PAYMENT OPTIONS To lose someone you love is a devastating human experience. It s frequently traumatic and can seriously impact judgement. It is important to celebrate the life of someone important to us when that person has died, and to make the experience as meaningful as possible. It is also important to be realistic in respect to funeral expenditures. We do not want a family to spend more than they are able, nor less than they would like. What is important is that the arrangements be in keeping with the family s wishes and budget. Because funeral costs are often shared by different individuals and frequently divided between family members, financial arrangements must be resolved prior to the funeral service. In this way, the possibility for misunderstanding between family members is avoided. Arlington s policy requires that all financial arrangements must be completed prior to the setting of any viewing times or services. We offer several payment options to you at this time. The following methods of payment are available for your convenience. OPTION A: Pre-Arranged Funeral Plan You may pay with a pre-arranged or pre-paid funeral arrangement. If pre-arrangements have been made, please inform your Funeral Director OPTION B: Cash or Credit Card You may pay the full amount at the time of funeral arrangements using either cash, check or major credit card. We do not accept third party checks. OPTION C: Verifiable and Assignable Insurance This option allows you to assign over all or a portion of insurance proceeds to pay for funeral expenditures. Policies, under two years old, are generally contestable, and are not acceptable. When acceptable insurance has been assigned, a monthly statement will be sent to the purchaser until payment is received from the insurance company. (We do not accept future payment from Estate Funds.) Disclaimer of Warranties Arlington makes no representation of warranties regarding caskets, outer burial containers, urns, or other funeral goods sold by the Funeral Home. The only warranties granted, express or implied, in connection with the goods sold with this funeral service are the express written warranties, if any, extended by the manufacturers thereof. No other warranties, express or implied, including implied warranties of merchantability or fitness for a particular purpose, are extended by the Funeral Home. 2

ITEMIZED SELECTIONS The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. PROFESSIONAL SERVICES Basic Services of Funeral Director and Staff... $1,370 Our basic services charge includes, but is not limited to 24-hour availability of funeral director and/or staff to respond to initial request for service, arrangement conference with family and/or responsible party, arrangement of service, tribute or memorialization. Staff to advise, assist and recommend appropriate personalization and individualization of the services to commemorate the unique life of your loved one. Obtaining and recording vital statistics and biographical information. Death Certificate preparation, filing, obtaining of necessary permits, authorizations, consents and assignments. Coordination with other parties such as hospital, doctor s office and staff, cemetery, crematory, clergy and others involved in the services and disposition. Also included in this charge are overhead expenses relative to our facilities such as professional licensing, administrative services, insurance, maintenance, building and utility expenses, legal and accounting fees, record keeping, equipment and inventory expense. The fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains.) This charge is also included in the other packaged funeral offerings as allowed by law. STAFF SERVICES, FACILITIES AND EQUIPMENT We believe the services of the funeral home are essential for the family to grieve properly. The service provides a time to say goodbye, but more importantly a time for family and friends to remember and pay tribute to a life that was lived. Everyone leaves a legacy and it is that life that needs to be celebrated. Visitation at the Funeral Home (Per Four Hour Period)... $150 Our services include: visitation setup, use of visitation room, display of floral arrangements, supervision and attendance during the viewing, visitation or wake. Funeral Service at the Funeral Home or Other Facility... $200 Our services include: coordinating the funeral arrangements, supervision of funeral, staff coordination of the funeral ceremony, setup and use of facilities for funeral ceremony and supervision of committal or other disposition service. Memorial Service at Funeral Home or Other Facility... $200 Our services include coordinating the memorial service, supervision of the memorial service, staff coordination of ceremony, setup and use of facilities and equipment for the memorial service. Graveside Service... $200 Our services include coordinating the funeral arrangements at graveside, supervision of the graveside service, equipment and staff necessary to conduct the graveside service. Holiday Overtime charge... $350 3

CARE OF THE DECEASED We require the use of licensed personnel to supervise all technical aspects of our services. Utmost care is taken in all steps of our preparation. Our Patient Care Facility is OSHA and State compliant. Our personnel and management take their role very serious and it shows in the dedication to our clients. Embalming... $495 Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. Our drivers are versed on the best routes and can provide directions to family and friends. Care is taken by our staff at all times when operating our funeral vehicles. This charge does not include visitation, rites or ceremonies or airfare prior to forwarding of the remains. With minimum shipping container... $1,620 Air Tray to encase casket in air transit......$195 RECEIVING REMAINS FROM ANOTHER FUNERAL HOME... $1495 This charge includes temporary sheltering of remains, reduced basic services of funeral director and staff, transfer of deceased to funeral home and transportation of deceased to Cemetery and cross filing of permits. This charge does not include the use of staff and facilities for visitation, rites or ceremonies or any other service or merchandise not specifically listed. IMMEDIATE BURIAL.......$1,995 to $11,990 This charge includes transfer of deceased to funeral home, reduced basic services of funeral director and staff, local transportation to the cemetery, necessary authorizations and permits. This charge does not include cemetery or government fees, the use of staff and facilities for visitation, rites and ceremonies or any other service or merchandise not specifically listed. Immediate burial with casket provided by purchaser......................... $1,995.00 Immediate burial with minimum casket selected from our funeral home.......... $2,490.00 DIRECT CREMATION.......$595.00 to $6,480.00 This charge for a direct cremation includes transfer of deceased to funeral home, local transportation to crematory, crematory fees, necessary authorizations, and reduced basic services of the funeral director and staff. This charge does not include the use of facilities and staff or any visitation or ceremony, public or private, prior to the cremation. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are made of heavy fiberboard.
