2 Overview of the Surgery We are a 4 partner training practice, working from purpose-built premises in the pleasant mid Norfolk market town of East Dereham. Just twenty minutes drive from Norwich, we are also within easy reach of Cambridge, Bury St. Edmunds and Ipswich. The practice is currently part of South Norfolk CCG. Our surgery team comprises a full complement of ancillary staff, together with attached Community Nurses, Midwives and Health Visitor. We also have an Emergency Care Practitioner working with us for four days each week offering in-house appointments and home visits. We are fully computerised, using TPP SystmOne network with integrated Appointment System together with an extensive suite of ancillary Microsoft (and other) programmes. Our Financial Team uses Sage Accounts and Iris GP Payroll programmes. The practice operates registration, item of service and pathology service links and is comprehensively furnished with up to date office and clinical equipment. Our Practice currently uses IC24 for out of hours services between the hours of 6.30 p.m. and 8.00 a.m. weekdays and all weekend. Our patient list of approximately 9,000, falls within a fairly compact area - extending to an approximate 4.5 mile radius of the town centre. The practice is open every weekday between 8.00 a.m. and 6.30 p.m. Throughout the day there is always a nurse on duty and they are able to assess any emergencies which may arrive when there is no doctor on the premises. The daily duty doctor can be contacted by mobile telephone throughout the day. Our practice list is currently closed to new registrations. We are a training practice and we regularly have medical students in the practice as a result of the collaborative work between ourselves and the UEA Medical School. From time to time we also have trainee practice nurses or administrative staff working on the premises. All staff and partners are encouraged to attend external courses and we hold our own in-house sessions on a regular basis for all staff. In past years we have held five residential training weekend attended by all members of the practice team. Since April 2004 we have offered a range of enhanced and private services from our premises and we opened a dispensary in May 2004 that enabled us to dispense to approximately 1000 of our patients (depending upon where they live in the practice area). The dispensary was superceded by a Pharmacy in 2011 and this is operated by an external company and open for one hundred hours per week. We had our first CQC inspection in March 2014 and were randomly selected from other South Norfolk CCG practices to have a second, new-style CCG inspection in October Both inspections were passed with flying colours and were a reflection on the excellent staffing, facilities and systems that are in place at Theatre Royal Surgery. We feel that we are a happy, efficient and caring practice where all members of the team are encouraged to use their initiative and particular skills to ensure that we offer the highest possible standard of care to our patients.

3 Job Specification We require a motivated and enthusiastic individual to be part of the clinical team. They will possess a pleasant, calm and mature disposition and be able to work enthusiastically as part of a very busy team, using their initiative and confidence to provide a first class service to our patients. We expect the person appointed to be well spoken, of smart appearance they must be conscientious, adaptable and have a good sense of humour. The successful candidate will hold a nurse prescribing qualification. To prevent a conflict of care between employer and possible medical need, the successful candidate will be required to register with another practice (if currently registered with us). Line Manager Responsible to the Partners. Type of Post and Holidays The post will be salaried and the initial contract will run until 31 st March There are five weeks of holiday per annum plus Bank/Public holidays. The holiday year runs from 1 st January and holidays are booked on a first come first served basis. Leave not taken cannot be carried forward to the following year. Popular holiday periods may have to be allocated on a rotational basis if demands exceed possibility. Any reasonable periods of leave with proven confirmed bookings at the time of taking up this post will be honoured for the current year. Salary & Pension The salary for this post will be by negotiation and will be dependent upon previous experience. The post is superannuable through the NHS Pension Scheme. The surgery also operates a Group Collection Personal Pension Scheme through Aviva Ltd to which employees are welcome to contribute. Our stakeholder pension provider is Natwest Bank plc. Sick Pay After a qualifying period, the practice has a sick pay scheme maximum benefit is 3 months on full pay and 3 months on half pay. Training There will be a full induction programme and regular in-house training. All staff are required to attend in-house training sessions unless on sick leave or annual leave, regardless of whether the training session occurs on one of their working days or not. Study leave for this post equates to a maximum of 37.5 hours per annum. Hours of Work This post is for 37.5 hours per week over 5 days. The surgery is currently open between and hrs. week days and shifts of varying duration between those hours are worked.

