2 Be an Informed Consumer Purchasing a funeral service is one of the most expensive investments we make in our lifetime. Like purchasing a home or a car, you wouldn t call around and just go with the lowest price you could find. Your purchase does need to fit your available funds, but what you get for those dollars can make a huge difference depending on where you go. Some things to consider: What if you are not satisfied with your services? Howe-Peterson carries a 100% satisfaction guarantee with all of our services. We monitor surveys and follow up with our families to make sure that we are providing you with distinctive and unparalleled service. Have you toured the facility? You should always tour the facility before you commit to services there. Just because a relative had services there 10 years ago does not mean the business has been maintained at that same level. Howe- Peterson boasts state of the art facilities and well maintained equipment and vehicles. For your convenience we have two locations to serve you, the Dearborn Chapel on Michigan Avenue in Dearborn and the Taylor Chapel on Telegraph Road in Taylor. When you visit the funeral home are the doors unlocked? Do you need to call and set up an appointment so that someone can meet you there? The hours at Howe-Peterson are 9:00 a.m. to 9:00 p.m. We are available 365 days per year. Note: If we are not serving families our evening location hours may be limited. We are always available via telephone 24 hours a day 7 days a week. You can also access our website, which contains information on current funeral services, pricing, prearranging, and grief support. Who will provide support during the visitation and service? Will there be staff present or just a sign with an arrow pointing to the general direction of the chapel? Our staff is highly trained and dedicated to provide support to your family and guests throughout the memorialization process. What if you want to preplan a funeral and finance it? Will you be talking to a licensed funeral director or an insurance sales person? Charles Gury is our Certified Advance Planning Professional and is also a licensed Funeral Director. He has been a part of our family for over 27 years. He is licensed in the state of Michigan to process insurance sales, and is certified for preplanning through the Michigan Funeral Director s Association. Many people benefit from grief support after the funeral. Does the funeral home have a program? Howe-Peterson has one of the most complete grief support programs in the state of Michigan. We have ongoing groups that meet weekly, a survivors of suicide group that meets twice per month, and a series that meets in the spring, fall and winter. Our website provides overviews of these programs, a library of articles by Dr. Alan Wolfelt PhD, an internationally noted author, educator and grief counselor, and is an excellent guide to other grief resources. Online Obituary/Guestbook We offer each of our families a free online obituary and guestbook on our website. The online guestbook allows family and friends to share stories and photos of your loved one. It will be on our website forever and people can continue to sign it forever. A new feature is the ability to order and send a sympathy card while signing the online guestbook. Preplanning Whether you are looking for general information or desire to begin the process of preplanning a burial or cremation, our website offers complete information and an online preplanning form to get things started. Immediate Need Forms There are two forms under this menu option: The Death Notice Form and the Death Certificate Form. Both can be printed and completed manually, or filled out online and submitted electronically. These forms help you to see what information you will need in order to submit a Death Notice or to complete the application for a Death Certificate. You can compile the information in advance of coming to the funeral home and it will help you to be prepared for the questions we will ask. Grief Support Howe-Peterson boasts one of the most complete grief support programs in the state. Most often we are not prepared for the emotions and behaviors associated with loss. Please know that you are not alone. Howe-Peterson wants to be your support system to help you and your family through your grief. We encourage you to attend one of our free, non-denominational groups when you are ready. Our groups address special needs for adults, teens, children, older adults and those who have lost a loved one to suicide. We are unique in offering such a vast array of grief support groups. Howe-Peterson could not offer so many groups and individualized attention to each family we serve without the expertise of Elaine Lamparski, Howe-Peterson s Aftercare Coordinator. Elaine possesses more than 25 years of funeral service expertise and has known profound losses in her life, so she understands the experience of loss and is empathetic to those who are grieving. Below is a suggested list of who should receive one. If someone did something extra special, including a note would be a nice gesture. Those contributing to memorial organizations or churches Clergy Those who participated in the service (music, readings, etc.) Those who sent flowers Pallbearers Those who brought food Anyone who helped in special ways - 2-

