1 GUIDANCE NOTES FOR APPLICANTS Please read these notes carefully, as well as the Job Description and Person Specification, before completing and submitting your application. APPLICATION INFORMATION Please complete all sections of the application form. CV s will not be accepted. Applications received after the closing date/time will not be considered. Please provide details of all your work experience and account for all time since you left school, college or university, explaining any gaps in your employment history and reasons for leaving employment. Ensure that one nominated referee is your current or most recent employer. It is our intention to appoint the most suitable candidate for every vacancy in accordance with our Equality and Diversity policy. We decide who to interview by assessing the information you have provided on your Application Form against the criteria stated in the Person Specification. It is therefore important for you to evidence and explain how you meet the knowledge, skills and experience outlined in the person specification. It is not enough to say that you have experience, you need to give examples and describe what you have learned from your experience that will help you in this role: similarly, please outline how you have been able to demonstrate the relevant knowledge and skills and what you have learned that is relevant to this role. Shortlisting for advertised posts is generally done during the week following the closing date. 2 DATA PROTECTION Your application will be treated confidentially at all times. Its contents will only be disclosed to the persons involved in the recruitment and selection process. If you are appointed, this form will be used as part of your personal employee file and to gather workforce information. The information will be held in accordance with the Data Protection Act Forms for unsuccessful candidates are retained for a period of six months after which they are confidentially shredded. We may keep information for statistical and monitoring purposes. INTERVIEW INFORMATION Qualifications Candidates will be asked to bring along to interview original copies of all relevant educational and professional qualifications. A copy will be taken for our records and destroyed if the application is unsuccessful. ELIGIBILITY TO WORK IN THE UK Health in Mind has a legal obligation to check documentary evidence to confirm that all potential employees are eligible to work in the UK. The successful candidate will be required to provide proof of their eligibility to work in the UK. DISCLOSURE CHECKS PVG (Protecting Vulnerable Groups) Scheme All nominated candidates who will be employed to carry out regulated work must be a member of the PVG Scheme. If you are the successful candidate and are not a member, you will be required to join the PVG Scheme before a formal offer of employment is made. You will be required to pay the membership fee of 59. Existing PVG Scheme Members If you are already a Scheme Member for the same type of regulated work, Health in Mind will ask for a Scheme Update. We will ask to see your Scheme Record as evidence that you are already a Scheme Member. Health in Mind will pay for the cost of a Scheme Update.

3 Standard / Basic Disclosure If the post does not meet the requirements for PVG membership, the successful candidate will be required to complete either a standard or basic disclosure (the type of disclosure required will be specified in the job advert). You will be required to pay the cost of for this disclosure. Overseas Criminal Record Checks Disclosure Scotland is not able to check the criminal history system of any other country outwith the UK. If you have lived or worked overseas for 3 months or more in the last 10 years (while aged 16+) then a criminal records check from the relevant country must be provided. The successful candidate will be responsible for providing this (translated into English where necessary) prior to a formal offer of employment being made.

4 BACKGROUND INFORMATION GUIDED SELF HELP Health in Mind delivers a range of psychological therapies designed to assist individuals with their personal recovery journey. Central to this is our Guided Self Help service (GSH), this service offers a Cognitive Behavioural Therapy (CBT) based approach to supporting people with mild to moderate anxiety, depression or stress. This provides an evidence-based, problem focused approach to changing the way people think, feel and behave. It is delivered over four sessions and generally these are spread over several weeks. Health in Mind has been funded by the NHS to deliver this service in Edinburgh and Midlothian. The Guided Self Help workers provide the service in local health centres in Edinburgh and Midlothian as well as our offices in Shandwick Place in Edinburgh. We are currently looking to appoint two motivated and dynamic individuals to two key posts within the Team, (Senior Guided Self Help Worker and Guided Self Help Worker, Edinburgh). In both of these roles you will work with clients, assessing their self-help needs, providing guidance in using self-help materials and signposting to appropriate local organisations, social prescribing options or other appropriate information and advice. The post holders will be required to liaise with primary care,, voluntary sector and secondary psychological services. We would like to hear from you if you have appropriate experience and have excellent communication skills. Health in Mind work in partnership with NHS Lothian to provide both line management and clinical supervision. Both posts are fixed term until March 2016 (extension beyond this date will be dependent on funding)

