1 COMPANY VEHICLE POLICY Plicy The purpse f this Plicy is t ensure the safety f thse individuals wh drive cmpany vehicles. Vehicle accidents are cstly t ur cmpany, but mre imprtantly, they may result in injury t yu r thers. It is the driver s respnsibility t perate the vehicle in a safe manner and t drive defensively t prevent injuries and prperty damage. As such, the Cmpany endrses all applicable state mtr vehicle regulatins relating t driver respnsibility. The Cmpany expects each driver t drive in a safe and curteus manner pursuant t the fllwing safety rules. The attitude yu take when behind the wheel is the single mst imprtant factr in driving safely. All vehicles are equipped with a GPS system fr detectin purpses. All Drivers Must Be Apprved Prir t perating cmpany vehicles. Driver Eligibility Cmpany vehicles are t be driven by authrized emplyees nly, except in emergencies, r in case f repair testing by a mechanic. Spuses and ther family members are nt authrized t drive the Cmpany vehicle. Any emplyee wh has a driver s license revked r suspended shall immediately ntify management, and discntinue peratin f the cmpany vehicle. Failure t d s may result in disciplinary actin, including terminatin. The use f ALL Cmpany Vehicles Are SMOKE-FREE f First and Secnd Hand Smke. All accidents, regardless f severity, must be reprted t the plice and t management. Failing t stp after an accident and/r failure t reprt an accident may result in disciplinary actin, including terminatin. Drivers must immediately reprt all traffic citatins and summnses received fr vilatins t management. Mtr Vehicle Recrds will be rdered peridically t assess emplyees driving recrds. An unfavrable recrd will result in a lss f the privilege f driving a Cmpany vehicle. Eligibility t Operate Cmpany Vehicle ALL TYPE A VIOLATIONS (as defined belw) WILL RESULT IN TERMINATION OF DRIVING PRIVILEGES FOR EMPLOYEES AND WILL DISQUALITY ANY POTENTIAL DRIVER EMPLOYEES. ANY DRIVERS (EMPLOYEES OR APPLICANTS) SHOWING ONE OF THE FOLLOWING WILL BE RESTRICTED FROM DRIVING COMPANY VEHICLES: One (1) r mre type A Vilatins in the last 3 years Three (3) r mre accidents (regardless f fault) in the last 3 years. Three (3) r mre type B Vilatins in the last 3 years. Any cmbinatin f accidents and type B Vilatins which equal Fur (4) r mre in the last 3 years.

2 Type A Vilatins: Driving While Intxicated Driving While Under the Influence f Drugs Negligent Hmicide Arising ut f the use f a Mtr Vehicle (grss negligence) Operating During a perid f Suspensin r Revcatin Using a Mtr Vehicle fr the Cmmissin f a Felny Aggravated Assault with a Mtr Vehicle Operating a Mtr Vehicle Withut the Owners Authrity (grand theft) Permitting an Unlicensed Persn t Drive Reckless Driving Speed Cntest (racing) Hit and Run (Bdily Injury r Prperty Damage) Type B Vilatins: All Mving Vilatins nt listed as type A Vilatins Driver Safety Rules The use f a cmpany vehicle while under the influence f intxicants and ther drugs is frbidden and is sufficient cause fr discipline, including terminatin. N driver shall perate a cmpany vehicle when his/her ability t d s safely has been impaired by illness, fatigue, injury, r prescriptin medicatin. All drivers and passengers perating r riding in cmpany vehicles must wear seat belts, even if air bags are available. N unauthrized persnnel (nn-cmpany emplyees) including family members and/r friends are allwed t ride in cmpany vehicles. Drivers are respnsible fr the security f Cmpany vehicles assigned t them. The vehicle engine must be shut ff, ignitin keys remved, and vehicle drs lcked whenever the vehicle is left unattended. If the vehicle is left with a parking attendant, nly the ignitin key is t be left. Head lights shall be used ½ hur after sunset and ½ hur befre sunrise, r during inclement weather r at any time when a distance f 500 feet ahead f the vehicle cannt be seen clearly. All ther state f lcal laws must be beyed. Defensive Driving Rules Drivers are required t maintain a safe fllwing distance at all times. T estimate yur fllwing distance, pick a statinary bject ahead f yu. As the vehicle in frnt f yu passes the bject, begin cunting 1001, 1002, 1003, etc. Until yu reach the same bject. This cunts the number f secnds between yu and the vehicle ahead f yu. Drivers f passenger vehicles shuld keep a tw-secnd interval between their vehicle and the vehicle immediately ahead. During slippery rad cnditins, the fllwing distance shuld be increased t at least fur-secnds. Drivers f heavy trucks shuld keep a minimum f a three-secnd interval when nt carrying carg; and at least fur-secnds when fully laded. Fllwing distance shuld als be increased when adverse cnditins exist.

