SUMMER PROGRAM APPLICATION Welcome from everyone at MarineQuest! We are so excited about our programs this year and we are looking forward to seeing all of you for another fun summer of marine science adventures. All students 10 and up (Coast Trek, Ocean Trek, TechnOcean Jr, Shore Shots, ArtSea, Ocean Lab, Surf-n-Science, Surf-n-Science Groms, Ocean CheMystery, Sea GEMS, Sea GEMS Jr., Sea LAB, Sea Sports Science Olympiad) are required to fill out the MarineQuest Summer Program Application. This application is designed for both student and parent, so let us explain how it works Section I Student and Parent/Guardian information. Be sure to complete the entire form, especially your contact information. All student application acceptance e-mails will be sent to the e-mail addresses provided. Make sure your spam blocker will allow e-mails from addresses ending with @uncw.edu (for example, doej@uncw.edu). Section II The Student/Parent Questions. The first two Safety & Behavior questions are to be answered by the student after they have discussed them with the parent. The last four Science & Summer Program questions are to be answered by the student only. Section III The Student Code of Conduct. This must be signed by both student and parent. Section IV - Teacher Recommendation. This form is to be distributed by the student to their teacher. Make sure you fill in your name and the summer program title on the form before you give it to your teacher. Teachers should mail this form directly to MarineQuest or place in a sealed envelope and give it back to the student to mail in (Note: teachers may also fax this form in). MarineQuest Tip: Many students provide their teacher with a self addressed stamped envelope for mailing convenience. Please Note: A completed application does not register you for your summer program. All applications will first be reviewed by the MarineQuest Staff and upon acceptance, you will receive an e-mail explaining the final registration step. Enrollment is based on a positive review of this application by the MarineQuest staff and upon a first-come, first-serve basis. There are four components: I) Student and Parent/Guardian Information, II) Student/Parent Questions III) Student Code of Conduct and IV) Teacher Recommendation. III) UNCW MarineQuest Summer Program Application Student Code of Conduct To ensure that everyone has a positive experience while participating in a camp or program located at facilities of the University of North Carolina Wilmington ( UNCW ), students attending the Program are expected to behave in a disciplined, responsible and respectful manner when on or off the UNCW campus. Failure to follow these rules and regulations will subject the students to immediate disciplinary action as described more fully below. As a condition of the student s participation, both parents and students must agree that neither UNCW (nor the instructors, staff members, resident assistants and desk staff) are guarantors of the safety of the students, particularly for the students unsupervised time and instances where the students fail to follow applicable rules and regulations. Students will: - respect Program staff requests and follow all of their directions with a positive attitude; - respect all safety guidelines required by Program staff including prescribed locations and boundaries; - respect all curfew, wake-up and lights-out policies (residential students); - wear appropriate clothing and closed-toes shoes during all field activities; - respect the differences in other people, make an effort to include everyone and not behave in a manner that may hurt another student or staff member s feelings; - pick-up after myself (at all locations) and throw all trash away; - respect other people s personal property; - treat all animals (invertebrate and vertebrate) and their environments with respect; - respect all UNCW property including equipment, boats, buildings and the campus; - be respectful of UNCW students and staff when in university buildings; - let my Program counselor know if I am not having a good time; - immediately bring disagreements to the attention of Program staff; - behave in an age-appropriate manner. Students will not: - fight, tease or bully other students using any part of their body, gestures or words; - put themselves, other students or Program staff at risk - go anywhere without the permission and accompaniment of counselors; - go into water above knee-level unless given permission while in a lifeguard s presence - use derogatory terms or offensive language at any time; - bring inappropriate items to program (weapons, drugs, cigarettes, R-rated materials, etc) - enter the sleeping quarters or shower areas of the opposite sex (residential students) - alter, damage, destroy, and/or deface the MarineQuest brand and/or logo (including, but not limited to, tee-shirts, posters, and signs) - open windows or prop open locked doors; - bring any type of electronic equipment (including but not limited to: tablets, ipods, computers) with the exception of cell phones which subject to the MarineQuest cell phone policy which can be found in the Rule and Regulations page under cell phone policy; - not use cell phones for any purpose other than calling/texting (cell phones may not be used to take pictures/videos; access internet, Facebook, Twitter; post to social media) or in any way that violates the cell phone policy terms and conditions. Parents will: - provide all necessary information to ensure that their child has a safe and positive program experience. Parents will not: - interact with Program staff in a threatening/abusive manner at any time (in person or on the phone) or their child will be dismissed from the Program without tuition/fee refund. If I chose not to follow the Student Code of Conduct, I acknowledge (with my signature below) that on my: First offense - I will be given a verbal warning explaining the expected behavior; Second offense - I will be removed from the activity and my parent(s) will be called to discuss my behavior. Third offense - my parent(s) will be called notifying them of my dismissal from program. Furthermore, I understand that MarineQuest has a zero tolerance policy for some actions. In these cases students will be immediately dismissed and will not receive a program refund.
