However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. PROFESSIONAL SERVICES & FACILITIES Minimum Services of Funeral Director and Staff $ Our charge includes funeral counseling, necessary arrangements, recording vital statistics, securing permits, filing and obtaining death certificates and other forms and claims, preparation of necessary notices, and coordination of service plans with parties involved in the final disposition of the deceased. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. This fee is already included in our charges for direct cremations, immediate burials and forwarding or receiving remains. Embalming $ Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. Other Preparations of the Deceased $ Includes cosmetology, dressing and casketing. Additional charge for autopsied cases and special organ donation. Facilities and Services $ For Visitation or Viewing at Our Facilities or your Church. The services of our staff necessary in coordination of the visitation or viewing, provide the necessary personnel to answer telephone inquiries, staff public functions, handle floral and memorial tributes, and all other duties of the staff necessary beyond the basic minimum services of the staff. Facilities and Services $ For Funeral Ceremony or Memorial service at Our Facilities or your Church. The services of our staff necessary in coordination of the visitation or viewing, provide the necessary personnel to answer telephone inquiries, staff public functions, handle floral and memorial tributes, and all other duties of the staff necessary beyond the basic minimum services of the staff. 1

2 TRANSPORTATION Up to 75 miles. Additional mileage is $1.95 per mile by our staff. Other companies transport services will be based upon their charges. Transfer of Deceased from location of death $ The care, removal and transport of the deceased to our office. Funeral Coach $ Use of hearse and staff member to transport casketed deceased or cremated remains to Church, Cemetery or other facility. Service Vehicle $ Use of vehicle and staff member to transport flowers and equipment to Church, Cemetery or other facility. TOTAL OF COMPONENTS $2, ADDITIONAL SERVICES AND MERCHANDISE Graveside Service $ Coordination and supervision of the service, equipment and staff necessary to conduct the graveside service. 100 Memorial Folders, Thank You Notes and Register Book $ Memorial DVD $ One memorial DVD that consists of 25 pictures provided by the family. Receiving of Remains from another Funeral Home $1, This charge includes the minimum services of staff, temporary protective shelter of remains, transportation of remains to cemetery and necessary services of staff. This charge does not include merchandise or other services. Forwarding of Remains to another Funeral Home $ This charge includes removal of deceased, the minimum services of staff, embalming, local transportation to airport and minimum shipping container. Does not include air-cargo charges, merchandise or use of facilities for visitation or ceremonies prior to forwarding the deceased.
