3 Summary This report and grades represent our assessment of the quality of the areas of performance which were examined during this inspection. Grades for this care service may change after this inspection following other regulatory activity. For example, if we have to take enforcement action to make the service improve, or if we investigate and agree with a complaint someone makes about the service. We gave the service these grades Quality of Care and Support 2 Weak Quality of Environment 5 Very Good Quality of Staffing 5 Very Good Quality of Management and Leadership 5 Very Good What the service does well Overall the nursery provides very good opportunities for the children using the service. The nursery had very good systems which allowed parents and carers to participate in improving the quality of the care and support, environment, staffing and management. The nursery had begun to develop their approach to responsive planning for children. We saw this to be working well within the nursery environment and we saw that alongside this, outcomes for children could still be evidenced through the curriculum for excellence. The nursery demonstrated various creative methods for supporting children's learning such as floor books and mind mapping. What the service could do better The service should continue to develop the very good systems that they have to encourage child and parent/carer participation within the nursery. This will ensure children and their parents/carers views are considered and continue to contribute to the on-going development of the nursery. The service should develop a more systematic approach to nursery planning in the under 2's room. The nursery should develop this in line with best practice guidance. The service should ensure that staff are clear about infection control practices and that this is monitored through the quality assurance systems in place. Nursery Rhymes, page 3 of 26

4 What the service has done since the last inspection Inspection report continued Since the last inspection the nursery has continued to develop the ways in which they involve children and their families in developing the service. The service has opened an under 2's room since the last inspection. This area was well presented during the inspection and the nursery improvement plan clearly demonstrated the projected developments for this area. The service has been working in partnership with Dundee City Council on developing responsive planning approaches to the children's learning experiences. Conclusion Overall we found that the nursery offers children very good experiences. We found the nursery to provide a wide range of opportunities for participation with parents, carers and children. We saw that the children were engaged, confident and comfortable within the setting. Staff were enthusiastic, intuitive in their responses to children needs and children responded well to this. Where there are breaches of the Regulations, Orders or conditions, a requirement must be made. Requirements are legally enforceable at the discretion of the Inspectorate. Nursery Rhymes is located in the Broughty Ferry area of Dundee. The nursery has two rooms for children. Downstairs which is for children aged 2-5 and the baby room on the upper level which is for under 2's. The service is registered under the following conditions: Number(s) and Age(s) of person(s) to whom service may be provided: To provide a care service to a maximum of 30 children at any one time, aged from birth to five years as follows: On the ground floor of the accommodation the service may care for a maximum of 18 children aged from 2 to 5 years at any one time. 6 On the upper floor of the accommodation the service may care for a maximum of 12 children aged birth to 2 and a half years old at any one time. 7 2 How we inspected this service The level of inspection we carried out In this service we carried out a low intensity inspection. We carry out these inspections when we are satisfied that services are working hard to provide consistently high standards of care. What we did during the inspection In this service we carried out a low intensity inspection. We carry out these inspections when we are satisfied that services are working hard to provide consistently high standards of care. We wrote this report following an unannounced inspection visit on 12th November 2012 between 9.20am and 4.30pm. The inspection was carried out by Care Inspectorate Inspector Donna Borek. We returned to give feedback to the service on 14th November 2012 and follow up feedback again on the 11th January As requested by us the service sent us an annual return and a self assessment. We issued questionnaires to people using the service. Four completed questionnaires were returned before the inspection. Overall all 4 people strongly agreed with the statement that 'Overall I am happy with the quality of care my child receives in this service.' In this inspection we gathered evidence from various sources, including the relevant sections of policies, procedures, records and other documents and included: * Observations of staff working with children * What the service told us that they do well in their self assessment * Personal plans and risk assessments of children using the service * Staff records and training records * Health and safety and maintenance records * Accident and incident records * Complaints records * Discussions with: The owner and the manager The staff team The children using the service Their parents and carers Nursery Rhymes, page 7 of 26

