2 The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. SIGNATURE SERVICE PACKAGES Traditional Visitation and Ceremony Package (Burial)... $ The Traditional Package includes these services: Basic services of Funeral Director and staff, embalming, other preparation of remains, use of facilities, staff & equipment for visitation in Rose or Wedgwood Room, or at Eureka (Eureka $100 less), use of facilities, staff & equipment for funeral ceremony at The Schrader Funeral Home or other location, transfer of remains to The Schrader Funeral Home, motorcycle procession escorts, and casket coach (hearse). Same-Day Visitation and Ceremony Package... $ The Same-Day Package includes basic services of Funeral Director and staff, embalming, other preparation of remains, use of facilities, staff and equipment for funeral ceremony at The Schrader Funeral Home or other location, one hour of visitation, transfer of remains to The Schrader Funeral Home, motorcycle procession escorts, and casket coach (hearse). Graveside Ceremony with Viewing Package... $ This complete Graveside Package includes basic services of Funeral Director and staff, embalming, other preparation of remains, use of facilities, staff and equipment for one hour of visitation, use of staff & equipment for graveside service, transfer of remains to The Schrader Funeral Home, motorcycle procession escorts, and casket coach (hearse). Cremation with Memorial Gathering (Memorial Visitation) Package... $ The Cremation Memorial Gathering Package includes basic services of Funeral Director and staff, other preparation of remains (limited), use of refrigeration facility, use of facilities, staff and equipment for gathering in Rose or Wedgwood room or at Eureka (Eureka $100 less), transfer of remains to The Schrader Funeral Home & Crematory, and cremation process. To substitute a memorial service in the chapel in lieu of a gathering, subtract $70; to add a memorial service in addition to the gathering, add $525). IMPORTANT NOTE: Funeral merchandise, such as a casket, outer burial container, visitor s registry, memorial cards, acknowledgment stationery, flowers, urn, etc. are not included in the above packages. Cash Advance items, such as sales tax, honorariums, newspaper notices, death certificates, etc. are not included in packages, nor are items not specifically listed.

3 Basic Services and Overhead of Funeral Director and Staff... $ Our charge includes, but is not limited to, arrangement conference with family and responsible parties; arrangement of funeral, cremation, burial and visitation details; preparation, filing and obtaining the death certificate and other necessary authorizations and permits; recording vital statistics; preparation and placement of the obituary, funeral and news notices; filing and obtaining other forms and claims; coordination of service plans with parties involved in the final disposition of the deceased, (cemetery, crematory, vault companies, florist, clergy, musicians, livery, etc.). Also included in this charge are overhead expenses relative to our facilities such as sheltering of remains, professional licensing, legal and accounting fees, insurance, building and utility expenses, parking lot and grounds, maintenance, taxes, equipment, furnishings, inventory costs, recordkeeping and secretarial and administrative expenses. This fee for our basic service and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials and forwarding or receiving of remains). Embalming... $ Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. Direct Cremation, range of prices... $ to $ Our charge for a direct cremation includes basic services of funeral director and staff, assistance with newspaper notices and obituaries, memorial service planning assistance, care and unlimited refrigeration of remains, transfer of remains to our funeral home and crematory, and crematory charges. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The container we provide is a cardboard container. All cremations are performed at our on-site Schrader Crematory in Ballwin unless we tell you, or you request otherwise. 5 S SIMPLE PACKAGES When cost is of the utmost concern and funds are limited... ALL charges for SIMPLE arrangements must be paid in full at time of arrangement conference by cash, check or credit card. Insurance assignments are not accepted for this service, and you cannot pre-pay at this price. At the SIMPLE price point, we do not prepare or place newspaper obituary notices, offer service or interment coordination, or any service not specifically listed. No deletions or additions may be made to the SIMPLE packages and there are NO exceptions. For additional services, see other package offerings. SIMPLE Cremation... $ Local transfer of remains to funeral home, brief in-person conference at our Ballwin location with licensed funeral director to execute contracts, documents and authorizations required for cremation, Simple alternative cardboard cremation container, brief one-person identification of the remains before cremation (required), limited refrigeration of the remains not to exceed 48 hours, and cremation process. SIMPLE Anatomical... $ Local transfer of remains to funeral home, brief in-person conference at our Ballwin location with licensed funeral director to execute contracts, documents and authorizations, limited refrigeration of the remains not to exceed 48 hours while verification is completed with medical school, and transfer of remains to medical school. BUDGET PACKAGES Graveside Ceremony Package (no viewing)... $ Local transfer of remains to funeral home, in-person arrangement conference at our Ballwin location with funeral director to execute contracts, documents and authorizations and to complete details, limited preparation of remains for burial (no embalming), refrigeration of remains for up to two days, transfer of remains to cemetery and funeral director to conduct graveside ceremony. Value Burial Package with Brief Visitation & Ceremony (a savings of $1035)... $ Value Burial Package with Brief Private Viewing & Ceremony (no embalming or cosmetics) Local transfer of remains to funeral home, in-person arrangement conference with funeral director to execute contracts, documents and finalize details, embalming and other preparation of remains, use of facilities, staff and equipment for same-day visitation and funeral ceremony at the funeral home (up to two hours total), transfer of remains to cemetery, and 20 gauge steel Apollo casket. Services will be scheduled for the funeral home s convenience. No procession and no graveside ceremony. No additions or deletions from this package and there are no exceptions.

6 TERMS OF PAYMENT At the completion of the arrangement conference with the funeral director, you will be presented with a Statement of Funeral Goods and Services Selected, which is a contract between you and Schrader Funeral Homes & Crematory. This Statement lists the services and merchandise we will provide for you, and the terms of the payment you will make to Schrader for rendering the services and merchandise. You, or the parties responsible for payment, will be required to sign this contract, and you will be given a copy. There are often items on the Statement that are estimated, for instance newspaper notices, mileage charges and other items for which we do not have a final amount at the time of the arrangement conference. We make an educated estimation of these charges, however the total and final cost will be added to your contract later. Payment is expected at the time of the arrangement conference for all of the cash advance items (Column B on the Statement), and 50% of the funeral home s charges (Column A ). Cash, check and/or VISA, MasterCard, Discover or American Express are accepted for this payment. The balance of the charges is due 20 days from the date of the contract. If payment is not received in this time period, we may charge a fee for late payment, as explained on the contract. You may assign an insurance policy to pay all or part of the expenses. Insurance policies will be verified with the insurer, and information and signatures will be obtained from the beneficiary in order to assign the proceeds of the policy to Schrader. If the insurance proceeds are less than the amount necessary to pay our bill, they will be applied toward the account, with the balance remaining the responsibility of the purchaser who signed the contract with Schrader. There are more stringent payment terms for the two SIMPLE packages, which require immediate and full payment at the time of the arrangement conference. Insurance policies cannot be assigned as payment of SIMPLE services. We offer services and merchandise in a widespread price range to accommodate the wide range of client families we serve. If you are in a hardship situation, your funeral director will make suggestions to reduce costs, with your cooperation and understanding.