3 Drivers must yield the right f way at all traffic cntrl signals and signs requiring them t d s. Drivers shuld als be prepared t yield fr safety s sake at any time. Pedestrians and bicycles in the radway always have the right f way. Avid driving in ther driver s blind spts; attempt t maintain eye cntact with the ther driver, either directly r thrugh mirrrs. Drivers must hnr psted speed limits. In adverse driving cnditins, reduce speed t a safe perating speed that is cnsistent with the cnditins f the rad, weather, lighting, and vlume f traffic. Tires can hydrplane n wet pavement at speeds as lw as 40 MPH. Turn signals must be used t shw where yu are heading; while ging int traffic and befre every turn r lane change. When passing r changing lanes, view the entire vehicle in yur rear view mirrr befre pulling back int that lane. Be alert f ther vehicles, pedestrians, and bicyclists when appraching intersectins. Never speed thrugh an intersectin n a cautin light. Apprach a state green light with yur ft pised ver the brake t reduce yur reactin time shuld it be necessary t stp. When the traffic light turns green, lk bth ways fr ncming traffic befre prceeding. When stpping behind anther vehicle, leave enugh space s yu can see the rear wheels f the car in frnt. This allws rm t g arund the vehicle if necessary, and may prevent yu frm being pushed int the car in frnt f yu if yu are rear-ended. Avid backing where pssible, but when necessary, keep the distance traveled t a minimum and be particularly careful. Check behind yur vehicle. Operatrs f heavy trucks shuld walk arund their vehicle befre backing and/r have smene guide yu. Back t the driver s side. D nt back arund a crner r int an area f n visibility. What t D In Case f an Accident In an attempt t minimize the result f an accident, the driver must prevent further damages r injuries and btain all pertinent infrmatin and reprt it accurately. Call fr medical aid if necessary. All n the jb emplyee injuries are t be reprted t Human Resurce t file a Wrkers Cmpensatin claim. Vehicle Accident Descriptin frms shuld be in the glve cmpartment at all times. This shuld be given t yur Supervisr and the Risk Manager. Secure accident scene pull nt shulder r side f rad, redirect traffic, set up rad flares/reflectrs, etc. Call the plice. All accidents, regardless f severity, must be reprted t the plice. Recrd names and address f driver, witnesses and ccupants f the ther vehicles and any medical persnnel wh may arrive at the scene. Gather infrmatin. Pertinent infrmatin t btain includes: License number f ther drivers Insurance cmpany names and plicy numbers f ther vehicles Make, year, mdel f ther vehicles Date and time f accident Overall rad and weather cnditins Draw a diagram f the accident scene and nte the street names and lcatins f traffic signs, signals, etc.

