JOB DETAILS Job Title: Responsible to: Department & Base: District Nurse Deirdre Moss District Nurse Team Leader Community Nursing Galashiels Health Centre Date this JD written/updated: April 2016 Job Reference Number: PCS JOB PURPOSE To demonstrate specialist competence, innovation and clinical leadership in the management and delivery of district nursing in all contexts appropriate to clients needs in the community. 3. ORGANISATIONAL POSITION The purpose of this section is to establish how the job fits into the rest of the organisation. It should be clear to whom the post holder is responsible and whether they have any other key lines of accountability, e.g. to a professional head. Please therefore record: a) the immediate line managers job b) job titles of colleagues reporting to the same boss c) job reporting directly to the post holder It is usually best to draw the chart with the immediate line manager in the centre at the top, the job in question immediately below with peers on either side. jobs should then be shown below. MAIN DUTIES/RESPONSIBILITIES To work as part of a community nursing team using specialist knowledge and skills to Provide and enhance evidenced based clinical care Identify community health needs and develop direct interventions to address them To work with individuals, families and carers as partners Develop and embed anticipatory and preventative care within practice To coordinate and manage services as a case manager within integrated working partnerships Plan care to promote and enable self care Provide leadership for quality improvement and service development Negotiate and influence locally to promote and manage professional development and practice Work independently and also as a team to coordinate, delegate and supervise care Act as a change agent 6. SYSTEMS AND EQUIPMENT Utilise and develop a range of IT applications to support and enhance practice Use established models of supervision and mentorship Car driver Mobicare safe system of working via mobile phone

5 TERMS AND CONDITIONS Grade and salary: Hours of work: Annual leave: Band 6: 26,565-35,577 pro rata per annum 30 hours per week, Permanent 27 days pro rata per annum Details of other types of leave entitlement (such as sick leave) are set out in the Agenda for Change handbooks. Public Holidays: Superannuation: 8 days pro rata per annum on dates designated by NHS Borders Details of other types of leave entitlement (such as sick leave) are set out in the Agenda for Change handbook. The post-holder is entitled to join the NHS superannuation pension scheme. 6 If your work involves exposure-prone procedures, you must keep to the document Protecting Health Care Workers and Patients from Hepatitis B, and the NHSiS Management Executive Directive on this issue. You must be immune to Hepatitis B, and if you cannot prove that you are immune, OHS will investigate to find out whether you are Hepatitis B positive or not. Health and Safety at Work: You must look after the health and safety of yourself and anyone else who may be affected by what you do at work.
