2 Dear Head Professional, Please find enclosed a Hole in One program designed exclusively for you! For the eighth consecutive year, the PGA of Alberta, working in conjunction with Morgex Insurance, will be providing all Members with the opportunity to purchase Hole in One Insurance for your outside tournaments and club events. Enclosed you will find all the information you need to increase revenue sales at your club and provide your membership and corporate tournaments with added value. Applications, quotes, cancellations, and claims are all coordinated through the zone office thus making the administration simple. Claims are paid directly to your club by the PGA of Alberta; therefore you are able to determine whether the lucky winner receives cash, prizes, or gift certificates to the shop. For those of you who have been involved in the past, prices for the 2009 season have been reduced. And by using the online application you will receive a significant discount. Applications that are faxed in will not receive this online discount. 3 QUICK FACTS APPLICATIONS Hole in One applications must be received by the PGA of Alberta one business day prior to the event. If there is a rain out or cancellation - you must let us know within 24 hours or your event will not be credited or refunded. Any changes to an event must be made in writing. If it is a weekend and you need coverage please submit your application online or fax the application to both the PGA of Alberta / and Morgex *Please note - the application still must be faxed 24 hours prior to the start date of the event. PRICE The online purchase price for the 2009 season is $40 for $2,500 worth of coverage. Any denomination can be purchased up to $35,000 per hole - this ratio acts as a guide. Go to at anytime to register your Hole in One event online. Organize and setup all of your events at the beginning of the season and modify them based on any changes that occur throughout the year. YARDAGES & FIELD SIZE For any coverage of $2,500 and less, your minimum yardage is 150 men's and 135 ladies. For any coverage over $2,500, your minimum yardage is 160 men's and 145 ladies. *Please be sure of your yardages to avoid any delays in case of a hole in one claim. Maximum field size is 160 players. *Call for a quote if field size is larger or enter values online to calculate your cost. MAKING A CLAIM The PGA of Alberta must be notified within 3 BUSINESS DAYS of the successful hole in one. *Failure to notify the office within this time frame could result in a denied claim. In the event of a hole-in-one claim, the insured must get the correct and current names and phone numbers of all members playing in the foursome/threesome, the hole monitor(s), as well as the original tournament scorecard. The PGA of Alberta will fax an affidavit and claim form as soon as we are notified of a hole-in-one claim. This must be filled out in its entirety and returned within 5 working days. Failure to provide a completed and signed claim affidavit within the specified time period WILL result in the claim being DENIED. Spotters must be used for prize values over $2,500 and the spotter must be 18 years of age. If there is 25 players or less, a representative from the golf course must monitor the hole. (ie: club pro, club manager) If a claim is made - the club can pay their winner in cash or in pro-shop gift certificates - your choice. Once the claim is submitted and approved by Morgex - the PGA of Alberta will send a cheque to the club.

4 2009 HOLE-IN-ONE TERMS & CONDITIONS 1. The PGA of Alberta must be notified, in writing and prior to the start of the event, of any changes being made to a policy. Failure to do so may result in a denied claim. 2. The minimum yardage on any insured hole must be 160 yards from the tee to the flagstick. Ladies may play from a minimum of 145 yards. The insured hole shall remain in its normal position with regard to tee box & green. If your prize is valued at $2500 or less the minimum yardages can be 150 for men and 135 for ladies. 3. All equipment must conform to the rules of golf as determined by the RCGA. This includes, but is not limited to the Flagstick, Hole, Golf Club, Golf Ball. 4. In the event of a cancellation, the PGA of Alberta must be informed, in writing, within 24 hours. Failure to provide notification will result in the full premium being charged. 5. One non-biased tournament official must be stationed and monitoring play at each of the insured holes at all times during the insured event. 6. The tournament official stationed at the insured hole cannot be younger than 18 years of age. 7. If the event has 25 players or less, a representative from the golf course must monitor the hole. (Ie, club pro, club manager, etc.) 8. The PGA of Alberta must receive all applications for holes under $35,000CDN at least one day prior to the start of the event. All applications with holes over $35,000CDN must be received at least three days prior to the start of the event. 9. A participant is any individual, not including golf club pros or professional golfers, entered and playing in the insured event for which the premium has been paid. 10. Pros may be made eligible with the underwriter's approval and must play from a minimum of 175 yards. The PGA of Alberta must be notified of any pros, in writing, prior to the start of the event. (An additional charge may be applied.) 11. Coverage is only applicable in the amount not exceeding the limit of insurance stated in the policy declaration. Insurance is for the cost incurred in providing an award to the participant whom, during the policy period, makes the first hole-in-one on the insured hole(s) in an insured event. 12. The applicant must inform the PGA of Alberta of a hole-in-one claim within three working days of the event. (It is in the applicant's best interest to call from the golf course as soon as a hole-in-one claim occurs.) 13. In the event of a hole-in-one claim, the insured must get the correct & current names and phone numbers of all members playing in the foursome/threesome, the hole monitor(s), as well as the original tournament scorecard. The PGA of Alberta will fax an affidavit and claim form as soon as they are notified of a hole-in-one claim. This must be filled out in its entirety and returned within 5 working days. 14. The participant must finish the entire round of golf and complete the scorecard. 16. In the event of a hole-in-one claim, certification of achievement must be made on the scorecard of the participant who has achieved the hole-in-one by the tournament official monitoring play at the insured hole and all participants within the group in which the hole-in-one was made. 17. The insured tournament must be played in groups made up of no less than three golfers. Groups of two are not acceptable. 18. A ball hit out of bounds and replayed does not qualify as a hole-in-one. This would constitute a hole-in-three. MULLIGANS ARE NOT eligible for a hole-in-one claim. Each golfer shall have a maximum of one attempt regardless of the outcome of the shot. A golfer who is 'rounding out' a group by taking an extra shot for the team is only eligible on their first shot. 19. Morgex will indemnify the first hole-in-one sponsor only. Please check with the tournament organizers to ensure that there are no other sponsors on the insured hole. 20. Please note that failure to adhere to all TERMS & CONDITIONS can result in a claim being denied.
