3 Summary This report and grades represent our assessment of the quality of the areas of performance which were examined during this inspection. Grades for this care service may change after this inspection following other regulatory activity. For example, if we have to take enforcement action to make the service improve, or if we investigate and agree with a complaint someone makes about the service. We gave the service these grades Quality of care and support 5 Quality of environment Quality of staffing 5 Quality of management and leadership 4 Very Good N/A Very Good Good What the service does well We saw that this day centre was a welcoming and friendly environment where service users told us they felt comfortable and well supported by the staff team. The considerate and respectful manner of staff in their support of service users was evident in their practice we saw during our visit and which was also confirmed by service users and relatives/carers. Involvement of service users in the management of the service was well established and regular forums gave people the opportunity to contribute to service development. What the service could do better The implementation of quality assurance systems and audits should be progressed to assist in the monitoring and improvement of aspects of the service. This would also show evidence of ongoing service development. page 3 of 29

4 What the service has done since the last inspection Inspection report Care plans have been further developed and the review system was planned to take account of any long absences from attendance as a result of extended holidays. The requirements we made at the last inspection had been fully implemented. The recommendation we made about staff training in adult protection had been implemented. We have noted progress in these under the relevant theme and statement. Conclusion We saw a very respectful approach to the support provided for service users in Milan and which also reflected the other principles of the national care standards, dignity, privacy, choice, safety realising potential and equality and diversity. Milan was described by many service users as an essential service for them to meet friends from shared ethnic background, language, culture and faith. During our inspection we saw that the service provided including staff practice confirmed the values of the organisation which were promoted and respected. page 4 of 29

5 1 About the service we inspected Inspection report The report was written following an unannounced inspection which took place in the service over two days during opening hours and time spent at the office base. During this inspection we gathered evidence from various sources, including the relevant sections of policies, procedures, records and other documentation including evidence from: - The Certificate of Registration. - The insurance certificate. - The service's action plan to address issues from the last inspection. - A sample of service users' support plan files. - Accident, incident and complaint records. - Observation of support staff practice with service users and with each other. - Examination of staff training records. - Examination of staff recruitment files. - Content of 22 (of 45 sent) service pre inspection questionnaires completed by service users and or their relatives/carers. - Consideration comments in the pre inspection questionnaires and during the inspection. - Consideration of the National Care Standards, Support Services. Discussion with various people including: - Service users attending the day centre. - Relatives/carers. - The manager. - Support staff and volunteers on duty. - The chair person and members of the management committee. - Members of admin staff. The above information was taken into account during the inspection process. page 5 of 29

6 Feedback on the outcomes of the inspection was shared with the manager, a member of support staff and the chair person. Recommendations A recommendation is a statement that sets out actions that a care service provider should take to improve or develop the quality of the service, but where failure to do so would not directly result in enforcement. Recommendations are based on the National Care Standards, SSSC codes of practice and recognised good practice. These must also be outcomes-based and if the provider meets the recommendation this would improve outcomes for people receiving the service. Requirements A requirement is a statement which sets out what a care service must do to improve outcomes for people who use services and must be linked to a breach in the Public Services Reform (Scotland) Act 2010 (the "Act"), its regulations, or orders made under the Act, or a condition of registration. Requirements are enforceable in law. We make requirements where (a) there is evidence of poor outcomes for people using the service or (b) there is the potential for poor outcomes which would affect people's health, safety or welfare. 8 2 How we inspected this service The level of inspection we carried out In this service we carried out a low intensity inspection. We carry out these inspections when we are satisfied that services are working hard to provide consistently high standards of care. What we did during the inspection Milan Day Care Support Service provides support to older people from Indian, Pakistani, Bangladeshi and Mauritian Communities in Edinburgh and Lothian. Support is tailored to meet individual social, cultural, recreational and language needs. The service has been registered with the Care Inspectorate since January The day centre is situated in Portlee Resource Centre and shared with a day care and support service provided by Edinburgh City Council. General facilities in the resource centre include a kitchen, hairdressing salon, treatment/therapy room, art room, activities rooms, a large dining room and a garden area. Milan have separate seating areas and share the dining room and some of the other facilities. Office accommodation is also provided outwith the day centre a short walk away. The service operates from Tuesday to Thursday between the hours of 11am and 2.00pm. The service can accommodate up to 25 people per day. A programme of activities is provided each week for service users and the involvement of people in the planning and implementation of activities and outings is supported and encouraged. Educational activities and recreational activities are provided which includes exercise, massage, healthy cooking, IT skills and informal talks on issues concerning older people. The support service is viewed as a tool to help integrate people into wider society and reduce vulnerability and isolation. The values of the service are to: page 8 of 29

