this evidence will result in a non-appointment. CONFIDENTIALITY All staff have a legal duty to uphold the confidentiality of patients and/or clients information, and information relating to Trust business. CLINICAL GOVERNANCE The employee should be fully conversant with the principle of Clinical Governance and is expected to contribute on an equitable basis to support the Department in discharging its responsibilities in providing a high quality service. Part of this will involve demonstration of and keeping of CME records. Regular annual appraisal will be conducted.
11 JOB SPECIFICATION Specialty Doctor in Emergency Medicine Essential Criteria: The possession of full registration with the General Medical Council AND Shall have completed at least four years full time postgraduate training (or it s equivalent gained on a part time or flexible basis) at least two of which will be in a specialty training programme in a relevant specialty or as a fixed term specialty trainee in a relevant specialty; OR Shall have equivalent experience and competencies. Desirable criteria: MCEM, MRCS A&E, MRCS, MRCP, or equivalent. Possession of ATLS, ALS, PALS or APLS certificates. Paediatric experience.
