French Camp Road - Manteca, CA (209) FD GENERAL PRICE LIST OF GOODS AND SERVICES When the purchaser selects one of the following "Traditional type" funeral services, the total cost may include but not be limited to items listed below. You have the right to use our Funeral Home even if burial will be at another cemetery. Payment for selected products and services is due in full at the completion of the arrangement meeting. Prices effective as of March 20, 2014 and are subject to change without notice. PRENEED DISCLOSURE Prior to drafting any contract for goods or services, the responsible party or the decedent s survivor who is handling the funeral arrangements is entitled to receive a copy of any pre-need agreement in the possession of the funeral establishment that has been signed and paid for, in full or in part, by or on behalf of the decedent. SERVICE PACKAGES (Substitutions and deletions are not allowed.) Although services may be selected from the itemized portion of the General Price List, the following list of commonly selected services packages may help you make appropriate arrangements. The following packages include only the items specifically listed in the package. Any merchandise you select is additional. All of the packages listed below include: Services of Funeral Director and Staff, obtaining and filing all necessary legal documentation, one staff member removal of the deceased from the place of death to our Funeral Home (within our local service area of Manteca, Stockton, Lathrop, and French Camp). If death occurs outside of our local service area, or a second staff member is necessary to complete the removal, additional charges will apply). TRADITIONAL SERVICE/CHURCH WITH PROCESSION TO CEMETERY: $1895 Direction and coordination of service, use of facilities for Funeral Ceremony or set up at church/other facility, service continued to graveside, all necessary refrigeration, washing, disinfecting, embalming, dressing, casketing, cosmetology and hair preparation, automotive equipment (funeral coach, flower van/utility vehicle), standard registration book, and 25 memorial folders/acknowledgement cards. TRADITIONAL SERVICE/PARK VIEW CHAPEL TO GRAVESIDE: $1695 Direction and coordination of service, use of facilities for Funeral Ceremony, service continued to graveside, all necessary refrigeration, washing, disinfecting, embalming, dressing, casketing, cosmetology and hair preparation, automotive equipment (funeral coach, flower van/utility vehicle), standard registration book, and 25 memorial folders/acknowledgement cards. 2 CREMATION WITH CHAPEL MEMORIAL SERVICE*: $1295 Direction and coordination of service, all necessary refrigeration, pre-cremation container, cremation process, temporary plastic container, use of chapel and facilities for memorial service, standard registration book, and 25 memorial folders/acknowledgement cards. All cremations are done at our facility and are not shipped elsewhere. DIRECT CREMATION*: $645 Our charge for a direct cremation (cremation without ceremony) includes: basic services of funeral director and staff, standard removal of remains within Our local service area, all necessary refrigeration, pre-cremation container, cremation process, and a temporary plastic container. All cremations are done at our facility and are not shipped elsewhere. (OR) Direct Cremation with container provided by the purchaser*: $635 If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are a heavyduty fiberboard container. *Note~ For the cremation options above the following cost will be added to the price noted above if the decedent exceeds the following weight: 300 lbs up to 399 lbs shall be an additional $100; 400 lbs up to a maximum of 500 lbs shall be an additional $200 Keep in mind that when a decedent s weight is above 300 lbs, the purchase of an alternate cremation container may be required. IMMEDIATE BURIAL: $995 Our charge for an immediate burial (burial without ceremony) includes: basic services of funeral director and staff, standard removal of remains within our local service area, all necessary refrigeration, and unaccompanied delivery of remains to any cemetery in the local service area. (This fee does not include a casket.) CHILD SERVICES: A. Traditional Service/Park View Chapel Conclusion: Direction and coordination of service, use of facilities for Funeral Ceremony, service concludes in chapel, transfer of remains to graveside, all necessary refrigeration, washing, disinfecting, embalming, dressing, casketing, cosmetology and hair preparation, automotive equipment (funeral coach, flower van/utility vehicle), standard registration book, and 25 memorial folders. Infant (2 years and under): $525 Child (3 years to 12 years): $725 B. Traditional Graveside Service: Direction and coordination of service, transfer of remains to graveside, set up at graveside for Funeral Ceremony, all necessary refrigeration, washing, disinfecting, embalming, dressing, casketing, cosmetology, automotive