Burial performed at our convenience. No family present. (Direct Burial charges do not include viewing/visitation, church, chapel, memorial or graveside service, casket or burial container.) MEMORIAL SERVICE $2, Services of the funeral director and staff, local transfer of the body to the funeral home (within Kitsap County), care and shelter of the body as necessary, securing necessary documents and permits, memorial service or ceremony at our chapel or church, local transportation to crematory, cremation fee, obituary preparation and submission, online memorial, filing of insurance forms, filing of VA paperwork. A casket, cremation container or alternative container is required for cremation and may be selected from the casket price list. DIRECT CREMATION $1, Services of the funeral director and staff, local transfer of body to the funeral home (within Kitsap County), care and shelter of the body as needed, securing necessary documents and permits, local transportation to crematory, cremation fee, obituary preparation and submission, online memorial, filing of insurance forms, filing of VA paperwork. A casket, cremation container or alternative container is required for cremation and may be selected from the casket price list. Direct cremation (option #1) with a cremation container selected from our showroom.. $1, plus cremation container (cremation container range is $165-$2,325) Direct cremation (option #1)with a cremation container provided by purchaser......$1, URNS: All cremated human remains must be placed into a suitable container generally referred to as an urn. A catalog of available urns and prices will be presented to you at the time of the arrangement conference. SHIPPING/RECEIVING FORWARDING OF REMAINS TO ANOTHER FUNERAL HOME...$1, This charge includes local transfer of body to the funeral home (within Kitsap County), securing necessary documents and permits, embalming, other preparation of the body as needed, obituary preparation and submission, online memorial, filing of insurance forms, filing of VA paperwork, and vehicle to Sea-Tac Airport. (This charge does not include a casket, viewing, church, chapel or memorial service, or airline charges). RECEIVING REMAINS FROM ANOTHER FUNERAL HOME OR MORTUARY.....$1, This charge includes transfer of body from Sea-Tac Airport to our chapel, temporary shelter of the body, transfer of body to local cemetery, necessary services of Funeral Director and staff.(this charge does not include a casket viewing, church, chapel, memorial or graveside service).
5 PROFESSIONAL SERVICES These prices may already be included in the service options listed on the previous page. BASIC SERVICES OF FUNERAL DIRECTOR AND STAFF $1,250 Our fee for the services of funeral director and staff includes, but is not limited to, staff to respond to initial request for service (24 hours per day); arrangement conference with family or responsible party; arrangement of services selected; preparation and filing of necessary authorizations and permits; recording vital statistics; staff assistance prior to, during, and following arrangements including coordination with those providing other portions of the arrangements, e.g. cemetery, crematory and others. Also included in this charge are overhead expenses which include, but are not limited to the following: legal, accounting, maintenance of facilities, licensing, secretarial and administrative costs, and equipment and inventory costs. The fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select. (This fee is already included in our charges for direct cremations, immediate burials, and forwarding or receiving remains.) Embalming......$400 Except in certain cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing or entombment in a mausoleum. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or direct burial. Washington State Law requires that upon receipt of a decedent, the funeral home must immediately embalm or refrigerate until the final disposition WAC ) Refrigeration of body (one time charge) $100 Special care required of autopsied body or long bone donation......$200 Other preparation of the body including dressing, cosmetology and other professional preparation of the body......$150 Crematory charges $275 Witnessed Cremation...$50 AUTOMOTIVE EQUIPMENT Basic fees apply to all vehicles used within the boundaries of Kitsap County. Additional charges will apply depending on the county/state of destination. Whether it is a traditional service with burial, or memorial service with cremation, please consider recording your specific wishes, thereby relieving the burden of trying to determine what your personal requests may have been. We offer pre-paid funeral planning as a part of our service to you and your family. Please ask us how we may be able to help you in advance. In most cases, easy monthly investments are available and the earlier in life you start, the greater the rewards.
7 Payment Policy Thank you for selecting Lewis Funeral Chapel and The Stone Chapel Poulsbo Mortuary to handle the final care of your loved one. We realize that death can come at a time when the family is not prepared financially. Please do not base your service selection solely on your ability to pay cash at the time arrangements are made. We accept Visa, MasterCard or Discover credit cards for payment of services. If preferred and approved by our credit department, contract terms up to 24 months at 1% per month interest may be arranged. Insurance assignments are also acceptable as payment, providing the insurance policy is assignable to the funeral home. Under certain limited instances, there may be funds available from the Kitsap County Indigents Program. The goods and services shown enclosed are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. Infants & Children Selections for our services, facilities and vehicles listed on our General Price List are offered at a discounted rate for infants and young children up to the age of 15. Please ask one of our funeral home counselors for a copy of our infant and children General Price List for reference. Benefits Assistance Our staff will assist in the application for and notification of Social Security, Veterans Administration, Veterans Affairs, personnel Departments and Fraternal organizations for available benefits that you may be entitled. When possible, our professional staff will also process life insurance claims on your behalf. 10 Payment Options Terms of payment are to be finalized at the time of the arrangement conference.
