Prior to drafting any contract for goods or services, the responsible party or the decedent s survivor who is handling the funeral arrangements is entitled to receive a copy of any preneed agreement in the possession of the funeral establishment that has been signed and paid for, in full or in part, by or on behalf of the decedent. The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. PAYMENT POLICY ***Payment is expected at the time the arrangements are made. No services will be performed until full payment is received. We except cash, personal check, Visa, MasterCard, Discover and American Express. Assignable Insurance policy after verification, 4% on amount claimed or minimum $100. PUBLIC VIEWING It is Oakmont Mortuary s policy that embalming is required when there is a public viewing. BASIC SERVICES OF THE FUNERAL DIRECTOR AND STAFF AND OVERHEAD: This fee for our basic services will be added to the total cost of funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains.) This fee includes the following: Personnel available 24 hours a day, 365 days a year to respond to initial call Arrangement Conference Coordinating service plans with cemetery, crematory, and/or other parties involved in the final disposition of the deceased Clerical assistance in the completion of various forms and permits associated with a funeral Also covers overhead, including facility maintenance, equipment and inventory costs, insurance and administration expenses, and governmental compliance $ 2,
2 II. PREPARATION OF THE BODY: Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. Embalming $ Other Preparation of the Body: Dressing, casketing, sanitation care, cosmetising, (any or all) of deceased or supervision thereof. $ Rental of any Room for viewing per hour over the contracted time. $95.00 Hairdressing $ Special care for autopsied cases $ Special care for repair following anatomical donations. $ Restoration (Per hour/where necessary) $ Refrigeration (California law requires refrigeration of an unembalmed body after 24 hours.) Each additional day after 72 hours will be charged at $ per day $ Family viewing of unembalmed person for 30 minutes, up to 8 people. (Immediate Family) $ III. USE OF FACILITIES AND STAFF: Use of Facilities and Staff Services for a Visitation of 1-4 Hours (Per day) (Our services include set-up of visitation area, placement of casketed remains, display of floral arrangements, supervision of and attendance during the visitation.) $ Use of Staff Services and or Facilities for Funeral Service or Memorial Service (without remains present) in Our Chapel, at Other Facility or location, or at Graveside (Our services include coordinating the funeral arrangements, supervision of service, and staff to assist with the Funeral or Memorial ceremony.) Does not include facility rental fees for outside venues. $ Chapel Rental & Staff of Oakmont for families who have used other Funeral Facilities (Oversight and Supervision by Oakmont) (Mon Fri.) ($2000 weekends, Holidays) $1, Disinterment Fee (requires a licensed Funeral Director) (Does not include refrigeration or new casket) $ Emergency filing fee to expedite permit for burial or cremation within 2 days. Additional fee. $ (Not all requests are possible) Refile permit (does not include State required permit fee) $ Additional charge for Use of Facilities and Staff for Evening Service $ Additional charge for Use of Facilities and Staff on Sat/Sunday or Holidays ( $1000 if Holiday occurs on a Sat/Sun) $ Celebrant (when available) $ Other use of facilities - Preparation Room $ Ritual bathing (between the hours of 9:00am -3:00pm) or use for Private Autopsy (Mon Fri.) $ Additional staff per hour: $
3 An Insurance Assignment processing fee of 4%, or minimum of $100 will be charged for the amount claimed. IV. TRANSPORTATION: Transfer of deceased from place of death to funeral home (within a 20-mile radius) Additional distance will be charged at $3.00 per mile. $ Additional person when required for transfers (residence, stairs, family present, size consideration) $ Funeral vehicle (e.g. Hearse) (within a 20-mile radius) Additional distance will be charged at $3.00 per mile. $ Base fee (First 20 miles) used on long distance transfers. Additional distance at $3.00 a mile. $ Family vehicle (e.g. Limousine)(Maximum 3-hour usage and within a 20-mile radius) Additional distance will be charged at $3.00 per mile and/or $ for each additional hour or portion thereof. (Tip not included) 48 hour cancellation needed. $ Transfer to or from Oakland or San Francisco airport Additional distance will be charged at $3.00 per mile. ($100 additional after business hours or holidays) $ Transfer of deceased from funeral home to place of autopsy and return to funeral home (20 miles) $ Utility/flower van and driver within 20 mile radius. Additional miles at $3.00 a mile. $ V. FORWARDING REMAINS TO ANOTHER FUNERAL HOME: This charge includes removal of remains, basic service of staff, necessary authorizations, embalming, use of preparation room, bathing and handling of remains and transportation to local airport or other place of shipment within a 20-mile radius, Archive fee. (excluding shipping charges and shipping container). This charge does not include visitation or ceremonies. This charge applies to shipment within the continental U.S. only. $ 2, VI. RECEIVING REMAINS FROM ANOTHER FUNERAL HOME: This charge includes basic services of staff, transportation from local airport or other shipping destination and transportation of remains to cemetery or crematory, Archive fee. This charge does not include visitation or ceremonies. $ 2, VII. DIRECT CREMATION $10, to $2, If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are alternative containers and other cremation containers, including containers made of wood, containers made of fiberboard, and containers made of other composite materials. We also offer a selection of cremation oriented caskets. Our charges for direct cremation (without ceremony) include removal of remains, transportation to crematory, refrigeration and basic services of staff. Archive fee. The crematory fee is included. (Does not include alternative container or cremation casket. $2, Direct cremation with container provided by the purchaser (All such containers must meet any applicable state or crematory requirements.) $ 2, Direct cremation with minimum alternative container (Standard Brown Medium density fiberboard Alternative container with dark stain color. (Interior not included) $2,
4 Due to the time it takes for document procurement and filing of required permits and certificates cremated remains will normally be available to a family 4-7 business days after the date of arrangements. If cremated remains are needed sooner, a RUSH CREMATION fee may be added for $ and the cremation will be performed within 1 business day after filing of the death certificate and issued Permit of Disposition. (Not all requests are possible. ) $ VII. IMMEDIATE BURIAL: $ 14, to $ 3, Our charges for immediate burial (without any rites or ceremonies at the funeral home, graveside or elsewhere) include removal of remains, transportation to cemetery, refrigeration, dressing and casketing and basic services of staff, Archive Fee. 1. Immediate burial with container provided by the purchaser (All such containers must meet any applicable state or cemetery requirements.) $ 3, Immediate burial with minimum casket from our facility $ 5, Immediate burial with other casket selected from our facility (In addition to cost of casket See Casket Price List for casket prices) $ 3, Urn Placement Service If a family wants to Schedule a placement date and time this fee will be utilized. When a service of any size with or without clergy is requested at Oakmont Memorial Park, an Oakmont Funeral Home representative will be at the service. (Does not include weekends, Holidays or Overtime fees.) $ VIII. CASKETS: (A complete price list will be provided at the funeral home.)