9 -Respect service user's dignity and support inclusion, irrespective of social, cultural or religious beliefs and faiths. -Respect privacy and confidentiality. Encourage and enable service users to have control of their own lives. -Enable choice, dignity, quality of life and independence. Service users attend on specified days agreed in advance at the time of accepting a referral. A minibus, handy cabs and taxis are used to provide transport but attendance may be limited due to space available in the minibus and or distance. There is a named Manager for this service and a senior worker who manages the day to day service provision and who reports to the Manager. Grading the service against quality themes and statements We inspect and grade elements of care that we call 'quality themes'. For example, one of the quality themes we might look at is 'Quality of care and support'. Under each quality theme are 'quality statements' which describe what a service should be doing well for that theme. We grade how the service performs against the quality themes and statements. Details of what we found are in Section 3: The inspection Inspection Focus Areas (IFAs) In any year we may decide on specific aspects of care to focus on during our inspections. These are extra checks we make on top of all the normal ones we make during inspection. We do this to gather information about the quality of these aspects of care on a national basis. Where we have examined an inspection focus area we will clearly identify it under the relevant quality statement. Fire safety issues We do not regulate fire safety. Local fire and rescue services are responsible for checking services. However, where significant fire safety issues become apparent, we will alert the relevant fire and rescue services so they may page 9 of 29

11 The annual return Every year all care services must complete an 'annual return' form to make sure the information we hold is up to date. We also use annual returns to decide how we will inspect the service. Annual Return Received: Yes - Electronic Comments on Self Assessment Every year all care services must complete a 'self assessment' form telling us how their service is performing. We check to make sure this assessment is accurate. A self assessment was submitted for this service in July Due to the change of manager the self assessment was not updated prior to this inspection. However, the content of the self assessment remained relevant. The self assessment included information under each statement to describe the service's strengths, areas they thought they did well, some areas for development and any changes they planned. The grades awarded through the self assessment were lower that what we assessed through inspection for the quality themes and statements we looked at. Taking the views of people using the care service into account We met with and spoke with service users on both days of the inspection when different service users attended. We spoke with approximately 16 service users and all were highly complimentary about the service they received. Some described the service as essential for them to get out of their home and meet and speak with people of a shared culture. Service users and relatives/carers spoke highly of the work of the staff team, their efforts to provide activities and outings which they enjoyed and the meal arrangements. The ability to maintain friendships and the companionship the service offered was valued highly by service users we spoke with. page 11 of 29

12 Some of the comments made by service users during the inspection and in pre inspection questionnaires were as follows: "It is a very friendly place, good company and I enjoy coming here." "We have been coming here for some time, it is an excellent service and we enjoy the company and meeting friends." "I am proud of this service, I have no issues or concerns. People come here to have a good time, to share problems and help to resolve them, We talk about our health and we try to look after each other. We have good staff they are very professional." "I make any appointments around my attendance here, it is important for me to come and meet friends and have some company. I enjoy coming here very much, we have good staff." "Things are better now that there has been a change of management, staff are nice, kind and respectful. I enjoy the activities, especially the yoga, chair exercises and flower arranging." "I would be sad if I could not come here. I enjoy the company very much, I enjoy helping others, and things are better now there has been a change of management. Milan is an important part of my life." "Milan is a part of my life and I can't leave Milan and Milan can't leave me." "I enjoy coming to Milan as the service gives me motivation to get out in the morning. Staff are extremely skilled in looking after the individual needs. If I have any reservations I get the opportunity to talk to the manager and all my suggestions are followed immediately. I am more than satisfied with the group." "I am very happy with this service Milan provides. I have been coming here a long time, I look forward to seeing my friends." "I am happy with everything. The main thing is I can get out of the house and page 12 of 29