8 Choices Related To Payment/Benefits and Financial Matters In an effort to provide the finest services and merchandise possible, we are dependent upon our client families for determining payment terms prior to services being rendered. Third Party Items are expenses incurred that are not Howe-Peterson charges and are paid for directly by the family. Some examples of third party items are, but not limited to, cemetery charges, newspaper notices, church, clergy and organist fees. Funeral Home Items are charges incurred with Howe-Peterson and are to be paid in full by the day of the service. These charges may be secured by (A.) Cash, check, debit or credit (Visa, MasterCard, Discover Card or American Express) (B.) verified assignment of a life insurance policy or (C.) fully funded prearranged funeral (additional monies may be owed for non-guaranteed items). Howe-Peterson, Inc. does not enter into contracts with a decedent s estate. In cases where the decedent s estate must stand the burden of payment, we ask that a family member pay the charges and file a claim against the decedent s estate. Financial responsibility must be mutually agreed upon by at least one family representative and a Howe- Peterson representative. You may have to make decisions involving filing a will; revision of wills for survivors; transfer of assets such as bank accounts, safe deposit boxes, trust funds, and deeds/mortgages; charge accounts; cars; and probate and tax procedures such as filing estate tax returns. We are able to refer you to a selection of competent attorneys, accountants, and financial advisors. We will assist you in filing life insurance claims at your request. SOCIAL SECURITY A lump sum death benefit of $255 is available for surviving spouse or minor children, if qualified. Call Social Security at (800) , Monday through Friday, from 7:00 a.m. to 7:00 p.m. You may apply for benefits by telephone or by going to any Social Security office. You may need some of the documents shown on the list below. If you don t have a document you need, Social Security will help you obtain it. Your Social Security number and the deceased s Social Security number A death certificate Proof of the deceased worker s earnings for last year (W-2 forms or self employment tax return) Your birth certificate Your marriage certificate if you are applying for benefits as a widow/widower Your divorce decree if you are applying for benefits as a divorced spouse Children s birth certificates and Social Security numbers if applying for children s benefits Your checking or savings account information if you want direct deposit of your benefits You will need to submit original documents or copies certified by the issuing office. You may mail or bring them to the Social Security office. VETERANS BENEFITS Burial benefits and a Presidential Memorial certificate are applied for by Howe-Peterson. A casket flag and grave marker are available to those who qualify. We will apply for the government marker at your request. Please note that you must pay the cemetery for the marker installation fee before we will order the marker. For more information regarding Veterans Benefits you may call (800) There is a county based Soldiers and Sailors Death Benefit for those who qualify. We are available to assist you with transportation to the Wayne County offices. -3-

9 What is a Funeral? Funerals are an important step in the grieving process, as well as an opportunity to honor a life lived. They offer loved ones a caring and supportive environment in which to share thoughts and feelings about the decedent. Narrowly defined, a funeral is an organized ceremony (burial, cremation or memorial) at which friends and loved ones acknowledge the death of a family member, friend or associate, and take note of that life and the person s role in the lives of others. When the ceremony is religious in nature, it affirms the values and beliefs of a particular faith. As you make funeral arrangements for yourself or a loved one you will need to determine if it will be a burial or cremation. That statement is a bit misleading because often people are not buried but are in crypts or mausoleums above ground. With cremation you will have to decide what will be done with the cremains. Cremains can be buried, placed in a niche above ground, scattered, or placed in one or more decorative urns. They can even be placed in jewelry. Often people have the misconception that if they choose cremation then there will not be an opportunity for a visitation or viewing. Simply put, you can do whatever you prefer. There are no limitations. As you can see on page 6, our Funeral Service Packages with visitation are for burial or cremation. What is a Licensed Funeral Director? In the state of Michigan a licensed funeral director is also a licensed embalmer, who has completed at least three years of college including a year of mortuary science college, and one year of residency or apprenticeship. Funeral directors, also called morticians and undertakers, arrange the details of funerals, taking into account the wishes of the deceased and family members. They provide support and guidance to families at a very overwhelming time of their life. At Howe-Peterson funeral directors transfer the deceased into our care, perform the embalming and other preparation of the deceased, conduct ceremonies that honor the deceased as appropriate for the spiritual needs of the family, and carry out final disposition of the deceased. Our directors are honored to arrange the details and direct these tasks for the grieving families, taking great pride in being able to provide comfort to the family. Funeral directors also prepare death notices and have them placed in the newspaper, arrange for clergy and pallbearers, coordinate services provided by a cemetery, and prepare the site of the service to comply with the wishes of the family, including transporting flowers when necessary. They also prepare decedents for shipment for out of state services