4 Should the practice decide to offer any form of Extended Hours service or to alter the surgery opening hours in any other way, the post holder may be required to alter their shift pattern to provide cover. No member of staff can work longer than 6 hours without a statutory 30 minute break (the usual lunch break is one hour). Morning coffee and afternoon tea breaks are usually 15 minutes. Shift Pattern There is some scope for flexibility and this can be discussed at your interview. DBS Check The post holder will be required to undergo an enhanced DBS check before the appointment is made substantive. Place of Work The post holder will be usually work at Theatre Royal Surgery but the post holder may be required to do visits to nursing homes and patients homes as determined by the practice and therefore a full driving licence and personal transport will be necessary. The post holder may also, from time to time, be required to work in other locations as determined by the practice

5 Job Description Summary This job description is not exhaustive and is intended as a guide only. The post holder is an experienced nurse, who will act within their professional boundaries. They will demonstrate safe clinical decision-making and expert care for patients within the general practice. In order to work at this level, NMC requirements for advanced practice must be met. The post holder must be able to prescribe independently. Patients with problems needing referral to secondary care should be discussed with their responsible GP before making such referral unless agreed pathways have been previously agreed between the practice and the nurse practitioner. Overview of Duties & Responsibilities To deliver a high standard of patient care as Nurse Practitioner (NP) in general practice, using advanced, autonomous clinical skills, and a broad and in-depth theoretical knowledge base. Make professionally autonomous decisions for which he/she is accountable. To undertake minor illness clinics or other such clinics as determined by the partners. To undertake home visits and visit to nursing homes for the frail elderly and those with Chronic Diseases. Job responsibilities Diagnosing and treating patients presenting with minor illness Ensure clinical practice is safe and effective and remains within boundaries of competence and to acknowledge limitations. Provide a first point of contact within the Practice for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision-making, to establish a diagnosis and management plan. Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports. Prescribe safe, effective and appropriate medication as defined by current legislative framework. Provide safe, evidence-based, cost-effective, individualised patient care. Offer an holistic service to patients and their families, developing where appropriate an on-going plan of care/support, with an emphasis on prevention and self-care and advise patients on general health care and minor ailments with referral to other members of the primary and secondary health care team as necessary. Fulfil patient requests by GPs.

6 Administration and professional responsibilities Participate in the administrative and professional responsibilities of the practice team. Ensure accurate and legible notes of all consultations and treatments are recorded in the patients notes. Ensure the clinical computer system is kept up to date with accurate details recorded. Ensure appropriate items of service claims are coded correctly and made accurately, reporting any problems to the Lead Nurse. Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice. Ensure collection and maintenance of statistical information required for regular and ad hoc reports and audit. Develop and maintain practice policies and protocols as required. Promote evidence-based practice through the use of the latest research-based guidelines and the development of practice-based research. Monitor the effectiveness of their own clinical practice through the quality assurance strategies such as the use of audit and peer review Training and personal development Training requirements will be monitored by annual appraisal and will be in accordance with practice requirements. Personal development will be encouraged and supported by the practice where practicable. It is the individual s responsibility to remain up to date with recent clinical developments. Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development ensuring PREP requirements are met and as agreed by the Nurse Liaison Partner. If it is necessary to expand the role to include additional responsibilities, full training will be given. Develop and maintain a Personal Learning Plan. Monitor the effectiveness of their own clinical practice through the quality assurance strategies such as the use of audit and peer review. Maintain their professional registration. Work within the latest NMC Code of Professional Conduct Confidentiality In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately In the performance of the duties outlined in this job description, the post-holder may have access to confidential information relating to patients and their carers, practice staff and other healthcare workers. They may also have access to information relating to the practice as a business organisation. All such information from any source is to be regarded as strictly confidential. Information relating to patients, carers, colleagues, other healthcare workers or the business of the practice may only be divulged to authorised persons in accordance with the practice policies and procedures relating to confidentiality and the protection of personal and sensitive data.

7 Liaison As well as the clinical team there is a need to work closely with the practice doctors, managerial, reception, and administrative staff to ensure the smooth running of the surgery. Any problems encountered must be reported to the relevant team leader. Meetings It will be necessary to attend and contribute to various practice meetings as requested. The only reason for not attending will be annual, study or sick leave. Health & safety The post holder must ensure compliance with relevant Health and Safety legislation and local policies and procedures, at all times. Assess the risk of the work environment and use results to improve practice. They must have an awareness of national standards of infection control and cleanliness and regulatory / contractual / professional requirements, and good practice guidelines. Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the team s performance. Work effectively with individuals in other agencies to meet patients needs. Effectively manage own time, workload and resources.
