2 Ruebel Funeral Home This General Price List is provided in compliance with Federal Trade Commission regulations. All funeral homes are required to price their services in a similar format, and a General Price List must be given to every family served. At Ruebel Funeral Home, we feel every family should have the opportunity to create a funeral service that fulfills its unique needs. In this spirit, we are ready to assist you in arranging the services you desire in the price range you prefer. Our goal is to faithfully serve your needs, regardless of the type of service you select. ALL OBITUARY INFORMATION MUST BE RECEIVED BY THE FUNERAL HOME BY 12 NOON TO RUN IN THE NEXT DAY PAPER. PAYMENT FOR SERVICES IS DUE AT THE TIME OF THE ARRANGEMENT CONFERENCE Effective February 1, 2014 (Prices are subject to change without notice) Professional Services A. BASIC SERVICES OF FUNERAL DIRECTOR AND STAFF $2,200 Our fee for the service of funeral director and staff includes, but is not limited to, staff to respond to initial request for service 24 hours daily; consultation with the family or responsible party; coordinating service plans with cemetery, crematory, and/or other parties involved in the final disposition of the deceased; securing official documents and necessary registrations and sheltering of the deceased. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains.) B. EMBALMING $795 Embalming is the chemical sanitation and temporary preservation of the deceased by a professional licensee of the state. Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. C. OTHER PREPARATION OF THE BODY $350 This service includes cosmetology, hairdressing (when necessary), and placement in casket. Facilities A. Continuous use of Facilities for Visitation/Rosary/Reception between 8:30 a.m.-4:30 p.m.(2 Hours) $350 B. Use of Facilities and Staff for Visitation/Rosary/Reception after 4:30 p.m. (2 Hours) $550 C. Use of Facilities, Staff, and Equipment for Ceremony in our Chapel or Other Facility $400 D. Use of Equipment and Staff for Graveside/Committal Service $350 E. Use of Facilities and Staff for Viewing and Identification of Deceased..$250 Transportation Transportation charges contained in this General Price List apply to local area only (within 30-mile radius of our funeral home). An additional charge of $2.50 per mile, one way, will be for mileage exceeding 30 miles for each vehicle used. A. Transfer of deceased into our care $400 B. Use of Hearse for Transportation to Place of Disposition $350 C. Use of Limousine $150/hour-7 passenger (Or Any Portion of an Hour) D. Use of Flower Van / Utility Vehicle for Floral Offerings & Equipment $175 E. Airport Pick Up/Drop Off.$200 2

3 The goods and services described here are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. (Without Ceremony)..$500 Immediate Burial Our fee for a direct burial (without ceremony) includes: basic services of funeral director and staff; a proportionate share of overhead costs; transfer/removal of remains from place of death into our care; transportation to the cemetery.$3300 Forwarding of Remains This charge includes: basic service charge, initial transfer of the deceased from place of death to our funeral home, embalming, other preparation of the body, local transportation (airline or other shipping charges are not included)...$3500 Receiving of Remains This charge includes: basic service charge, care of the deceased and local transportation of the deceased to place of disposition....$2495 3

4 Funeral Service Package Price List These packages represent the most common types of service provided by Ruebel Funeral Home. Ruebel Funeral Home has taken the opportunity to combine these services and show them as a package price. There are services not included in the package price and are listed as optional. Third party items are not included in the package price unless otherwise listed. All items can be found with further explanation on our general price list. Services listed in the packages are not interchangeable and no credit will be given in the event they are not used. Prices do not include casket, outer burial container, or weekend/holiday services. Traditional Funeral Service Package $5550 This package includes the Basic Services and Overhead of Funeral Director and Staff, Embalming, Other Preparation of Body, Use of Stateroom between hours of 8:30 and 4:30, Use of Staff for Funeral Ceremony in our Chapel or Other Facility, Use of Staff for Graveside Committal Service, Transfer of Remains to Funeral Home Care, Use of Hearse to Place of Disposition, Use of Utility Vehicle for Floral Offerings and Memorial Package (includes personalized blue or burgundy register book, 100 memorial folders, acknowledgement cards and online memorial.) Optional Services Not Included in Traditional Funeral Service Package Use of Facilities and Staff for Evening Visitation or Ceremony after 4:30 P.M $550 Use of Limousine..$150 per hour 7 passenger Traditional Graveside Service Package $5150 This package includes Basic Services and Overhead of Funeral Director and Staff, Embalming Other preparation of Body, Use of Stateroom between hours of 8:30 and 4:30, Use of Staff for Graveside Committal Service, Transfer of Remains to Funeral Home Care, Use of Hearse to Place of disposition, Use of Utility Vehicle for Floral Offerings, and Memorial Package (includes personalized blue or burgundy register book, 100 memorial folders, acknowledgement cards and online memorial.) Optional Services Not Included in Traditional Graveside Service Package Use of Facilities and Staff for Evening Visitation or Ceremony after 4:30 P.M $550 Use of Limousine..$150 per hour 7 passenger Basic Funeral Service Package $4450 This package includes the Basic Services and Overhead of Funeral Director and Staff, Embalming, Other Preparation of Body, Use of Staff for Graveside Committal Service, Transfer of Remains to Funeral Home Care, Use of Hearse to Place of Disposition, Online Memorial. This Package Includes No Viewing, No Visitation, No Limousines, No Utility Vehicle and Only an Online Memorial There are No Optional Services Connected With This Package. 4

5 Terms and Payment Options OPTION I: Cash or Check OPTION II: Visa / Master Card / Discover Ruebel Funeral Home accepts Visa / Master Card / Discover / American Express (plus surcharge) for payment on funeral accounts. OPTION III: Insurance Assignment Proceeds from one or more life insurance policies, that can be verified, can be assigned to Ruebel Funeral Home, if desired, in an amount sufficient to satisfy the funeral account. A fee of 5% of the services and merchandise will be collected for this service.