13 meet my friends." "I feel I get a good service here." "I am happy with Milan services. Since I stared coming to Milan my health has improved." Overall service users we met and who completed pre inspection questionnaires were very satisfied with the quality of service they received. Taking carers' views into account Some of the comments made by relatives/carers during the inspection and in pre inspection questionnaires were as follows: "My relative has been coming to Milan for some time, she really likes to meet other people from our community and friends. She would be very isolated at home as she lives on her own but enjoys attending." "My relative attends and the company of others helps keep them interested in things." page 13 of 29

14 3 The inspection We looked at how the service performs against the following quality themes and statements. Here are the details of what we found. Quality Theme 1: Quality of Care and Support Grade awarded for this theme: 5 - Very Good Statement 1 We ensure that service users and carers participate in assessing and improving the quality of the care and support provided by the service. Service Strengths We did not look at all aspects of this quality statement but looked at progress of the recommendation made at the last inspection to develop a participation strategy. A service users participation strategy had been developed and included ways in which service users could contribute to giving their views to improve and develop the overall service provided. This recommendation had been implemented. We also saw that the strengths identified at the last inspection were still evident and included regular contact with the manager, staff and members of the management board, service users' forums, a suggestion box, complaints policy and procedures and links with advocacy and information advice services. Areas for improvement The service should continue to develop service user participation to support and encourage people to contribute to assessing and improving the service they receive. page 14 of 29

15 Grade 5 - Very Good Number of requirements - 0 Number of recommendations - 0 Statement 3 We ensure that service users' health and wellbeing needs are met. Service Strengths The service demonstrated the following strengths when ensuring residents health and well being. The outcomes of 22 pre inspection questionnaires returned from service users or relatives/carers on their behalf : - Eighteen agreed that they had a care and support plan which contained information about their support needs, one disagreed and three did not know about this. - Twenty -one agreed or strongly agreed that their needs and preferences had been detailed in the care and support plan, one did not know about this. - Twenty two agreed or strongly agreed that the service checked with them regularly that they were meeting their needs. - Twenty two agreed or strongly agreed that overall they were happy with the quality of care and support provided. We looked at a sample of care and support plans and saw that although these followed the same format the information was person centred and noted the specific needs, preferences and choices of the individual. Risk assessments were included in the support plans. We saw that support plans including the risk assessments were reviewed and updated where necessary at least once in a six month period. However, some service users may take extended holidays over the winter months and care support plans were reviewed with them on return to ensure the information was still relevant. Service user had signed the documents to confirm agreement page 15 of 29

16 with the content and the discussions and decisions as a result of reviews. We saw that care /support plans included any relevant contact details for relatives/carers and where necessary health care professionals for example, the GP. Any specific health care needs were included in the care and support plan and any actions required of staff to support the service user whilst attending Milan. Service users told us that they were well looked after and felt confident that staff would assist with any health related concerns. We saw records which confirmed that staff had acted appropriately in contacting emergency service and relatives/carers in the event of service users' ill health. Drinks, snacks and lunch were provided for service users who were complimentary about these arrangements. We saw that meals provided were in accordance with service users' cultural preferences. The overall impression we gained about the provision of this service was the way in which service users were treated with dignity and respect. This was apparent in interactions and of how staff spoke with and about service users. Service users and relatives/carers confirmed that this was also their experience and staff were also praised for their kind and considerate approach to the support provided. We also saw that appropriate humour and laughter was part of these interactions and service users looked comfortable and at ease with members of staff. The National Care Standards - support services Standard 4- Support arrangements and Standard 10 - Feeling safe and secure, Standard 15 - Eating well and Standard 16 - Keeping well were met at this inspection. Areas for improvement The service should continue to develop the care/support planning and review system to ensure that service users' health care needs and any changes to these can be met by the service provided by Milan. page 16 of 29

