3 General Price List The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. Professional Services BASIC SERVICES OF FUNERAL DIRECTORS AND STAFF... $3, Our fee for the services of funeral director and staff includes, but is not limited to, staff to respond to initial request for service, arrangement conference with family or responsible parties, arrangement of funeral, preparation and filing of necessary authorizations and permits, recording vital statistics, preparation and placement of obituary notices, and coordination with those providing other portions of the funeral, e.g. cemetery, crematory, vault companies and others as required. Also included in the charge are overhead expenses relative to our facility such as insurance, maintenance and utility expenses, secretarial and administrative cost, and equipment and inventory expenses. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremation, immediate burials, and forwarding or receiving remains.) EMBALMING... $ Embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. Embalming is a chemical process which provides temporary preservation of the body and eliminates health hazards; therefore, it is a policy of this funeral home that remains be embalmed or refrigerated if held over 24 hours after death. This is for the protection this facility, the general public, and our employees. 19 We respectfully ask that all bills be paid in full at the completion of the arrangement conference. For your convenience, we have listed and accept the following methods of payment: 1. Cash, Check or Credit Cards. We accept Visa, MasterCard and Discover. 2. Verified and Assignable insurance for Robert Massie Funeral Home charges only, that Robert Massie files for your family. (Policy must have been in effect at least two years at the time of death). *Any funeral costs paid with insurance assignments will have a surcharge of $85.00 and an additional 2.5% added for the life insurance filing fee 3. Any local or out-of-town funeral home prearranged funeral plans that can be verified. (The contract balance is due at time of arrangements). *Any local funeral home pre-need policy is welcomed at Robert Massie either before time of death or it may be used at time of death. It s not a funeral home pre-need, it s your pre-need. All pre-arranged funeral plans may be transferred to whichever funeral home you choose. 19
