Despite the commitment and efforts toward the delivery of safe high quality care, patients may be harmed or injured. And, despite these efforts, we still see communication failures, medication errors, human errors and systemic failures that result in allegations of medical malpractice or negligence. The panel of experts will share their perspectives on three cases and discuss risk reduction strategies. 2 Mary G. Gregg, MD, FACS, MHA Chief Medical Officer, Senior Vice President MAG Mutual Insurance Company, Atlanta, GA Dr. Mary Gregg oversees the Risk Management and Patient Safety activities of the company. As Chief Medical Officer, she is responsible for developing, implementing and communicating patient safety initiatives that make MagMutual a leader in the field of medical liability. Prior to joining MagMutual in 2013, Dr. Gregg served as Vice President of Quality and Patient Safety at Swedish Medical Center, a large non profit health care provider located in Seattle, Washington. She received numerous awards from Qualis Health, a CMS initiative, the Washington State Hospital Association, and the Institute for Healthcare Improvement (IHI). In addition, Dr. Gregg was instrumental in attaining the Leapfrog top hospital award for the past three years. After her fellowship, Dr. Gregg began her early practice of cardiothoracic surgery at the University of Washington in Seattle. She developed the open heart program, and subsequently performed the first open heart surgery at the Seattle Veterans Hospital. She then entered a private practice cardiac surgery program at Swedish Hospital in Seattle, which subsequently merged to become one of the largest cardiac surgery programs in Washington State.

3 John C. West, JD, MHA, DFASHRM, CPHRM Principal West Consulting Services, Signal Mountain, TN Mr. West is currently employed as the Principal of West Consulting Services, an independent risk management and patient safety consulting firm. He holds a law degree from Chase College of Law, a master s degree in Health Services Administration from Xavier University, and a bachelor s degree (Biology) from the University of Cincinnati. He holds the designation of distinguished fellow of the American Society for Healthcare Risk Management (ASHRM). He received the Presidential Citation Award (2011) and the Distinguished Service Award from ASHRM (2001), which is its highest honor. He has also been a certified professional in healthcare risk management since Mr. West has broad experience in loss prevention, including professional and general liability, bl occupational safety and health, h and environmental impairment. He was certified in industrial hygiene by the American Board of Industrial Hygiene in Mr West hasbeen the a thor of the Case La Update col mn hich Mr. West has been the author of the Case Law Update column, which appears quarterly in the Journal of Healthcare Risk Management, since He is a regular speaker at national and regional risk management meetings and has published numerous articles on risk management.

4 Darcy D. Dober, RN, BSN Corporate Physician Services Risk Manager Novant Health, Winston Salem, NC Darcy currently functions in the role of Corporate Physician Services Risk Manager for Novant Health, an integrated healthcare delivery system located throughout North Carolina, South Carolina and Virginia, which includes Acute Care Hospitals, Physician Clinics, Ambulatory Care Centers and Diagnostic Imaging Centers. In her role, she proactively identifies, evaluates and mitigates risk within the Outpatient Physician Practice setting and affiliated free standing Diagnostic Imaging Centers. With 400 physician clinics and 1,700 providers, she actively supports leaders, providers, and staff throughout the care continuum with a focus on patient safety.

5 Nan L. Holland, RN, BSN, MPH, CPHRM Vice President, Corporate Risk Management Novant Health, Winston Salem, NC Nan is an innovative healthcare professional with expertise in identifying, evaluating and addressing risk in the healthcare environment, with specific expertise in acute care, ambulatory care, skilled nursing, managed care and the physician practice setting. As Vice President, Corporate Risk Management, Nan leads the corporate risk management and claims team for Novant Health. Novant Health is an integrated healthcare delivery system in NC, SC and VA, including 14 acute care hospitals, 400 physician i clinics, i 1,700 providers, 40+ ambulatory care centers and over 60 diagnostic imaging i centers. The system provides self insured and risk retention group solutions for the facilities and physician partners. Prior to moving to Novant Health Risk Management, Nan served as Senior Director of Clinical Resource Services for Novant Medical Group. She and her team were responsible for clinical operations with specific focus on monitoring and supporting clinical quality initiatives, regulatory and clinical safety support, lab compliance and Physician Group Practice Demonstration (PGP, PGP TD) programs. Some of these programs include development of COMPASS Disease Management program and SafeMed Medication Therapy Management programs to focus on managing high risk patients of NMG as well as Novant employees. That team also supports the NCQA recognition for Diabetes, Heart Stroke and Patient Centered Medical Home as well as coordination of care with hospitals, community partners and community based physicians. Prior to re joining Novant Health, Nan worked with USI and GE Insurance Services as Vice President, Healthcare Risk Management and a Senior Healthcare Risk Management Consultant respectively. She spent 6 years with Partners National Health Plan of North Carolina holding positions including: Director of Quality Improvement/Disease Management, Director Provider Credentialing/Risk Management, Risk Management Analyst, and Quality Improvement Coordinator. Nan also served as a Healthcare Risk Consultant for PHICO Insurance Company. Prior to that time, she held various clinical and administrative positions in emergency medicine, critical care, ambulatory care, post surgical care and staff education and administration in two large integrated delivery systems Nan holds a Masters in Public Health, Health Policy and Administration, from the University of North Carolina at Chapel Hill, and a Bachelor of Science in Nursing, from East Carolina University. She has also earned the designation of Certified Professional Healthcare Risk Manager (CPHRM) and Certified Emergency Nurse (CEN). Throughout her career, she has held many affiliations, including, but not limited to, National Association Medical Staff Services, Southeast Regional Risk Management Conference Faculty, American Society for Healthcare Risk Management (ASHRM), ASHRM Annual Conference faculty, Forsyth County Immunization Tracking Steering Committee, National and Regional Faculty for American Medical Group Association (AMGA) and World Congress. Nan has held positions, as President, Vice President, Board of Directors, Program Chair person for NC ASHRM. She has also served as Councilwoman for the Village of Clemmons, NC.
