1 Lake Shore Funeral Home General Price List This General Price List is available for your retention, if you so desire. These prices are effective November 1, 2013, but thereafter are subject to change without notice. Itemization of Services and Products The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. PROFESSIONAL SERVICES BASIC SERVICES OF FUNERAL DIRECTOR AND STAFF... $2,140 Includes, but is not limited to, staff to respond to initial request for service 24 hours a day, 7 days a week; arrangement conference with family and responsible party; arrangement of service including preparation and filing of necessary authorizations and permits, recording vital statistics, preparation and placement of obituary notice, and coordination with those providing other portions of the funeral, e.g., clergy, cemetery, crematory, vault companies and others as required. Also included in this charge are overhead expenses relative to our facilities such as professional licensing, legal and accounting fees, insurance, building and utility expenses, parking lot and grounds, maintenance, taxes, equipment, furnishings, inventory costs, record keeping, secretarial and administrative expenses. This fee will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains.) PREPARATION EMBALMING / SCIENTIFIC PREPARATION BY A LICENSED EMBALMER $ 575 Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. Other preparation of the deceased: Cosmetic Application, Dressing, Hairdressing, and Placement of Deceased... $275 Bathing and Sanitary Procedures when No Embalming has been performed...$150 Restoration / Special Cosmetology / Other Specialty Services...$75 per hour Necessary Care Following an Autopsy / Organ Donation Procedure(s)....$150 Refrigeration of Unembalmed Remains $ 75 per day (In accordance with Texas Law, unembalmed human remains sheltered by the funeral home shall be placed in refrigeration after 24 hours from time of death until final disposition.) It is the policy of Lake Shore Funeral Home that all remains that are not embalmed and/or not casketed will not be placed in public areas for public viewing. Viewing of remains that are not embalmed and/or not casketed is limited to immediate family members and will be done in a private area at the funeral home for a specified time frame set forth by the funeral director. AUTOMOTIVE EQUIPMENT (40 mile radius included) Transportation outside of a 40 mile local service radius will be subject to an additional charge of $2.50 per mile (one-way) per vehicle. Catering is also available or you can bring your own food. Ask us for a listing of area caterers and fees. When a caterer is not involved, a Refundable Clean-up Fee Deposit will be charged at $150 and returned if facility is left in proper order. Also, for Waste Disposal Fee, add: $150. Room rental is subject to availability. Service Only / Chapel Rental (Monday thru Friday, 9:30am to 3pm)... $795 Our services include: coordination of the service, supervision of the memorial service, and additional staff necessary to conduct ceremony at the funeral home (choice of three chapels) or at another facility when Lake Shore Funeral Home does not provide the service / cremation. Weekend Service Fee... $200 Late Service / Evening Fee... $200 Surcharge applied when funeral, memorial, or graveside service is scheduled after 3:00PM Additional Service... $ 90 Use of our chapel(s) for additional Vigil, Wake, Prayer Service, or Rosary Holiday Fee... $350 This fee will be added if funeral arrangements are scheduled or services are conducted on the following holidays: Memorial, Labor or Independence Days. * No Arrangements or Services will be held on: Easter, Thanksgiving, Christmas or New Year s Day. Overnight Fee (One Director per Night)... $300 This charge will be incurred when the funeral is conducted by our staff in a location that is in excess of 300 miles from our funeral home for lodging, meals and staff time. Disinterment... $595 This is our professional charge for securing permits, arranging disinterment with a licensed funeral director supervising disinterment (within 40 miles). Additional charges will be incurred for opening and closing of the grave, mileage, permits, fees and merchandise. 3 Temporary Grave Marker..$15 MEDIA Webcasting the Service......$195 This service includes: coordination / supervision of funeral staff to broadcast the chapel ceremony or visitation live by way of webcasting over the internet through a secure site that is password protected for the family s privacy. This is intended for those who cannot attend the ceremony; webhosting for rerun of the ceremony for 30 days, and one copy of the ceremony on DVD for the family s retainage. SPECIAL CHARGES Forwarding Remains to Another Funeral Home - Minimum Transfer $1,530 This charge includes: transfer of deceased to funeral home, basic services of staff, embalming, necessary authorizations and permits, and transportation to DFW airport or service vehicle and first 40 miles of transportation. This charge does not include casket / container for shipment, airline or other shipping charges, visitation, rites or ceremonies prior to forwarding of the remains. - Standard Transfer (Private Family Viewing ONLY, NO Ceremony)... $2,170 This charge includes: transfer of deceased to funeral home, basic services of staff, embalming and other preparation, necessary authorizations and permits, and transportation to DFW airport or service vehicle and first 40 miles of transportation. This charge does not include casket / container for shipment, airline or other shipping charges, visitation, rites or ceremonies prior to forwarding of the remains. We appreciate the family s choice to view when we provide Other Preparation-Dressing, Cosmetic Application, Hairdressing and Placement so as to verify that our work in this regard is satisfactory and to avoid any additional charges by the receiving funeral home for any changes to the appearance of the deceased. This charge does not include DFW transfer, visitation, rites or ceremonies or hearse. With Graveside Service (Weekdays), Visitation same day of Ceremony, and Casket Coach $1795 DIRECT DISPOSITION ALTERNATIVES - Immediate Burial... $1,900 to $7,900 This charge includes: transfer of deceased to funeral home, refrigeration (two days included), local transportation to the cemetery, necessary authorizations and basic services of staff. This charge does not include the use of facilities and staff or any visitation or ceremony, public or private, prior to burial. The time of burial will be determined by the funeral home dependent upon current scheduling and commitments. Immediate Burial with Container Provided by Purchaser $1,900 Immediate Burial with Alternative Container $2,150 Immediate Burial with Casket Selected from Our Funeral Home...$2,395 to $7,900 Immediate Burial is the process of arranging for burial of an unembalmed body as soon as possible. Prices for Immediate Burial are not for graveside services. Interment will be completed at the convenience of the funeral director. Cemetery property, grave opening, outer burial container (if required) or viewing (other than for identification purposes) are not included. - Direct Cremation... $1,275 to $5,870 Direct Cremation with Container Provided by Purchaser $1,275 Direct Cremation with Fiberboard Alternative Container Selected from Our Funeral Home $1,395 Direct Cremation with Casket Selected from Our Funeral Home $1,770 to $5,870 The Crematory surcharge for bodies over 350lbs., please add $240 Private Witnessing of Cremation (weekdays by appointment) please add... $195 Family Identification (one to two People ONLY, 5 Minute Time Limitation)...No Charge Our service includes: staff services, and non-public viewing area Our charge for a direct cremation (without ceremony or viewing) includes, but is not limited to, basic services of Funeral Director and staff, a proportionate share of overhead costs, transfer of deceased to funeral home, obtaining necessary authorizations, consents, refrigeration (2 days included) and crematory fee. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The Page 4

Notice to Consumers: In most areas of the country, state or local law does not require that you buy a container to surround the casket in the grave. However, many cemeteries require that you have such a container so that the grave will not sink in the future. Either a grave liner or a burial vault will satisfy these requirements.
