3 These prices are effective October 4, 2006, but are subject to change without notice. The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. You may choose only the items you desire. If you are charged for any items you did not request, we will explain the charges on the written memorandum. Please note that there may be charges for items such as cemetery fees, flowers, and newspaper notices. PROFESSIONAL FEES AND SERVICES SERVICES OF FUNERAL DIRECTOR AND STAFF $ 725 Our professional service charge includes, but is not limited to, consultation with family and clergy, arrangement and direction desired, preparation and filing of necessary notices, authorizations and consents, and other services and attendance prior to, during, and following the funeral including coordinating with those providing other portions of the funeral (cemetery, crematory, vault companies, musicians, etc.). Also included in this charge are overhead expenses relative to our facility, such as insurance, maintenance, utility expenses, secretarial and administrative costs, and equipment and inventory expenses. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. This fee is already included in our charges for direct cremation, immediate burial, and receiving of remains. If making an insurance assignment for payment of funeral home charges, a $100 processing fee will be added to the total charges. EMBALMING $ 450 Embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. OTHER PREPARATION OF THE BODY $ 195 This service includes restoration, the usual cosmetology, dressing, and casketing of the body. SANITARY CARE $ 185 Minimal care and disinfection of remains when in our care and custody, includes basic preparation and refrigeration facilities up to 48 hours. Also care and disposal of regulated medical waste and compliance with state and federal OSHA blood borne pathogen requirements. 1

However, many cemeteries require that you have such a container to prevent the grave from settling. Either a grave liner or a burial vault will satisfy these requirements. 9 Direct Cremation $ 730 to $ 4,025 *All of our cremation services include, but are not limited to, consultation with the family, clergy, (when needed) arrangement and direction desired, (when needed) preparation and filing of necessary permits and notices, authorizations and consents, other services and attendance prior to cremation, sanitary care and removal from the place of death, and transportation to crematory in Temple, Texas. Also included are the crematory charges and return mailing of cremated remains. Additional trip to crematory to pick up cremated remains is not included in these charges. (Additional miles are $2.50 per mile one-way). Also included in this charge are overhead expenses relative to our facility such as insurance, maintenance, utility expenses, secretarial and administrative costs, and equipment and inventory expenses. *This fee for our basic services and overhead will be added to one of the following cremation service options of your choice. If you do not want one of the following service options then your cremation service price for a direct cremation is $ *If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the body and can be of materials like fiberboard or composition material (with or without outside covering). Direct cremation with container provided by the purchaser is $ (all containers must meet any applicable state crematory requirements). *The cost of a direct cremation with a minimum alternative container provided by Brazos Funeral Home is $ Professional Display of Floral Tributes Only Includes: Staff to professionally display floral tributes at the place of service other than funeral home. Also includes transportation of floral tributes to two locations after the memorial service. 12 FORWARDING OF REMAINS TO ANOTHER FUNERAL HOME $ 1,295 This charge includes transport of remains to funeral home, embalming, cosmetology, dressing and casketing of remains, limited services of funeral director and staff, and necessary authorizations. It does not include casket, air tray, any transportation other than initial transfer to funeral home or any of the services of the staff or use of facilities for any ceremonies prior to forwarding the body. RECEIVING OF REMAINS FROM ANOTHER FUNERAL HOME $ 895 This charge includes limited services of funeral director and staff for receiving the casketed remains, temporary shelter of remains, and transportation to cemetery within 40 mile radius of Waco. This charge does not include visitation or ceremonies. DISINTERMENT $ 395 This is our professional charge for securing permits, arranging disinterment, state health department fee ($25), etc. Additional charges may include the opening and closing of the grave at disinterment site, container, the opening and closing of the grave at interment site, mileage, cemetery fees, etc. BODY SENT TO MEDICAL SCHOOL $ 295 This charge includes transport of remains to funeral home, (40 mile radius) or personnel to receive remains at the funeral home, sanitary care, temporary shelter of remains and professional staff services for handling the body and necessary paperwork. IMMEDIATE BURIAL $ 955 Our charge for an immediate burial without ceremony includes removal and temporary shelter of remains, sanitary care, transportation to cemetery (40 mile radius), necessary services of staff and authorizations. Embalming if desired, and cemetery charges are not included in this price.