equipment (funeral coach, flower van/utility vehicle), standard registration book, and 25 memorial folders. Infant (2 years and under): $475 Child (3 years to 12 years): $675 C. Immediate Burial: Our charge for an immediate burial (burial without ceremony) includes: basic services of funeral director and staff, standard removal of remains within local service area, all necessary refrigeration, and delivery of remains to any cemetery in our local service area. Infant (2 years and under): $395 Child (3 years to 12 years): $595 D. Direct Cremation: Our charge for a direct cremation (cremation without ceremony) includes: basic services of funeral director and staff, standard removal of remains within Our local service area, all necessary refrigeration, pre-cremation container, cremation process, and a temporary plastic container. All cremations are done at our facility and are not shipped elsewhere. Infant (under 20 weeks gestation): $200 Infant (over 20 weeks gestations to 2 years old): $350 Child (3 years to 12 years): $495 2

3 ITEMIZATION OF PRICES The goods and services listed below are those we can provide to our customers. You may choose only those items you desire. However, any arrangement you select will include a charge for our services. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. BASIC SERVICES FEE $500 This fee for our basic services and overhead and will be added to the total cost of the funeral arrangements you select. This fee is already included in our charges for traditional services, direct cremations, and immediate burials. REMOVAL AND TRANSPORTATION OF REMAINS TO FUNERAL HOME: $250 This includes single man removals on nights, weekends, and holidays within the Stockton/Manteca area. For transportation outside of this area please consult the mileage guide. Second attendants are needed in some cases. In these cases an additional fee will apply. (Note: Decedents weighing over 300 pounds will require use of bariatric gurney. See "Other Care Of Decedent") PROFESSIONAL EMBALMING $250 Except in certain special cases, embalming is not required. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you may choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. Embalming is a chemical process, which provides temporary preservation of the body and eliminates certain health hazards. VIEWING AND VISITATION: A. Park View Chapel Use (3 hours)... $225 B. Full Day of Visitation... $425 C. Rosary (Park View Chapel/Church)...$350/$450 D. Service of remembrance with service staff, 25 memorial folders, and guest book... $425 E. Required Preparation For Identification... $95 F. Overtime charge for staff on Sat., Sun. & Holidays (available from 10am to 6pm only).... $350 OTHER CARE OF THE DECEDENT: A. RECEIVING REMAINS FROM ANOTHER FUNERAL HOME:... $500 This charge includes minimum service of staff, and transportation of remains to Park View. Transportation from outside the Stockton/Manteca area is not included, please consult mileage guide. DISPOSITION OF CREMATED REMAINS: SCATTERING OF CREMATED REMAINS:... Family to contract with licensed disposer. SHIPPING OF CREMATED REMAINS - INTERCONTINENTAL USA:... $115 Park View is happy to serve our client in this regard. However, it is important to note that once Park View accepts the receipt of Registered Mail after arranging shipment, we do not accept responsibility for the U.S. Postal Service delivery. 3

We charge you for our service in buying these items except where prohibited by law. You will be charged only for those items that you request. If we are required to use any items, we will explain the reason in writing within our purchase agreement. If you select a funeral, which requires embalming, such as a funeral with viewing, you may have to pay for embalming. You do not have to pay for embalming that you did not approve of, such as in an immediate burial or direct cremation. CASH ADVANCES Cash advances cannot always be itemized due to the variance of requests from each individual. Variable CASKETS & MERCHANDISE We offer many different styles and prices of caskets from which to select. Since many caskets that seem similar in appearance may differ greatly in quality and construction, we offer the following to assist you in making an informed decision. PROTECTIVE These caskets are designed by the manufacturers to resist the entrance of air, moisture and other outside elements. They may be constructed of varying gauges of steel, copper, or bronze. NON-PROTECTIVE These caskets are not designed by the manufacturers to resist the entrance of air, moisture, and other outside elements. They may be constructed of hardwood or wood-products covered with fabric. There is no scientific or other evidence that any casket with a sealing device will preserve human remains. 