Our goal at Sierra View Mortuary is to provide thoughtful and efficient service to all whom we are privileged to serve. Our staff can help by taking care of the many details that must be handled after death occurs. We will also work to accommodate any special requests that might add meaning or comfort throughout the services selected and help the family to face the future with renewed strength. Prior to drafting any contract for goods or services, the responsible party or decedent s survivor who is handling the funeral arrangements is entitled to receive a copy of any preneed agreement in the possession of the funeral establishment that has been signed and paid for, in full or in part, by or on behalf of the decedent. METHOD OF PAYMENT Payment for goods and services is expected prior to the services being rendered, unless other arrangements have been made prior to need. For your added convenience, we accept cash, check, credit card (Visa, MasterCard, Discover and American Express), a verifiable irrevocable insurance assignment or financing through a financial institution. Although services may be selected from the itemized portion of the General Price List, we have a list of convenience package services that may help you select the appropriate arrangements. NOTE: The convenience packages include only the items listed. You may select additional merchandise and services; however, you may not delete or substitute any items from the convenience packages. Some cash advance items are optional. 2 I. General Information: Disclosures The goods and services shown on the following pages are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you select. This list does not include prices for certain items that you may ask us to buy for you, such as cemetery or cremation services, grave opening, flowers and newspaper notices. The prices for those items will be shown on your bill or the statement describing the funeral goods and services you selected. II. Professional Services of Funeral Director and Staff $1, This is the basic charge to each family which we serve for our professional expertise in assisting them. It includes, but is not limited to, a proportionate share of the taxes, licenses, utilities, and business expenses necessary to serve the public in a professional manner. Also included in this charge is the consultation with the family, clerical and administrative services, preparation and filling of necessary permits, consultation with the clergy, cemetery, crematory or common carrier, assistance with insurance papers, planning the funeral or memorial service and placement of obituary notices. There will be an additional charge for the direction and supervision of each service requested or required. This fee for our basic services and overhead will be added to the cost of the funeral you select. Notice: This fee or a portion of this fee that pertains to the service you select is already included in our charges for cremation services, immediate burials, forwarding and receiving remains, traditional and graveside services. III. Embalming and Other Preparation of the Body Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. A. Funeral Home Facilities, Staff and Equipment Our charges include coordinating and directing service and participants; staff to supervise and coordinate church or memorial services, preparing, supervising, setup, and attendants for visitation periods, arranging floral pieces and memorial tributes, supervising and attending at graveside, parking assistance, additional supplies and equipment. 4 VI. Funeral Merchandise Sierra View Mortuary offers a variety of caskets, urns and alternative containers from which to select. Caskets many times appear similar although they may differ greatly in quality and construction. The following is offered to better enable you to make an informed decision. Protective Caskets: These caskets are designed by the manufacturer to resist the entrance of air, water and other outside elements. They may be constructed of varying gauges of steel, copper or bronze. There is no scientific or other evidence that any casket with a sealing device will preserve human remains. Non-Protective Caskets: These caskets are not designed by the manufacturer to resist the entrance of air, water and other outside elements. They may be constructed of metal, hardwood or wood-product covered with fabric. A complete list will be provided at the funeral home. Disclaimer of Warranty: The only warranty of the casket or any merchandise sold in connection with this service is the express written warranty, if any, granted by the manufacturer. This funeral home makes no warranty, express or implied, including an implied warranty of merchantability and implied warranty of fitness for a particular purpose, with respect to the casket. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are alternative containers and other cremation containers, including containers made of fiberboard, and containers made of other composition materials. We also offer a selection of cremation oriented caskets. A complete price list will be provided at the funeral home for all of the below listed items. Private Tribute $2, This service includes Basic Professional Services of funeral director and staff, Local Transportation from place of death to our funeral home (within a 40 mile radius), Cosmetics, Dressing and Placement of remains, Private family identification (30 minutes), during normal business hours, Utility vehicle and driver, Cremation Fee, Cremation Casket/Container, Website Tribute and DVD, Memorial Package, Personalized Urn Allowance of $195, Memorial Keepsake or Remembrance Jewelry Allowance of $150. This service DOES NOT include flowers or cash advance items. XVI. Immediate Cremation $1, to $1, State and local laws do not require a casket for direct cremation. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). A. Direct cremation with container provided by purchaser $1, B. Direct cremation with Standard Brown Alternative Container and $125 Urn Allowance $1, Direct Cremation is the minimum service offered for disposition of remains. Basic Professional Services of funeral director and staff, Local Transportation from place of death to our funeral home (within a 40 mile radius), Refrigeration of un-embalmed remains up to 7 days, ($ per day thereafter), Placement of remains in an alternative container for cremation, Crematory Fee, Flower/Utility vehicle and drivers. This service DOES NOT include use of facility and staff for visitation or memorial services, memorial register book, memorial folders, acknowledgment cards, limousine, or cash advance items. XVII. Forwarding Remains to another Funeral Home $2, This service only includes Basic Professional Services of the funeral director and staff, Local Transportation from place of death to our funeral home (within a 40 mile radius), Embalming, Cosmetics, Dressing and Placement, Transportation to Commercial Transit. This service DOES NOT include facility charges for visitation, funeral or memorial service, additional automotive expenses such as flower vehicle, utility vehicle, limousine, merchandise, casket, memorial register book, memorial folders and acknowledgment cards, merchandise used to ship remains on commercial transit such as an air tray, combination container or alternative container, or cash advance items. Airline charges and receiving funeral director s charges are not included. XVIII. Receiving Remains from another Funeral Home $1, This service includes Basic Professional Services of the funeral director and staff, Transportation from commercial transit (with in 40 mile service radius) to funeral home or cemetery. This service DOES NOT include embalming, other preparation of remains, facility charges for visitation, funeral, memorial service, additional automotive expenses such as flower vehicle, utility vehicle, limousine, merchandise, casket, memorial register book, memorial folders and acknowledgment cards, or cash advance items.

We will also place the military medals into the display case when the military medals arrive. XX. Ceremonial Dove Release $ Witnessing a dove release provides many people with a soothing, healing experience at the end of a service. Children are delighted and adults feel a sense of release as the doves take flight heavenward. Package options include: Graveside Trinity & Spirit Dove Release Includes 3 doves, representing the Trinity (The Father, Son, and Holy Spirit) are released to circle. Then a single dove, representing the spirit of the loved one is released to join the other birds. Together the birds fly heavenward. A hand release of additional doves, representing family members or angels symbolizes closure, unity, and peace. A basket of doves can be released to celebrate the homecoming of a loved one. XXI. Butterfly Release $ Beautiful Painted Lady butterflies in individual boxes that are released by those in attendance of the graveside service. Our butterflies are bred and hand fed at Cloverlawn Butterfly Farms, located in Southern Florida. They are flown Fed-Ex overnight to Sierra View Mortuary. Both children and adults alike will enjoy the wonder and beauty of the butterfly release.
