2 Planning Section 1: Commissioning for positive outcomes Identification of key priorities following needs assessment relating to commissioning system: Evaluate the implementation of HORIZON Review the year old service which is being delivered by the Hub and monitor the effectiveness of the service and ensure that it does not have a detrimental impact on the young people s service To support the strengthening of the Blackpool Service User Group Evaluation of the Inpatient Detox framework for preferred provider list Set specific targets around successful treatment completions for both drug and alcohol treatment Review BBV pathways, improve access to testing & vaccination across the treatment system and explore new harm reduction interventions to further prevent the spread of BBV and drug and alcohol related deaths Implement the pharmacy testing campaign and evaluate the effectiveness of this intervention Review pharmacy role of those delivering needle exchange and develop audit tool to further engage the treatment naïve population with offer of testing and vaccination Review the pharmacy service level agreement and pricing structure for both supervised and needle exchange Support the Community Budget Pilot to ensure children are protected and families are supported. Preparation work for the transition of Public Health in to the Council. Work with Providers to support them during the transition and moving them from NHS Contracts to Council contracts Support the development of an Abstinent Bar in Blackpool Offer Clients choice Young People agenda remains a priority Ensure that a single computerised case management system is in place Expected outcomes 2012/13: An effective integrated Drug and Alcohol treatment System Effective year old service Continuation of clear and transparent joint commissioning processes/systems for children, adult and family services Clients successfully complete their treatment journey and continue with sustained recovery Providers deliver the same standard and quality of service To ensure that all partnerships work collaboratively within the HORIZON consortia. 3 Improvement in uptake of testing and vaccination for BBVs within both services and pharmacies Improved reporting on all systems. Achieve the required TOPS, DirWeb, DMIS and NDTMS targets, so that information can be utilised as part of the needs assessment process Improving the care pathways for young people A successful service user forum, that support delivery of harm reduction and recovery message in the community Improve successful completions Continued achievement of engaging clients in effective treatment Increased awareness of Contraception and Sexual health due to risky behaviours All providers working on a single computerised case management system Prescription generation at each clinical site via the single computerised software package. To continually review treatment centres who offer detoxification and residential rehabilitation to ensure they meet the requirements of the Commissioners and the individual needs of the clients and to devise an audit tool in line with NICE Guidance and the Care Quality Commission. Develop a system to monitor the outcomes of clients who have attended detoxification and rehabilitation to review the effectiveness of Tier 4 providers and review unplanned discharges to be reported to the Joint Commissioning Group on a quarterly basis. 4 Public Health leads to monitor the uptake of residential rehabilitation and identify any blockages for clients accessing rehabilitation. To review packages of care that meet the needs of the clients i.e. reduced length of time within rehabilitation and structured day support given to clients who wish to be supported in the Community The partnership s ambition for a sustained recovering community will not only focus on a person s recovery, but on the reintegration of an individual s holistic needs by establishing links with quality housing, education, employment and the rebuilding of family and community networks. The partnership need to continue to build on the recovery agenda by exploring new models of working which will incorporate the four domains whilst establishing a vision for the population within Blackpool, mutual aid groups being an integral part of this agenda, and to support service users who are developing social enterprise initiatives e.g. Abstinent bar To refine the Key Performance Indicators so that they are specific for an outcome driven service in line with local and national strategy for both adult and young peoples treatment Continue to monitor the Community Contracts and Service Level Agreements with the providers to ensure that they have effective and robust governance arrangements in place Review the Practices of the Providers through NDTMS, TOPS, DIRWEB and the DIP dashboards to ensure that there is robust and accurate monitoring in place. 5 The partnership will ensure the continued alignment of DIP, DRR and ATR with the local PPO scheme and expand further alignment with Integrated Offender Management The will work with the Providers to ensure that they improve the reporting mechanisms for NDTMS, TOPS, DIRWEB, DMIS and quality review reports. These are now all included within the Community Contracts and Service Level agreements and the HORIZON treatment system and the Hub Young Peoples service will be monitored on its performance The will work closely with the Providers to ensure that accurate information is recorded on all reporting systems. To support service users in delivering advocacy and peer led harm reduction training Continue to engage with the Service User Group, parents and young people in the consultation and planning process within the Commissioning Cycle via Blackpool LINK service BSafe Partnership/ JCG/Criminal Justice Forum/ for for /JCG/Criminal Justice for for for Continue Identify where service user representation should be integrated within the partnership structures and ensure that all providers promote the service user forum. 