5 JOB DESCRIPTION Job Title: Salary: Hours: Responsible to: Holidays: Senior Guided Self Help Worker 24,460 - AP4 Pt 29 (fixed) hours per week Depute Chief Executive 30 days, plus 12 days public holiday 1. PURPOSE OF THE JOB To guide adult patients living in Edinburgh and/or Mid Lothian, with mild to moderate psychosocial problems to make positive changes through the use of self help materials and signposting to other social prescribing options. To be responsible for the development and co-ordination of the Health in Mind Guided Self Help service in accordance with the overall strategy for the service and in agreement with the Line Manager and the Guided Self Help Steering Group. The Senior Guided Self Help Worker will work across Edinburgh and Midlothian. The Senior Guided Self Help Worker will: Receive referrals from GPs and other member of the primary care team Complete assessments of specific self help needs, in keeping with the Integrated Care Pathway guidelines, and offer up to three follow up appointments Signpost to appropriate local organisations, other social prescribing options or other appropriate information and advice Refer back to the GP where a person is not appropriate for the service Provide operational supervision to a small team of Guided Self Help Workers To effectively report the outcome and output targets for the GSH Service to the GSH Steering Group Liaise with the GSH Team and report back to the line manager and GSH Steering Group on the operation of the service and raise any points of concern To be responsible for promoting the GSH service to GPs and the voluntary sector

MAIN TASKS 2.1 To use cognitive behavioural principles to implement problem solving strategies and guide clients in the use of self-help materials 2.2 To work flexibly and innovatively with clients to help them find their own solutions to psychological problems 2.3 To actively listen and empathise with clients, guiding them to an understanding of their difficulties 2.4 To ensure good communication with referring GPs and practice staff 2.5 To develop and maintain a thorough knowledge of and encourage referral to the voluntary sector 2.6 To ensure appropriate record keeping is maintained and data is collected for audit and evaluation of self help service 2.7 To contribute to the team meetings and to assist in developing the service 2.8 To deliver the output and outcome targets for Health in Mind s Guided Self Help service (GSH) 2.9 To advise the ling manager and GSH Steering Group regarding resources needed to deliver the targets for the GSH Service 2.10 To provide operational support for GSH workers 2.11 To lead the development of operational policies, procedures and processes as required in agreement with the line manager and the GSH Steering Group 2.12 To monitor and evaluate the quality of service delivery including client feedback for the purpose of service development and improvement To monitor performance of the service in relation to output and outcome targets and prepare monthly summary reports 2.14 To ensure that appropriate record keeping, referral processes, are maintained 2.15 To ensure that GSH workers have appropriate training and continued professional development 2.16 To take an active role in recruitment, induction and ongoing day to day management in agreement with the line manager and in accordance with organisational guidelines and policies 2.17 Lead, support and develop the GSH team to a high standard and in agreement with the line with Health in Mind s policies and procedures

RISK MANAGEMENT 3.1 To contribute to and ensure the implementation of appropriate Health and Safety policies and risk management 3.2 To ensure that own working practices are compliant with relevant Health in Mind policies eg Lone Worker policy; Health and Safety policy 4. RESOURCE MANAGEMENT 4.1 To adhere to the relevant office, finance, administrative and reporting procedures of Health in Mind 4.2 To contribute to Health in Mind s service planning and review processes and systems 4.3 In agreement with line manager monitor financial data 5. COMMUNICATION 5.1 To liaise with GSH Steering Group, key stakeholders, external agencies and relevant statutory bodies 6. SUPPORTING EQUALITIES 6.1 To identify potential barriers to accessing the service and ensure that relevant adjustments are made, with a view to encouraging clients from all parts of the community. 6.2 To ensure that Equalities Data is gathered and regularly analysed. 6.3 To comply with Health in Mind s Equalities Policy 7. SUPPORT AND SUPERVISION 7.1 The Senior Guided Self Help Worker will report to Depute Chief Executive 7.2 To receive regular clinical supervision on an individual/group and peer basis co-ordinated by the NHS Lothian Clinical Psychology lead 7.3 To receive regular line management support and supervision and an annual appraisal from the line manager 7.4 To make opportunity to attend in-house training events. External training will be made available subject to appraisal recommendations. 7.5 To attend and liaise with the GSH Steering Group