8 Grading the service against quality themes and statements We inspect and grade elements of care that we call 'quality themes'. For example, one of the quality themes we might look at is 'Quality of care and support'. Under each quality theme are 'quality statements' which describe what a service should be doing well for that theme. We grade how the service performs against the quality themes and statements. Details of what we found are in Section 3: The inspection Inspection report continued Inspection Focus Areas (IFAs) In any year we may decide on specific aspects of care to focus on during our inspections. These are extra checks we make on top of all the normal ones we make during inspection. We do this to gather information about the quality of these aspects of care on a national basis. Where we have examined an inspection focus area we will clearly identify it under the relevant quality statement. Fire safety issues We do not regulate fire safety. Local fire and rescue services are responsible for checking services. However, where significant fire safety issues become apparent, we will alert the relevant fire and rescue services so they may consider what action to take. 9 What the service has done to meet any recommendations we made at our last inspection The recommendations from the previous inspection had been met. The annual return Every year all care services must complete an 'annual return' form to make sure the information we hold is up to date. We also use annual returns to decide how we will inspect the service. Annual Return Received: Yes - Electronic Comments on Self Assessment Every year all care services must complete a 'self assessment' form telling us how their service is performing. We check to make sure this assessment is accurate. The service submitted their self assessment on time. We spoke with the manager about using the self assessment document dynamically and filling it in as changes are made to the service. We also discussed the importance of the service outlining outcomes for the children within it. 10 Taking carers' views into account "The staffing arrangements have changed in the last year and then new members of the team have made my child feel comfortable and happy. They have made a huge effort to get to know her and have fitted in perfectly with the learning procedures in place and the care for my child is consistent and has always remained of an extremely high standard. So much so that she will miss them very much when she starts school this year." "Staff are very approachable and friendly, Nursery seems to be a step ahead of other nurseries. i.e. talking tots, kids learning French etc." "The children's needs are definitely their first priority. 11 3 The inspection We looked at how the service performs against the following quality themes and statements. Here are the details of what we found. Quality Theme 1: Quality of Care and Support Grade awarded for this theme: 2 - Weak Statement 1 We ensure that service users and carers participate in assessing and improving the quality of the care and support provided by the service. Service strengths Overall we found that the service was very good at involving children and their families in assessing and improving the quality of care and support, environment, staffing and management and leadership of the service. The service had a range of methods in place to gather children and their families' thoughts about these areas of service provision, evaluate this and feedback any changes made to the service as a result. This was based on a robust participation policy which underpinned the process and supported very good outcomes. The service provided parents with regular questionnaires with specific consultation about on-going developments within the nursery and a yearly questionnaire about overall service provision. These were given out as and when required depending on the development the nursery wished to make. The nursery provided examples of this where they had consulted with parents and children seeking their views about topics such as colours of the rooms, nutrition and menu development and home books in the under 2's room. The feedback about outcomes from these questionnaires was provided to parents and carers through either slips with specific information about outcomes or nursery newsletters. Parents were kept informed of their children's progress through an annual parents evening which offered them an opportunity to discuss their child's development with staff on a one to one basis. Parents received a report about their child at the end of term and were offered an opportunity to discuss this with staff from the nursery, ensuring that parents are kept informed and involved with their child's progress. Parents were regularly invited to come into the nursery and support children with Nursery Rhymes, page 11 of 26

12 their learning. We saw photographs of parents attending the nursery and supporting children with various activities ensuring that parents had opportunities to be a part of their child's learning and strengthening home to nursery links. We saw that the nursery keeps a book of telephone and face to face discussions with parents when dropping off or collecting their children. This demonstrated that any concerns from parents are responded to by nursery staff and also that this information is shared across the nursery allowing the whole staff team to respond to children who may require a little extra support or observation. The nursery offered parents an opportunity to attend an evening meeting to discuss Curriculum for Excellence, the Scottish Government's Framework for Education, in relation to their child's learning. The Curriculum for Excellence outcomes were also displayed on the wall of the cloakroom alongside pictures of the children. This supported parents and carers to visualise what and how their children were learning. We saw that children were consulted in various ways some of which were very creative. The nursery showed us very good examples of mind maps and floor books which were being used to gather children's thoughts, explore them further, develop and evaluate them. This was clearly underpinned by the services moving approach to responsive planning for children and demonstrated that the staff team are developing a very good working knowledge of the model. We saw that the nursery uses circle time, graphs and charts and speech bubbles as methods of evaluating children's learning and ideas. This meant that children had a variety of ways in which they could contribute to the development of the nursery. Staff used a planning system to identify, develop and evaluate children's learning using the best practice frameworks they had in place. This ensured that outcomes were being clearly identified and evaluated in line with best practice. The nursery has recently developed a Facebook page for parents and carers to receive regular and instant updates. The nursery also has a website which provides information to parents and carers and any potential service users. The nursery displayed staff qualifications and registration certificates ensuring that parents, carers and visitors to the nursery were aware of this information. Areas for improvement Inspection report continued We saw that the nursery had recently developed a new way of connecting with parents and carers using social media. The nursery did not have a policy in place to support the use of this. We made a recommendation in relation to this. (See recommendation 1) We spoke with service about ways in which they could continue to develop their self assessment documentation in consultation with children, parents and carers and Nursery Rhymes, page 12 of 26