6 Support the Development of an abstinent cafe bar within Blackpool to help create a more visible recovery community for To Commission a Tier 1 life style Coaching Officer to raise alcohol awareness for To advise, explore and lobby for wider determinants to reduce alcohol harm i.e. minimum pricing and alcohol awareness. 8 Planning Section 2: Maintaining and improving access to treatment Identification of key priorities following needs assessment relating to access to the drug treatment system: Continue engaging clients in effective treatment. Evaluation of the year old service Reducing the length of stay within Service Develop the work force and ensure service users aware of all the changes with the treatment system Ensure that all monitoring across the new treatment service is robust and effective Support the Providers implementing and delivering the new treatment system Reduction in alcohol related hospital admissions Ensure that young people receiving specialist treatment within 15 working days of referral Continue to provide flexible service provision including drop in facilities for young people Expected outcomes 2012/13: Equitable access to HORIZON for all clients Needs of Diverse Groups are met Recovery culture is embedded with in all staff and service users in to further develop the recovery Community Clients are not retained in service for more than 5 years Expected outcomes : Clients who are considered appropriate should not be retained in service for more than 2 years, with the aim to reduce the number of clients who are on long term methadone. Robust monitoring of clients to ensure that the needs of children are identified through the Adult Treatment Services. Criminal Justice Ensure all Provider Staff have an awareness of the hidden harm agenda, every child matters and that all staff are trained in child protection procedures. 11 Planning Section 3: Delivering recovery and progress within treatment Identification of key priorities following needs assessment relating to recovery and effectiveness of the drug treatment system: There is a need to improve on TOPS reporting and to review an effective alcohol recording system to ensure that the data is robust, accurate and represents HORIZON. To improve successful completions and reduce the number of unplanned discharges within HORIZON Increase the number of young people leaving treatment in a planned way Continue to reduce drug related death and alcohol related deaths and improve the engagement of clients for all screening for BBV, and Hep B vaccinations. 13 To work with the providers to improve the uptake of Hep B vaccines Public Health lead for Harm Reduction/Horizon Review the returns policy to ensure there are an increase in both Community Pharmacists and the Specialist Service to prevent injury and the spread of blood bourne viruses Explore other harm reduction initiatives to prevent drug related deaths, alcohol related deaths and prevent the spread of BBV Continue to provide overdose awareness & first aid training for service users and carers on monthly basis Public Health lead for Harm Reduction Public Health leads Harm reduction Harm Reduction/Horizon Review the Naloxone Scheme and the effectiveness of the scheme HORIZON to work collaboratively with the wider partnership to improve access to social support and wrap around services in primary care, education, employment, housing and accommodation. Harm Reduction Moving Forward and Recovery/Horizon Continue to develop opportunities for clients to achieve their full potential in the workplace through personal, education and skills development. Moving Forward and Recovery/Horizon Pilot a Dry House facility in Blackpool and review the effectiveness of such a pilot Substance Misuse Pilot a non-prescribed community detoxification service for clients who do not wish to receive substitute prescribing and explore options for clients to have a direct route to inpatient detoxification. 14 Continue to develop the Safe Haven project and monitor its effectiveness Substance Misuse Maintain and improve the Alcohol Liaison Service provision to reduce hospital admissions and develop KPI s to review the effectiveness Continue and embed the hospital outreach workers, and review their effectiveness within the Horizon treatment system and develop KPI s to report the effectiveness of the service Support the development of the newly qualified nurse prescriber for young people services and embed within the Horizon treatment system Substance Misuse Substance Misuse Substance Misuse/Horizon Development of Nurse Prescribing within Horizon to reduce the pressure on the medical staff clinics Substance Misuse/Horizon Ensure the availability of brief intervention training for front line professionals and volunteers on the impact of on individual young people and their families. 15 Continue to ensure that children who are looked after, young offenders, pregnant young women are a priority accessing service provision. Continue with the successful collaboration between the Hub and CAMHs for young rs with mental health issues Support the development of Recovery champions throughout the horizon treatment system giving every individual an opportunity to be involved. The responsibility will lie with the newly appointed peer mentor coordinator to implement recovery champions in Blackpool. Recovery champions will spread the message throughout the treatment system that recovery is a worthwhile goal while being involved in the recovery movement nationally.
