1 GENERAL PRICE LIST 9850 West Thunderbird Road Sun City, Arizona Telephone: (623) Fax: (623) Website: Prices effective as of January 1, 2014 The goods and services shown below are those we provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected.
2 MINIMUM SERVICES OF THE FUNERAL DIRECTOR AND STAFF This fee for our basic service and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving the remains.) The fee includes the following: Personnel available 24 hours a day, 365 days a year to respond to initial call Arrangement conference Coordinating service plans with cemetery, crematory, and/or other parties involved in the final disposition of the deceased Clerical assistance in the completion of various forms and permits associated with a funeral Also covers overhead, including facility maintenance, equipment and inventory costs, insurance and administration expenses, and governmental compliance PREPARATION OF THE BODY Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. Embalming Dressing, casketing and/or cosmetizing of deceased Basic Preparation of deceased of no embalming Bathing, disinfecting and dressing of deceased if no embalming Refrigeration per day (It is the policy of our firm to require refrigeration of unembalmed remains after 24 hours.) 3 USE OF FACILITIES AND STAFF Use of facilities and staff services for visitation (1 hour during business hours) Our services include set up of visitation area, placement of encased remains, display of floral arrangements, supervision of and attendance during the visitation. Use of facilities and staff services for funeral service in our facility Our services include coordinating the funeral arrangements, supervision of the funeral, and staff to assist with the funeral ceremony. Staff services for funeral service at other facility Our services include coordinating the funeral arrangements, supervision of the funeral, and staff to assist with the funeral ceremony. Use of facilities and staff services for memorial service (without remains present) in Our Facility. Our services include coordinating the memorial service arrangements, supervision of the memorial service and staff to assist with the service. Staff services for Memorial Service (without remains present) at Other Facility. Our services include coordinating the memorial service arrangements, supervision of the memorial service, and staff to assist with the service. Equipment and staff services for graveside service. Our services include accompaniment of remains to cemetery, supervision of graveside service, and staff to assist with the service. Immediate Family Viewing (spouse, children, parents). Our services include staff and facilities for a private nonpublic viewing for immediate family as defined as spouse, children and parents for one half hour TRANSPORTATION Transfer of deceased from place of death to funeral home (within a 50 mile radius) Additional distance will be charged at $2.50 per loaded mile. Funeral Coach (Hearse) (within a 50 mile radius) Additional distance will be charged at $2.50 per loaded mile. Limousine (7 passenger) (within a 50 mile radius) Additional distance will be charged at $2.50 per loaded mile. Flower car / Utility vehicle (within a 50 mile radius) Additional distance will be charged at $2.00 per loaded mile Transportation to Phoenix Sky Harbor Airport Delivery of cremated remains to local cemetery 65.00
4 FORWARDING REMAINS TO ANOTHER FUNERAL HOME This charge includes removal of remains, basic service of staff, necessary authorization, embalming and transportation of remains to Phoenix Sky Harbor Airport (excluding shipping charges and shipping containers). This charge does not include visitation or ceremonies and applies to shipping within the continental U.S. only RECEIVING REMAINS FROM ANOTHER FUNERAL HOME This charge includes basic service of staff, transportation to local airport or other shipping destination, and transportation of remains to cemetery or crematory. This charge does not include visitation or ceremonies DIRECT NON-CEREMONIAL CREMATION If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without outside covering). The containers we provide are alternative containers and other cremation containers, which include containers made of wood, containers made of fiberboard, and containers made of other composite materials. See descriptions below. We also offer a selection of caskets designed for cremation. Range: To Our charges for direct cremation (without ceremony) include removal of remains, transportation to crematory, service vehicle and minimum services of staff. The crematory fee is included. 1. Direct cremation with container provided by the purchaser (All such containers must meet any applicable state or crematory requirements) Direct cremation with Alternative Container (Basic Cardboard Box)
