You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. Prior to drafting any contract for goods and services, the responsible party handling the final arrangement is entitled to receive a copy of any pre-need agreement in the possession of the funeral establishment that has been signed and paid for, in full or in part, by or on behalf of the decedent. To provide the services requested, it is necessary for financial arrangements to be settled before the remains are casketed or cremated. We accept cash, visa, master card, money orders, and cashier s checks. We do not offer payment plans nor accept postdated checks or insurance assignments. Professional Services: Basic Service Fee of Funeral Director and Staff, and Overhead.$ 1, Our basic service fee includes: 24 hour availability of staff to respond to initial request for service; arrangement conference; planning selected tribute ceremony; securing vital statistics; coordination with cemetery, crematory, and other third parties; overhead costs such as, insurance, maintenance, utility, secretarial, administrative, and equipment expenses; and ordinary sheltering of remains (up to 72 hours). This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains). This service is provided at the convenience of our facility once all legal requirements are completed. Cremated remains are generally available within 14 days from date of permit filing but can and may take longer. Decedents in excess weight of 250 lbs will incur additional fees. A minimum combustible cremation container must be one that can be closed and is leak-resistant. The deceased remains are not dressed or embalmed but these services can be provided, at additional fees. Memorial services are not available with this service option, but can be provided for by selecting another service option that offers that provision. The only warranties expressed or implied, granted in connection with the goods sold with this funeral service are the expressed written warranties, if any, extended by the manufacturer thereof. No other warranties, expressed or implied, including the implied warranties of merchantability or fitness for a particular purpose, are extended by Ferrara Colonial Mortuary. In most areas of the country, state or local law does not require that you buy a container to surround the casket in the grave. However, many cemeteries require that you have such a container so that the grave will not sink in. A grave liner or a burial vault will usually satisfy these requirements. Ferrara Colonial Mortuary is not responsible for delays in service due to the failure of the physician to discharge their duty to complete the medical portions of the death certificate within 15 hours of death, or if incomplete or inadequate information is provided by the physician or next of kin. Additional attempts to obtain a physician s signature will result in additional charges. Burial or cremation cannot take place until a valid death certificate is filed with the county health department and a disposition permit is issued. Ferrara Colonial Mortuary is not responsible for delays in shipping of remains due to failure of the physician to provide adequate causes for the death certificate, delays in paperwork processing by the consulates of foreign governments, and unavailability of ground or air transportation. Ferrara Colonial Mortuary accepts no responsibility for jewelry or other valuables left with the deceased while the deceased is in view of the general public. Funeral processions on public streets or roads must be escorted in accordance with guidelines set forth by local law enforcement agencies. Therefore, Ferrara Colonial Mortuary is authorized, if applicable, to order at their discretion, sufficient escorts for this service. Additional charges for this service, if any, not included on the original agreement will be added at the prevailing rate. Page 5
