In order to maintain this commitment Dengel & Son has established a uniform payment policy to serve all families fairly. This policy allows us to contain our costs and offer families financial options to accomplish the memorialization they desire. Provisions for payment are due at the time of arrangements. To avoid any possibility of misunderstanding or embarrassment and for your convenience, we accept the following methods of payment: CASH-CHECK-DEBIT CARD MASTERCARD VISA - DISCOVER ASSIGNABLE INSURANCE- Insurance assignment of a verified policy(s) through Beta Capital, a charge of 5.00% of the assigned amount will be added to your statement of goods and services. In order to use this payment option we must have the insurance policy(s) verified by Beta Capital prior to scheduling service times. Dengel & Son Mortuary does not include arrangements with the estate of your loved one, but rather with the purchaser. 1

The goods and services shown below are those we can provide our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. ITEMIZED SELECTIONS BASIC SERVICES AND OVERHEAD OF FUNERAL DIRECTOR AND STAFF $ Our fee for the services of funeral director and staff includes, but is not limited to, staff to respond to initial request for service: Arrangement of funeral; conference with family or responsible party; preparation and filing of necessary authorizations and permits; recording vital statistics; preparation and placement of obituary notices; staff assistance prior during and following the funeral, including coordination with those providing other services, e.g. cemetery, and others. Also included in this charge are overhead expenses relative to our facility such as insurance, maintenance and utility expenses, secretarial and administrative costs, and equipment and inventory cost. This fee for our services will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains.) This is a Non-Declinable fee. If you do not want embalming, you usually have the right to choose an arrangement, which does not require you to pay for it, such as direct cremation or immediate burial. This charge does not include any preparation of the remains, casket or alternative viewing container, urn, memorial items, cash advance items, or overtime fees for weekends. OPTIONS AVAILABLE WITH THIS PACKAGE Add Private family viewing with Bathing and Disinfection $ Private family viewing with Bathing Disinfection, Dressing, and Casketing $ DIRECT CREMATION PACKAGE $2, When the decision has been made not to have a viewing or ceremony. Our charge for this service includes Basic services and overhead of funeral director and staff, transfer of remains to funeral home (within Franklin Co.), refrigeration of an unembalmed body prior to cremation, cremation container, cremation fee, and utility vehicle. This charge does not include merchandise, use of facilities and staff for visitation /viewing, ceremony, graveside service, public or private, prior to or following cremation. IMMEDIATE BURIALS $3, Our charge for this service includes transfer of the remains to the funeral home (within Franklin Co.), basic service of funeral director and staff, dressing and casketing, refrigeration (one day), utility vehicle, and transfer to the cemetery (within Franklin Co.). This charge does not include merchandise, use of facilities and staff for visitation/viewing, graveside service or ceremony, public or private, prior to or following burial. FORWARDING REMAINS TO ANOTHER FUNERAL HOME $4, Our charge for this service includes transfer of remains to funeral home (within Franklin Co.), embalming, basic service of funeral director and staff, dressing and or casketing, and utility vehicle to point of destination or transportation (plus mileage). This charge does not include merchandise, viewing, ceremony, commercial shipping charges, cargo tray or air tray for transit. RECEIVING REMAINS FROM ANOTHER FUNERAL HOME $3, Our charge for this service includes transfer of remains to the funeral home (within Franklin Co.), basic service of funeral director and staff, and hearse (within Franklin Co.). This charge does not include merchandise, viewing, and ceremony or graveside services. 5

6 FOREIGN DESTINATION CHARGES $ Our charges for this service include securing the added documentation required for foreign shipping as well as the necessary courier or over-the-road transportation to secure the same. It may also include the translation of documents, added long distance telephone expense, permit fees, and the added time involved in dealing with consulates/embassies of foreign countries and airlines. Should actual expenses be over this fee, adjustments in this amount will be made. This charge is in addition to any other charges for services and merchandise, embalming, other preparation, or use of facilities and transportation or remains. This charge does not include merchandise, viewing, ceremony/ rite or third party expenses. NOTICE: These disclosures are made in compliance with requirements set forth by the Federal Trade Commission. Service and Merchandise categories have been established according to guidelines set forth by the Federal Trade Commission. CUSTOMARY GRATUITES Third party participants should receive gratuities for the service they provide. The amounts shown below are guidelines to what is normally given.
