2 Purpose Consumers want expanded community-based services so they can live in their own home environment as long as possible. As the range of living options widens, expectations for more complex services are driving home care and housing-withservices providers to increase their clinical services to meet this consumer need. As a nurse in one of these settings, you need sufficient knowledge and skills in the complex healthcare needs of older adults and emerging trends in home care services. This in-depth educational series is specifically tailored to build the essential knowledge and skills needed to work effectively and efficiently in the challenging field of home care services, including home care assisted living programs in senior housing. About the Program This certificate series is focused on nurse practice in a wide variety of home care settings. 3 Day 1 October 10 The Foundation of Your Home Care Program Day 2 October 11 The Role of the Nurse in Home Care 8 a.m. 4 p.m. Presenters: BARB BLUMER, BARB BURANDT, CONNIE DREYER Working in home care in settings serving seniors is uniquely different from nursing in other settings. Develop a solid foundation for your organization s home care program by understanding the philosophy of home care, how it is incorporated into your work setting, and learn how to assess client needs and develop care and services plans. The nurse manager s role in driving home care services and programs. The philosophy of client-directed services and issues related to providing services in the client s home. Common admission/discharge issues what home care providers can do versus what housing providers can do. Practical strategies for building a strong relationship between home care and the housing setting and how to spot potential communication issues. Explore the assessment process and how it drives the care plan and the service plan/agreement. Learn to easily identify areas that require in-depth assessments. How to supervise clients services, make updates to assessments and service plans, and coordinate care with other providers. Become familiar with Advance Directives and the right of clients to refuse services. Legal relationships your client may have with others. 8 a.m. 4:15 p.m. Presenters: JEANETTE MEFFORD and SUSAN SCHAFFER As a nurse working in home care settings you need a specific skill set, and there are special demands placed on you in driving the clinical services offered. You must also be in compliance with Minnesota s Nurse Practice Act. Understand how the Act defines the clinical and operational roles of the professional nurse and what you need to know as a nurse manager as you shape your practice in home care settings. The nurse s role in driving the home care service program, in quality assurance, regulatory compliance, in budgeting/resource allocation and their role in HWS settings. How the Minnesota Nurse Practice Act: applies in home care settings and the definitions that are applicable to delegation and supervision; defines the roles of the registered nurse and the licensed nurse in the home care setting; and is consistent with the requirements of the Minnesota home care agency licensing rules. Developing and updating effective policies and procedures and what s required under home care licensure. Strategies to improve your quality improvement efforts and how home care licensure looks at quality improvement.

4 Day 3 November 14 Regulations and Human Resources Day 4 November 15 Financial Management and Training Staff 8 a.m. 3:30 p.m. Presenters: MARY YOULE and PATRICIA JUMP Wherever there are clinical services provided in licensed settings and human beings who provide them, there are regulatory requirements. Boost your knowledge and get practical tips for maintaining compliance. ALHC and Class A survey trends and their implications for you as a provider. Systems and operational strategies that help maintain and assure regulatory compliance. How and where to maintain the client record required components, documentation standards and how to maintain confidentiality. How to identify the type of staff best suited to deliver the services your home care agency provides. Challenges and opportunities for scheduling home care staff. How on-call issues relate to the requirements of the Nurse Practice Act. 8 a.m. 4:30 p.m. Presenters: LYNNE AULT BLAISDELL, SUE ANN GUILDERMANN, JEANETTE MEFFORD, LINDA NELSEN, MARY JO THORNE Most nursing programs don t usually include classes on accounting/financial management and how to teach adult learners. However, once you become a manager you ll need to have these skills. Today s program will explore the realities of developing this skill set, along with key concepts you need to know. How to identify the health care services you want to provide, payer sources, and how to price and document services for appropriate reimbursement. Key elements of an effective working budget for home care services and how to avoid service creep. Determining training requirements for your staff based on your license, a variety of regulatory requirements and how to coordinate all of this with the housing site. Training topics that must be covered in the annual employee orientation. Important principles of adult learning and how to use them in a successful training program. Effective ways to document training.

5 Day 5 December 5 Clinical Skills and Case Management Day 6 December 6 Supervision, Delegation and Leadership 8 a.m. 4 p.m. Presenters: LORI CALLAHAN, DENISE JOHNSON, JEFF LINDOO, NADINE PAITICH : Building on the clinical skill development begun on Day 2, today we explore the challenging issues of medication management iin home care settings where the Nurse Practice Act and licensure rules often intersect. We ll also explore case management as a way to organize the delivery of clinical services in home care settings. : Important survey trends related to medication administration. Current and near future trends in pharmacology that home care nurse managers need to know. Create and maintain a system for medication system and storage. Documentation and recordkeeping requirements for medication storage in home care and housingwith-services settings. Case management as a process to coordinate care and services in order to ensure client needs are met. Models of case management in the coordination of services between providers and the roles of the clinical services professional. Identify high risk and costly clients to evaluate if you can meet their needs in a home care setting. 8 a.m. 4:15 p.m. Presenters: CONNIE DREYER, MICHELLE KLEGON, JEANETTE MEFFORD, SUSAN SCHAFFER and LIZ SETHER : As a nurse manager you will supervise, delegate and lead all in the context of your organizational setting, the Nurse Practice Act, the regulatory and legal environments.
