FRAMEWORK JOB DESCRIPTION Band 6 Framework is a registered charity, Company Limited by guarantee and Registered Social Landlord providing a variety of supported accommodation and tenancy support for homeless and vulnerable people in Nottinghamshire. Salary 25,783-34,530 (Agenda for Change Band 6) Hours Line Management This is a full-time post. The normal hours of work for a full-time Framework employee are 37 hours per week. Framework operates flexible working at its services. This post will require some evening and weekend working. Line management for this post will be provided by the Clinical lead for Alcohol and Drug Services and will include supervision and support, appraisal, career development and training plans Main Purpose of the Job The Corner House in Lincoln is a purpose built 15 bed unit specialising in alcohol detox and the rehabilitation of homeless men and woman. The post holder will deliver training to workers in primary care and other settings to provide education on identification and brief advice, alcohol awareness and the physical effects of alcohol misuse. The post holder will also work alongside the Health Promotion Specialist for Alcohol at various events in Lincoln City. Page 1
Key responsibilities 1 Service delivery 1.1 Manage alcohol detoxes within the Corner House service 1.2 To promote the service to the NHS Trust, Primary Care and other relevant agencies Including Framework Housing Association core services. 1.3 To work in conjunction with existing roles and services such as Health Promotion and Alcohol Liaison 1.4 To be aware of NHS Lincoln targets to reduce alcohol related hospital admissions and presentations to the Emergency Department and incorporate these priorities in to every day working practice. 1.5 To Provide patients with relevant information and ongoing support as necessary 1.6 To support and be actively engaged in the research and evaluation of the service 1.7 To monitor all contacts and activities and submit to the data base weekly to ensure all Targets are being met as set by commissioners including NATMS if appropriate 2 Service development 2.1 Develop effective relationships with key stakeholders in Primary Care and operate in a way that promotes collaborative working. 2.2 Develop and facilitate education and training opportunities to enhance the knowledge and skills of clinical staff working with this client group. 2.3 Develop alcohol education services in line with local strategies 2.4 Ensure that service user and carer views are incorporated into the development of the service 2.5 Participate in the setting of standards and writing of policies and protocols 2.6 Be aware of latest developments and guidance in relation to substance misuse and mental health 2.7 Deliver ongoing monitoring as required by the service level agreement Page 2
2.8 Attend meetings, forums and working groups as appropriate Band 6 2.9 Maintain appropriate records and case notes in line with the service outcomes and assist with the development of client held records 3 Liaison and communication 3.1 To liaise with GP s, CMHT s, Substance Misuse Services, ED and Inpatient Units as required 3.2 To Liaise with the Lincoln City Substance Misuse Service and other service s to ensure there is no duplication of services to patients within Nottingham City. 3.3. To engage in team meetings and development days to maximise the expertise of the Service 3.4 To engage in clinical supervision sessions and reflective practice to ensure patient safety. 3.5 To liaise with the Clinical Lead as required 3.6 To communicate with commissioners on all aspects of the service 3.7 Ensure that GP s and other referrers are aware of what on going care has been put In place for their patients within two working days 3.8 Engage service users and carers in all aspects of the service. 4 Training 4.1 To deliver alcohol training to Primary Care services and others as per commissioners targets. 4.2 To offer ongoing support and training to those agreeable to the delivery of identification And brief advice to patients. 4.3 To contribute to existing training programmes within primary care 4.4 To disseminate the best practice manual to staff in primary care settings 4.5 Identify and assist in the development of appropriate recourse materials for use in Primary Care and other settings 5 Maintenance of records 5.1 To provide Framework HA and partners with statistical information as Required including NATMS as appropriate. 5.2 Participate in clinical audits, training evaluation and research 5.3 Contribute to the implementation of the NMC Clinical Governance Strategy 5.4 Record working practice and monitor outcomes 5.5 Provide quarterly reports and an annual evaluation of the effectiveness or otherwise of the service 5.6 Adhere to the Framework Confidentiality of Service User Information Policy and Procedure in line with the Organisations Clinical Governance Agenda. 6 Professional development 6.1 To maintain professional status by complying with the appropriate requirements for Yearly registration and continuing professional development. 6.2 To constantly update knowledge and skills in order to deliver evidence based assessment and treatment. 6.3 Access training in line with personal development plan and mandatory training requirements. 6.4 Participate in regular clinical and managerial supervision. Additional clinical supervision can be sought from other areas if desired by the post holder GENERAL RESPONSIBILITIES To attend management forums and contribute to working parties to develop Framework s policies, procedures and provision.
