CSR for the Carolinas Educational Workshop at Kingston Plantation Myrtle Beach Purpose The Continuous Service Readiness (CSR) Program for the Carolinas is a joint effort between the (JCR) and South Carolina Hospital Association (SCHA) to structure and implement a program for hospitals to receive education, consultation, and feedback on a continuous basis concerning Patient Safety and Joint Commission compliance requirements. Through CSR, hospitals have an avenue to help share a continuous service readiness process that incorporates performance measurement and patient safety into the quality processes. Objectives Upon completion of this workshop, participants will: 1. Identify at least one best practice for infection prevention and control, 2. Identify potential problems related to sterilization or use of high level disinfectants, 3. State two characteristics of high reliability organizations, 4. Identify at least two challenges for Ambulatory Sites being surveyed under HAP Standards, 5. List two of the top findings from CMS and be able to discuss strategies for compliance, 6. Verbalize understanding of the CMS worksheets related to QAPI, IC and Transitions of Care. Continuing education The South Carolina Hospital Association is an approved provider of continuing nursing education by the South Carolina Nurses Association, an accredited provider by the American Nurses Credentialing Center s Committee on Accreditation. Participants who attend the conference and complete an evaluation will receive 11.75 contact hours. 2

No paperwork required! Payment Options: Credit card Invoice Registration and fees The early bird registration fee is $230. Registrations received after March 3 will be considered late and the fee will be $260. All walk up registrations are required to pay the late registration fee and payment must be collected onsite. All attendees must register online. To register online, visit the SCHA web calendar, www.scha.org/members/calendar, select this meeting, and click Register Online. A registration and payment confirmation will be emailed directly to you once you complete the registration process. Refund/Cancellation Policy The deadline for registration is March 3, 2016. Any registrant who cannot attend a meeting may send a substitute from the same organization. If possible, please notify SCHA of the substitution in writing (by letter or FAX). The registration fee (less a $25 processing fee) is refundable if SCHA is notified of the cancellation no later than March 3, 2015 (5 business days prior to the program.) Tobacco SCHA encourages tobacco free people and places so tobacco use is prohibited at SCHA events and on SCHA grounds. Healthy menu items that comply with the healthy food nutrition criteria will be available at each meal service. Please note that sugar sweetened beverages do not comply with the nutrition criteria and will not be readily available. Physical Activity- SCHA encourages the use of scheduled breaks for physical activity (stretching, walking, etc) to help maintain optimal health.