13 stakeholders. The service identified various developments for the new under 2's room. This includes developing the environment, planning using best practice and developing the information that is gathered about children. The nursery should develop a social media policy This is in accordance with: The National Care Standards Early Education and Childcare up to the age of 16, Standard 14, well managed service. Statement 3 We ensure that service users' health and wellbeing needs are met. Service strengths Overall we saw that outcomes for children in relation to their health and wellbeing needs were very good. Children had regular access to outdoor play, fresh air and exercise in the nursery garden and we saw pictures that showed us that children are regularly taken out and about in the local community. We saw that the nursery has strong links with services in the local community such as the local library and age concern. The nursery had a very good child protection policy and staff were aware of the procedures in relation to child protection. This ensured that staff knew how to respond if they had a concern about a child. There was a very good medication policy in the nursery and this was supported by a robust procedure and recording system. This supported the nursery staff's understanding of administering medication, the information required and how to record it in a clear and structured manner. Children's information was well recorded and each child had an 'All about me' profile. This was in the process of being reviewed in consultation with parents in the under 2's room during inspection. Profiles gathered lots of relevant information about children and were reviewed regularly. This ensured that staff had information about children needs. Staff undertook regular observations of children and linked this with their learning and on-going development. Profiles identified next steps and outline how staff intended to support the child to achieve this. This made sure that children's needs were being identified and monitored as they were developing. 14 We saw that information in relation to children with additional needs was recorded well with clear identification of needs and outcomes. The information was stored securely in the office. This ensured that sensitive information about children was protected. The nursery was well resourced and children had access to reading materials. The nursery environment promoted children being able to learn independently meaning that children were able to access the resources whenever they felt they wanted to. We saw that staff interacted with children in a very positive manner. This was supported by a positive behaviour policy. The policy was robust and discussed the use of non-aggressive strategies in detail when supporting children with behaviour management. Staff were calm, confident and considerate during the inspection and children were seen to respond very positively to this. Snack and lunches were seen to be healthy and children told us that they were enjoying the food on the day of inspection. Areas for improvement Inspection report continued The nursery were currently working on a new profile for the baby room. This was in order to provide parents with better information about their child. Despite the service having a very good policy in relation to infection control practices, during the inspection we observed poor practice in relation to hand washing from staff in the under 2's room. Furthermore we observed the bathrooms to have no liquid hand soap or paper towels. This was immediately rectified when pointed out to the provider of the service. Furthermore we observed that the staff in the under 2's room did not use appropriate personal protective equipment when changing children and did not follow the nursery policy in relation to this. Staff were using sanitising liquid as a substitute for hand washing. We made a requirement in relation to this. (See requirement 1) The nappy changing equipment in the under 2's room had a burst mat and was unhygienic. This was replaced during the time of the inspection; however we made a requirement in relation to this. (See requirement 2) During the inspection we were informed that a number of staff and children had been off due to a sickness bug. The service did not inform their local Health Prevention team of this as is required by law. We made a requirement in relation to this. The provider should refer to the Health Prevention Scotland website for definitions of outbreaks. We made a requirement in relation to this. (See requirement 3) Nursery Rhymes, page 14 of 26

Timescale: Immediately 3. The provider must ensure that Health Prevention Scotland are informed of any outbreak of illness within the nursery environment as soon as is reasonably possible. Grade awarded for this statement: 5 - Very Good Number of requirements: 0 Number of recommendations: 0 Statement 2 We make sure that the environment is safe and service users are protected. Service strengths Over all we saw that the service had very good outcomes for ensuring that the environment was safe and that service users are protected. The nursery appeared to be well maintained and was clean and hazard free on the day of inspection. Staff responded quickly to potential hazards and supported children well in recognising these for themselves. On entry we saw that the service has a secure entrance to the premises with a buzzer and a camera that was responded to by staff. We saw that the nursery has a signing in book for guests. The nursery has disabled access and a disabled toilet which was clean and hazard free. The nursery has good cleaning arrangements in place, including the kitchen area. We also observed staff in the nursery cleaning up and encouraging the children to do so. This supports the children to learn about respect for their environment and helps to keep the nursery hazard free and tidy. Nursery Rhymes, page 16 of 26