You may choose only those items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. This list does not include prices for certain items that you may ask us to buy for you, such as cemetery or crematory services, flowers, and newspaper notices. The prices for those items will be shown on your bill or the statement describing the funeral goods and services you selected. The only warranties, expressed or implied, granted in connection with goods sold with this funeral service are the expressed written warranties, if any, extended by the manufacturer thereof. NO OTHER WARRANTIES, EXPRESSED OR IMPLIED INCLUDING, BUT NOT LIMITED TO, WARRANTIES OF MERCHANTABILITY OR FITNESS FOR A PARTICULAR PURPOSE, ARE EXTENDED BY THE SELLER. Funeral Service Packages are available for less than the total of the individual General Price List items Basic Services of Funeral Director and Staff...$2, This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials and forwarding or receiving the deceased.) Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. Michigan law requires embalming when death was caused by certain infectious diseases or, with very limited exceptions, when the deceased is not taken to their final destination (such as a cemetery or crematory) within 48 hours. 14 Basic Cremation Services All cremations must take place in a combustible container. If you want to arrange a direct cremation, you can use an alternative container. Alternative containers encase the deceased and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are cardboard, fiberboard or wood. You will have peace of mind knowing that a professional licensed funeral director that is an employee of Howe-Peterson will do the embalming of your loved one. Our directors are there through every step of the care we provide including dressing, cosmetics, hairdressing (by a licensed hairdresser), and casketing the decedent for final presentation for visitation, services and/or ceremonies. If you choose a basic burial or cremation what type of care will your loved one receive? Our Signature Care prepares your loved one for identification. This service includes bathing, shampooing of hair, shaving and dressing the decedent in clothing you provide. You are also allowed a brief period of private viewing time. There is no charge for this. Be aware that this is not typical of all funeral homes. Cremation has unique concerns. Below are some important points to consider. Who performs the cremation? Howe-Peterson uses a crematory exclusively contracted to Howe-Peterson. One of our staff will be with your loved one throughout the cremation process. This includes transferring to and from the crematory. May you witness/participate in the actual cremation? Yes you may. We retain complete control at the crematory. What type of container is used? At Howe-Peterson there is a range of containers available. Our most basic construction is fiber board which is a sturdier shell construction than regular cardboard, and includes a pillow and mattress. There are other options ranging from wood composite to veneers and hardwood. What happens after the cremation? At Howe-Peterson you will be notified that the cremains of your loved one have been returned to the funeral home from the crematory. We will schedule a time for you to come to the funeral home. When you arrive we will present the cremains with a small ceremony and give you a moment to reflect in private. There has been a great deal of focus on going green and eco-friendly products. Where does the funeral home stand in that regard? Much has been said about using embalming fluid and how it contains formaldehyde. Howe-Peterson has not used that type of fluid for many years. We use a non-formaldehyde embalming fluid. We have upgraded approximately 650 incandescent light bulbs to CFL s. This may seem like a small thing, but the benefit is a reduction in electrical consumption of approximately 75% on exterior and interior building lighting; we offer printing on recycled paper; as we replace vehicles we are replacing them with vehicles that may operate on E85; Howe-Peterson recycles paper; we utilize programmable thermostats in all of our buildings; the roof at the Taylor Chapel is eco-friendly; we offer veneer caskets (97% of a tree can be utilized in making veneer caskets, while only 36% can be utilized in making solid wood caskets); insulated garage doors were installed in our garages. Since 1873 we have been trusted to provide exceptional services to families throughout metro Detroit. In 1864 Elba Howe came to the Dearborn area where he worked for the Michigan Central Railroad as the stationmaster. In the late 1860 s he began to assist local farmers in conducting funerals and as the local undertaker stocked a few coffins in the general store. After the birth of his son Louis in 1873, the Howe and Son Funeral Home was established near the corner of Michigan and Howe Streets, where Elba continued to conduct services and supply farmers with coffins. In 1928 Edwin Peterson entered into partnership with Louis Howe and the name Howe-Peterson Funeral Home appeared on the Michigan Avenue site across from where the Dearborn location now stands. Due to growing local communities, the funeral home facilities were moved to their present location in Edwin married Kathleen Finch in 1933 and she used her business acumen along with her warm and caring presence to partner with her husband in managing the funeral home. Kathleen obtained her funeral director s license in 1956 and after Edwin s death in 1962 continued to run the funeral home along with her youngest son, Joseph. Prior to Edwin Peterson s death, a new location was established in Taylor to share his dedication and service with the downriver communities. Joseph had a vision of a new and innovative funeral home, and the transformation of the Taylor Chapel was completed in late summer 1989, before his passing in The family legacy lives on as Dorothy Peterson, Joseph s wife, stays active in Howe-Peterson.