5 IMMEDIATE BURIAL Our charges for immediate burial (without any rites or ceremonies at the funeral home, graveside or elsewhere) include removal of remains, transportation to cemetery, service vehicle and basic services of staff. 1. Immediate burial with container provided by the purchaser (All such containers must meet any applicable state or cemetery requirements). Range: To Immediate burial with simple basic casket ( Nolan casket) Immediate burial with casket selected from our facility. (In addition to cost of casket) - see Casket Price List for casket prices ALTERNATIVE CONTAINERS (Refer to the Casket Price List as provided at the funeral home.) 13 Regency Mortuary What to do when a loved one dies A survivor's checklist. IMMEDIATELY FOLLOWING THE DEATH, YOU SHOULD: 1. Contact our funeral home to take your loved one into our care. Regency Mortuary (623) (24 hours a day) 2. Contact your minister. 3. Alert immediate family members and close friends. 4. If employed, contact the deceased's employer. 5. If applicable, notify agent under Power of Attorney. 6. Alert the executor of your loved one's Will. 7. Notify religious, fraternal, and civic organizations that your loved one was a member of. 8. Notify your attorney regarding the probate of the estate. 9. Arrange for the care of any dependents. 10. If the deceased had any pets, arrange for their immediate care. 11. Remove any valuables from the deceased's home, secure the residence, and take steps to make the home appear to be occupied (for example, use of lamp timers). 12. Arrange for the disposal of any perishables left In the deceased's home- such as food, refrigerated Items, and existing refuse. 13. Alert the Post Office to forward the deceased's mall. 14. Locate loved one's important documents: Will Social Security card Military discharge papers (DD-214) Copy of funeral prearrangements Birth certificate Marriage license Life Insurance policies

Compile the following Information that the funeral home will need In order to finalize the death certificate: Deceased's first, middle, and last name Deceased's Malden Name (If applicable) Deceased's Home Address Deceased's Social Security Number Deceased's Date of Birth Deceased's Date of Death Deceased's Age Deceased's Gender Race/Ethnicity Marital Status Spouse's first and last name Deceased's highest level of education attained Deceased's Occupation Deceased's Place of Birth {City and State) Deceased's Father's Name Deceased's Mother's Name If your loved one was a Veteran Entered Service Date Entered Service Place Service Number Separated from Service Date Grade, Rank or Rating Separated from Service Place Organization and Branch of Service
15 WITHIN ONE MONTH OF THE DEATH, YOU SHOULD: 1. Consult with an attorney about probate. 2. Meet with an accountant to discuss estate taxes. 3. File claims with life Insurance companies. 4. Contact the Social Security Administration and other government offices that may have been making payments to the decedent. If the decedent was your spouse, inquire about your eligibility for new benefits. 5. Notify the Registrar of Voters. 6. If the deceased's home Is unoccupied, cancel unnecessary home services, such as newspaper delivery, cable service, etc. 7. Cancel deceased's prescriptions. 8. Contact the Department of Motor Vehicles to cancel deceased's drivers license and transfer titles of all registered vehicles. 9. If your loved one was a veteran, inquire about benefits that you may be entitled to through the VA. 10. Contact the deceased's employer. Inquire about any 401 (k), pension, or company benefits that the decedent may be entitled to. 11. Notify all 3 credit reporting agencies. 12. Obtain a current copy of the deceased's credit report. 13. If the death was accidental, verify whether benefits are available on existing insurance policies. 14. Check for any life Insurance benefits available through existing credit card or loan accounts. 15. File any outstanding claims for health Insurance or Medicare benefits. 16. Obtain copies of deceased's outstanding bills. 17. Locate and/or obtain other important paperwork of the deceased that will be necessary for the settlement of their estate: Certified Copies of the Death Certificates Real estate deeds and titles Stock certificates Real estate titles Loan paperwork Bank and retirement account statements 18. Advise ail creditors In writing that a death has occurred. 19. Change ownership of assets and lines of credit. 20. Update your Will.
16 21. Update beneficiaries on your life Insurance policies, if necessary. 22. Send acknowledgement cards for flowers, donations, food, or any significant gestures of kindness. Also remember to thank pallbearers. 23. Organize and distribute decedent's personal belongings. 24. Remove love one's from marketing and mailing lists.
