6 Cremation Alternatives Many options are available to consider with cremation and the staff at Harris Funeral Home would be pleased to talk with you about your preferences. Over two-thirds of the families we serve opt for cremation. Sometimes a traditional funeral takes place prior to cremation. Other times, a memorial service is arranged; this is generally accepted to mean that the body will not be present for a service. When planning a memorial service family may choose to have a cremation urn present for visitation and services. As with traditional services, photos and flowers, music and ritual, may help to bring meaning and comfort. Our bright, homelike setting provides an ideal environment for family and friends to gather in remembrance. We also offer Immediate Cremation services. Our fees are very competitive and as always, our staff are compassionate and will be attentive to your wishes. 2. The following pages list fees for services and supplies (caskets, urns, outer burial container) which are charged if a package is not chosen from the available options in the Clear & Simple Funeral Pricing guide. 9 PROFESSIONAL & STAFF SERVICES FEE SCHEDULES Basic professional fees $ Professional consultation with licensed personnel, which may include but is not limited to, expert advice on options to meet your needs, including essential services relating to disposition, drafting notices, planning scheduling of services, advising regarding possible government benefits. A Basic professional fee is payable whenever we provide services to you. Coordinating rites and ceremonies $ Coordination of all rites and ceremonies, before, during, and after they have been provided, including services and supplies provided by us, as well as those provided by third party suppliers, such as flowers and death notices. Documentation permits, forms, etc. $ Completion and filing of all documents necessary to carry out the services and supplies requested, including, but not limited to, death registration, burial permit, coroner s certificate for cremation, documentation necessary to the ship the body out of the country. As part of this service, we will provide you with 15 proof of death certificates; such certificates are usually accepted for most estate settlement purposes, but sometimes a provincial Death Certificate will be necessary and is available from the Office of the Registrar General. Transport Remains $ Transportation of the remains from one place to another, for example the hospital to our premises, or from our premises to the cemetery or crematorium using a transfer vehicle, for distances not exceeding 50 km. A charge applies for each transportation request. Additional charge for distance per km, one way: $1.50/km Additional staff service charge for removal from residence or other non-institutional setting: $95.00 Additional staff service charge for remains in excessof 300 lbs. $ Basic preparation of remains $ (requires facilities for preparation/embalming/shelter) The preparation of the body for placement in the casket, including bathing, dressing and the setting of facial features for identification or viewing without embalming. Embalming of remains $ (basic preparation fee included embalming requires facilities for preparation/embalming/shelter) Embalming is the process of replacing blood and bodily fluids with chemical preservatives. It is a process of sanitation, restoration and temporary preservation. Embalming is not legally required, but may be recommended to preserve the body between the time of death and the visitation or disposition of the deceased. Our professional funeral directors will make a recommendation based on the condition of the body and expected service needs. Staff services for visitation $ Staff to coordinate and oversee the visitation (informal gathering) prior to ceremony, funeral or memorial service. Staff services for ceremony $ Staff to coordinate and oversee the ceremony or memorial. Staff services for after-service reception $ Staff to prepare beverages, serve and clean-up for an after-service reception of up to 85 people. Additional staff charge for after-service gathering up to 150 people $ Unless otherwise indicated, all prices do not include applicable taxes 9

A sedan may also be provided for family, comfortable for 3 passengers. A charge applies for each transportation request. Limousine: Limousines may be hired from local services as requested. The fee will be invoiced under disbursements at the costs charged by the limousine services and are not included in packages in the Clear & Simple Funeral Pricing guide. Photo Scanning Services $2/photo A surcharge to Tribute Supplies package - cost per.jpg scan and cropping In excess of 25 scans up to a limit of 125 photos. All.jpg scans will be saved on to one DVD (additional DVDs cost $5 per DVD) or USB flash drive. Visual Equipment (included in package prices) $ LCD monitor, as available on-site, to display scanned photos as a visual tribute Equipment for visitation at a remote location $ Delivery, set up and use of equipment necessary for visitation or ceremony other than the funeral establishment includes flower stands, tribute supplies, portable LCD monitor, casket bier Removal of a pacemaker or implant $50.00 The removal of a pacemaker or implant will be carried out by a licensed professional. Professional restorative procedures $100.00/hour In some circumstances, additional restorative efforts beyond those captured in the standard embalming procedure are necessary to enhance the appearance of the deceased. In such cases, the funeral director will discuss the available options and their cost. Professional restorative procedures will be charged per hour. Ship Out Services Charge (minimum cost) $ If remains are being shipped to another location, we will arrange for transport and all necessary paperwork. Scattering $ We will scatter cremated remains in accordance with Ontario and municipal laws and by-laws on land. Fees for scattering from a plane or boat will be in addition to this charge and will be discussed upon request. Refundable Deposit for Cremated Remains $ When human remains are to be cremated, a refundable deposit will be required to be paid. If the cremated remains are not claimed within one year of the cremation, the deposit may be used to inter the cremated remains in common ground. 16 Cleary 105 Hardboard disposition container ($295.00) $ $ DISBURSEMENTS Disbursements are items arranged for and possibly paid for on your behalf by us. You are expected to pay for these items at the time the contract is entered into. The following are some items that are often identified as disbursements. H.S.T. applies to almost all disbursements. Coroner s fee this is a fee charged by the coroner for issuing a certificate that allows cremation to take place. The fee is set by the Chief Coroner for Ontario. Catering cost of food supplied by caterer for reception. Selection of caterer at discretion of family. We would be pleased to recommend caterers. Clergy honorarium this is an amount paid to the clergy for their services. In some cases the amount is set by the clergy, in others it is an amount to be determined by you. Newspaper or radio notices these are charges for death notices placed in newspapers or radio or other media outlets. We are pleased to assist you with composing a notice and will look after submission to media outlets. Municipal death registration fee this is a fee charged by the local municipality to register the death. The fee is set by the municipality and may vary. PAYMENT Unless otherwise specified, payment is due within 30 days of signing the contract. Payment may be made by cash, cheque, or bank draft. Payment plans are also available to meet most budgets. Financing charges apply to payment plans. Beyond the allotted time, interest will be charged at a rate of 2% per month which is equivalent to 26.82% per annum compounded. If you are prepaying for services or supplies, pre-authorized chequing is also available. FINANCING Our payment plans give you the option of making payments over time. We offer a competitive financing program, administration fees and interest rates. FUNDING We offer a trust funding option when prepaying services or supplies. When prepaying, you pay for the services and supplies today and the money will be used to pay for the services and supplies when they are delivered at the time of death. Trust: You may pay the money directly to us and we will place it in trust, where it will accrue interest until the contract is fulfilled (when the services and supplies are delivered) or the contract is cancelled. 16
