Whether their choice is cremation or burial, those families tell us they appreciate our levels of professional care and find our prices to be much lower than they found elsewhere. Although we believe, and have been told, our services are more memorable than most, they are in no way less than you are accustomed to from other local funeral providers. 2 Page 2 GENERAL PRICE LIST These prices are effective as of February 1, 2015 The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. We do not warrant or claim that any vault or casket is air and/or water tight. Please refer to the manufacturer s warranty for full details. FUNERAL HOME STAFF SERVICES Basic Services of Funeral Director and Staff.....$995.00** Our fee for the services of funeral director and staff includes, but is not limited to, staff to respond to initial request for service; arrangement conference with family or responsible parties; arrangement of service; preparation and filing of necessary authorizations and permits; recording vital statistics; preparation and placement of obituary notices; and coordination with those providing other portions of the funeral, e.g., cemetery, crematory, vault companies and others required, as well as, staff assistance prior to, during and following the services. Also included in these charges are overhead expenses relative to our facility such as sheltering of remains, professional licensing, legal and accounting fees, insurance, building and utility expenses, parking lot and grounds, maintenance, taxes, equipment, furnishings, inventory costs, recordkeeping, and secretarial and administrative expenses. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains.) PREPARATION OF THE BODY Embalming... $500.00** Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. 4 Page 4 OTHER SERVICES Forwarding of Remains to Another Funeral Home... $2, Our charge includes: basic services of funeral director and staff; a proportionate share of overhead costs; removal of remains; embalming or other preparation of remains, if relevant; and local transportation. Receiving Remains from Another Funeral Home... $1, Our charge includes: basic services of funeral director and staff; a proportionate share of overhead costs; care of remains; transportation of remains to funeral home and to cemetery or crematory. Direct Cremation... $1, to $ Our charge for a direct cremation (without ceremony) includes: basic services of funeral director and staff; a proportionate share of overhead costs; removal of remains; transportation to crematory; necessary authorizations; and cremation if relevant. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are a fiberboard container or an unfinished wood box and are the minimum acceptable by crematory. A. Direct cremation with container provided by the purchaser... $1, B. 5 Page 5 OTHER CASH ADANCE ITEMS Cash advance items are those charges that may appear on our invoice; however, are charges that are paid on your behalf as a convenience to you. These charges are the direct charges of the vendor and are not marked up by our funeral home unless stated otherwise. Cemetery Fees, Vault Setting Fee, Opening & Closing of the Grave Certified Death Certificates, Obituaries Flowers and Stone Cutting Minister and Military Honorarium Caterer MONUMENTS Monuments are available at through our company, Woodlawn Monuments. We can have any custom design made to your specifications and set in any cemetery. See us before you make any purchases. Since we handle our own monument engraving in house, all pictures, graphics and special designs can be handled through us. PAYMENT POLICY We realize that losing someone you love is a devastating experience. It can be traumatic and influence the decisions that must be made. It is our intention that your decisions reflect your personal wishes and remain within your budget. We offer pricing considerably less than our competitors and are able so because we take advantage of early pay discounts with our suppliers. For us to be able to offer you this pricing, on all funerals and cremations that are not prefunded, we kindly ask that full payment be received prior to the service. We offer a 2% discount on our charges (side A of the contract) if full payment by cash or check is received prior to the funeral. We will accept credit card or insurance assignment to cover the full balance, but please understand these are handled by a third party, and they charge a 3% fee of the total cost of the funeral or on insurance assignments a minimum fee of $175. This fee will be added to your total bill. For families using life insurance, policies must be verifiable and assignable coverage on the life of the deceased. Contestable policies cannot be used as payment for funeral expenses as many times they can take months to complete claims process. We must have full cooperation of the parties involved: beneficiary, employers and the insurance company. At all Woodlawn Family Funeral Centre locations, our goal is to serve you with respect, dignity and professionalism regardless of race, nationality, religious beliefs or financial condition. We will assist you in all aspects of the funeral process from pre-planning or through your needs at the time of a death. If there are financial difficulties, please advise us and we will make every effort to accommodate you. Financial need selections such as Medicaid and Township Trustee funerals are available to qualified individuals, but we are limited to service guidelines the funding agencies have established. We also offer financing options with qualification guidelines.