5 Our Funeral Home makes no representations or warranties about the protective value of any casket other than those made by the manufacturer. The only warranties, expressed or implied, granted are the expressed written warranties, if any, executed by the manufactures thereof. The seller extends no other warranties for any product. A complete price list will be provided at the funeral home. CERMIMONIAL CASKET (Casket Rental) This is for the use of a rental casket for the funeral service only. At the end of the service the alternative container will be removed and the casket will remain the property of Park View Cemetery & Funeral Home. Our price does include the purchase of the inserted alternative container. WHY PARK VIEW CEMETERY & FUNERAL HOME? Park View Cemetery & Funeral Home provides specialized pre-need and at-need planning arrangements from customized traditional services to simplified services, which include minimum graveside services, direct cremation, and/or immediate burial. We offer the services Cemetery, Funeral Home, Crematory, Flowers, and Monuments. At Park View, we offer these varied products and services, at exceptional value, which can all be arranged at one convenient location. WHY PLAN AHEAD? Individuals have expressed to us the sense of relief and serenity they have experienced having known or discovered their loved ones thoughtfully attended to and completed the task of pre-planning and pre-paying their funeral arrangement. Imagine for a moment how these decisions might be made if never talked about or discussed. IDENTIFICATION CARDS Upon completing your pre-arrangements, you will be given two identification cards; one is intended to be carried within your wallet; the other is intended to be placed in your medical file within the office of your primary physician. You should also tell your family that you have completed pre-arrangements with us. FINANCIAL TERMS AT-NEED: Payment for selected products and services is due in full at the completion of the arrangement meeting. If you are wanting to pay for the services with an insurance policy, you may do so as long as the insurance policy is verifiable and is in force beyond the contestable period at the time of death of the insured; AND is assignable by the beneficiary to Park View Cemetery & Funeral Home, at the time of the arrangements; and is free of liens. We must also be able to define who the beneficiaries are at the time of assignment. Such policies are acceptable as a guarantee of payment of the balance due. This in no way relieves the beneficiary of his/her obligation to pay should the insurance company fail to honor the policy or assignment. Interest will be charged after 30 days beyond the initial service agreement date. Generally the personal representative or estate attorney has access to funds, payable immediately for funeral expenses. Payment of goods and services are due and payable upon completion of the arrangement session. Payment options are cash, money order, certified check or Visa, MasterCard, American Express and Discover credit cards. Due to the high rate of returned checks, we no longer accept checks for at-need service arrangements. PRE-NEED: Park View accepts funds that, by law, must be placed in a trust for prearranged funeral or burial. We are obligated to deposit said funds within 30 days of collecting them. We utilize California Master Trust for trust holding. We utilize Great Western Insurance for funeral insurance holding. We want our clients to understand the process and reasoning for the strict guidelines used in protection of their deposited funds. Park View cannot gain access to these funds until the individual has passed away, and a death certificate with verification is provided to the trust or insurance institution. Your funds are protected and safely deposited as required by California State Law. Estate claims may be filed by the next-of-kin or the person responsible for the service arrangements for reimbursement of funeral expenses. 5

6 GENERAL STATEMENT The survivor of the deceased who is handling the funeral arrangements, or the responsible party, is entitled to receive, prior to the drafting of any contract, a copy of any agreement that has been signed and paid for, in full or in part, by or on behalf of the deceased, and that is in the possession of the funeral establishment. All staff members of Park View strictly maintain family confidentiality and privacy. In 1916, the founders of Park View Cemetery began the tradition of conducting funeral and burial services within the grounds of Park View Cemetery. Dave, Kathy, Amber, and the rest of our team proudly carry on this family tradition. Park View Cemetery & Funeral Home exudes the echo of life, in the songs of the birds, and the loveliness of nature all around. Park View Cemetery & Funeral Home, located on French Camp road near Hwy 99, has a dignified country atmosphere. Our chapel, located within the cemetery grounds, has seating arrangements to accommodate 75 guests. Caskets are placed reverently in the Chapel in full view, undisturbed, as the family and friends enter for visitation or funeral services.