17 We observed that children are supported to wash their hands before snack and lunch and furthermore after any messy play activity. The children are provided with liquid soap and paper towels. This helps to prevent the spread of infection and teaches children good hand washing practice. The nursery has very good measures in place for the storage of chemicals and cleaning materials. This prevents children having access to potentially hazardous substances. There is a very good system for recording accidents within the nursery. The policy about how staff support children and record the accident is clear and parents are given a copy of the paperwork. This ensures that staff know how to help the children and that parents or carers are kept up to date with any events throughout the child's day. Furniture is at child height and this allows very good access for children to the nurseries resources. Furthermore this means that children's work is at their height and they can see what they have done. The nursery has a garden area for children to the rear, which is well maintained and the space has been used well. The nursery manager told us that this area is checked daily to ensure that there are no hazards before children are allowed to go outside. The garden area looked clean, tidy and hazard free during the inspection. This ensured that children could access the garden area safely. The service had very good risk assessments for the general environment within the nursery and for activities and outings. These were regularly reviewed ensuring that risk within the nursery was dynamic and monitored for the safety of the children. All staff were trained in first aid meaning that staff could attend to children quickly should they require first aid treatment. Areas for improvement Inspection report continued The provider should update the confidentially policy to ensure that it accurately states who can access information without parental permission and the purposes for this. An example of this would be the information that the Care Inspectorate can access for inspection purposes. During the inspection one area of the nursery was left under their registered staff to child ratios due to a miscalculation. This was quickly rectified on being pointed out to the supervisor. Grade awarded for this statement: 5 - Very Good Number of recommendations: 0 Number of requirements: 0 Statement 3 We have a professional, trained and motivated workforce which operates to National Care Standards, legislation and best practice. Service strengths Overall we found the service had a good professional, trained and motivated workforce. We saw that the nursery have daily planning meetings which provides staff with opportunities to discuss the planning for the day and any issues that arise for individual children. Staff contributed to the evaluation and improvement plan for the nursery and they told us that they had been asked for their ideas about how the nursery can develop over the coming year. This ensured that staff had ways of contributing to the planning and development of the nursery. The nursery has a very low staff turnover and good core group of established staff to promote continuity for children and their families. Staff told us that they felt valued and that they were listened to. The staff team were enthusiastic and interacted with the children well. They used positive language and children responded well to this. We observed the management team leading by example in this area and interacting Nursery Rhymes, page 19 of 26

20 positively with the staff team. This ensured that staff were working within a positive environment. We saw that staff had the minimum qualifications to be registered with the Scottish Social Services Council, we sampled the register and those who we checked were registered. This ensured that staff were being suitably employed. We saw that staff are given opportunities to expand their development, an example of this would be that the whole staff team had recently undergone both their first aid and food hygiene courses. Staff were able to access their own CPD on-line thought Dundee City Council and this supported them to continue to progress their training and development. Overall we found staff to be friendly and approachable with parents and children. Staff were observed to be enthusiastic and fun, kind and caring and this appeared to be a good balance as the majority of the children were observed to be happy throughout the day. Areas for improvement The nursery kept very good notes of the planning that was being undertaken on a daily basis within the setting. We discussed with the manager the importance of ensuring that staff meetings are recorded clearly. We spoke about having an agenda that staff can contribute to and that this should be rolling with previous tasks identified being checked up on. Training for both of the Child Protection Officers was out of date and should be updated. We made a recommendation in relation this. Grade awarded for this statement: 5 - Very Good Number of requirements: 0 Number of recommendations: 0 Statement 4 We use quality assurance systems and processes which involve service users, carers, staff and stakeholders to assess the quality of service we provide Service strengths Overall we found that the service is led by a strong management team of the owner and the manager. During the inspection we found them to be calm, confident and clear. We found them both to be approachable and friendly and this contributed to the overall ethos within the nursery. We saw that evaluations of parents, children's and staff questionnaires are regularly undertaken and fedback in a variety of ways. We saw that the nursery was very good at taking feedback on board and developing meaningful responses to this. As a Dundee City Council partnership nursery the manager attended cluster group meetings with other nurseries across the area. This offered opportunities to discuss practice across the city and also any wider considerations. We saw that the nursery were working towards small identified goals that had been outlined within their improvement plan, the staff team were regularly reviewing this. Nursery Rhymes, page 21 of 26

22 The nursery had developed lists for the management as a way of regularly assuring that the processes in the nursery were being regularly reviewed and monitored. Areas for improvement We discussed with the manager that a quality assurance calendar is a good way of keeping track of all of the quality assurance systems within the nursery. This provides a timeline and ensures that processes are not overlooked. We spoke with the nursery about ways in which they could actively seek and use feedback from their stakeholders. The service should ensure that all policies are dated and signed when they are reviewed. The manager should develop audits of accident and incident recordings in order to identify any recurring issues in these areas. Enforcements We have taken no enforcement action against this care service since the last inspection. Additional Information Action Plan Failure to submit an appropriate action plan within the required timescale, including any agreed extension, where requirements and recommendations have been made, will result in SCSWIS re-grading the Quality Statement within the Management and Leadership Theme as unsatisfactory (1).