4 D nt discuss the accident with anyne at the scene except the plice. D NOT accept any respnsibility fr the accident. DON T argue with anyne. Prvide the ther party with yur name, address, phne number, driver s license number, and insurance infrmatin. Immediately reprt the accident t yur supervisr. Prvide a cpy f the accident recrd and/r yur written descriptin f the accident t yur supervisr ASAP. Vehicle Maintenance Prper vehicle maintenance is a basic element f any fleet safety prgram, nt nly t ensure a safe, rad wrthy vehicle, but als t avid cstly repair expenses and unexpected breakdwns. Registratin and Inspectin is the respnsibility f the assigned driver. Be sure there is a current insurance card in the vehicle. In additin t inspectins that may be required by law fr passenger vehicles, rutine inspectins f critical items, such as brakes, lights, tires, wipers, etc., must als be cmpleted by drivers f passenger vehicles. The vehicle shuld be cleaned (interir & exterir) regularly t help maintain its gd appearance fr yu and the Cmpany. A clean vehicle makes a gd impressin n custmers. The vehicle manufacturer s preventative maintenance schedule shuld be referenced and clsely fllwing regarding recmmended maintenance and time intervals. Respnsibilities fr maintenance The assigned Cmpany driver is respnsible fr scheduling all regular maintenance and repairs. Prir apprval fr maintenance r repairs is nt required unless the estimated repair cst is greater than $1,000. Physical damages caused by any kind f accident must be reprted t Risk Management prir t repair fr insurance reprting purpses. Preventative maintenance and repairs shall be perfrmed by an authrized dealership, preferably where the vehicle was purchased.

5 Cell Phnes & Other Electrnic Equipment Cell Phnes Driving can be difficult enugh even when yu cncentrate cmpletely n the rad. But driving while yu dial a phne r balance it t yur ear can be distracting and ptentially dangerus. If nt used prperly, drivers with car phnes are a danger t themselves and everyne n the rad. Driving safely must take precedence ver phning. When n the rad, yu shuld cncentrate n safe and defensive driving, nt n making phne calls. General Guidelines All cell phnes used fr cmpany business must be perated with either a hands-free speakerphne ptin r hands-free headset ptin while perating a vehicle. Make sure yu are prperly trained and familiar with the peratins manual f yur cell phne and hands-free unit befre yu begin driving. D nt frget t cnnect the hands-free devices prir t driving. Assess the traffic situatin befre placing r receiving calls. Be fully aware f rad and vehicle distractins. If driving in a hazardus zne, heavy traffic r inclement weather, d nt use the cell phne. Allw the vice mail feature t answer the call and return the call under safer cnditins. Our cmpany recmmends dialing phne numbers while the vehicle is stpped. Frequently called numbers shuld be stred within the phne s memry t minimize dialing while driving. While n the phne, drive in the slw traffic lane in case yu decide t pull ver t cmplete the call. Avid changing lanes r passing ther vehicles while n the phne. Yu must pull ver int a safe stpping place if the call requires yu t write dwn infrmatin, r requires advanced cgnitive tasks such as negtiatin discussins r ther stressful tpics. Always keep the call duratin t a minimum while perating a vehicle. Nte: A cellular phne is a respnsibility and a privilege. Nt perating by these general guidelines is cnsidered an unsafe act and may result in disciplinary actin and/r lss f cell phne privileges. Other Electrnic Equipment Our cmpany cnsiders the use f all ther electrnic equipment including laptp cmputers and tablets an unsafe act while perating a vehicle. The vehicle must be parked in a safe stpping place t use this equipment. Since we cnsider speeding an unsafe act, the use f all radar detectrs is banned in cmpany wned vehicles and persnal vehicles while being driven n cmpany business. Vilatin f these plicies may result in suspensin f cmpany driving privileges I acknwledge that the infrmatin cntained in the Cmpany s Vehicle Fleet Safety Plicy has been reviewed with me and a cpy f the plicy and driver rules have been furnished t me. As a driver f a cmpany vehicle, I understand that it is my respnsibility t perate the vehicle in a safe manner and t drive defensively t prevent injuries and prperty damage.