17 Grade 5 - Very Good Number of requirements - 0 Number of recommendations - 0 Statement 6 People who use, or would like to use the service, and those who are ceasing the service, are fully informed as to what the service provides. Service Strengths Milan provides written information for prospective service users and their relatives/carers about the service. The welcome pack outlines the service provision including information about: - Who we are. - The service. - A typical day at Milan. - Equal opportunities. - Staffing. - Care plans. - Service user participation. - Transport. - Complaints (a copy of this can be provided on request and can be translated into the individuals community language. This included the Care Inspectorate contact details). Staff were also available to visit and speak with prospective service users and their families about what Milan can provide. Other written information was also available to assist service users to access information and advice and befriending and outreach services. Health information leaflets, the last inspection report and complaint procedure was also on display in the service for reference. An initial assessment was undertaken with the prospective service user and or page 17 of 29

18 their relatives/carers to assess individual needs and to inform the care and support the individual will need whilst attending Milan. A trial visit can also be offered prior to any decision about admission and attendance. Regular reviews, service user forum meetings and focus group meetings also assisted to keep service users informed about any changes, developments and events planned. Service users told us they were asked their opinions and felt that they were listened to and their views considered. We concluded that having service users on the management board could also help to make informed decisions about the service and how these decisions would affect service users. The National Care Standards - support services Standard 1 was met at this inspection. Areas for improvement The service should review and update information to prospective service users as the services develops. 20 Quality Theme 3: Quality of Staffing Grade awarded for this theme: 5 - Very Good Statement 2 We are confident that our staff have been recruited, and inducted, in a safe and robust manner to protect service users and staff. Service Strengths During our inspection at Milan day support service we looked at a selection of staff recruitment files for staff who had been employed in the service since our last inspection. The files were kept securely in the Milan offices outwith the day support service. We found that systems and practices in place followed best practice guidance from The Scottish Executive "Safer recruitment through better recruitment" and Milan recruitment policy and procedure. In the recruitment files we saw: - A completed application. - A person specification for the role applied for. - Request and receipt of references, including one from the previous employer. - PVG check (Protecting Vulnerable Groups). - The check of SSSC (Scottish Social Services Council) where necessary. The application form included: - Skills, knowledge, qualifications and experience relating to the job for which the applicant has applied. - Two references to support the application. - A health check self declaration. - An Asylum and Immigration declaration. page 20 of 29

21 - A Rehabilitation of Offenders declaration. Shortlisting guidance was in place for staff and an assessment form was used to inform selection for interview. Service users and a representative from Edinburgh City Council were part of the interview panel and the interview process included the candidates' ability to demonstrate their skills, knowledge and abilities against the person specification. Staff were not able to be employed until all aspects of the recruitment selection process had been completed. These processes assisted the service to satisfy themselves of the applicants' suitability before the offer of employment and an introduction to service users. Induction training was in place for all new staff. Thereafter supervision and appraisal systems and refresher training were in place to monitor and improve staff practice. We concluded that the recruitment and induction practices showed that the service provider had robust procedures and practices which were very well managed systems. This assisted them to make sure appropriate staff were employed and contributed to safe guarding service users and their relatives/ carers. Areas for improvement The service should continue to use robust staff recruitment and induction practices to ensure appropriate staff are employed and which assists the service to safe guard service users and their relatives/carers. 22 We have a professional, trained and motivated workforce which operates to National Care Standards, legislation and best practice. Service Strengths In the 22 pre inspection questionnaires service users or their relative/carers told us: - Twenty one strongly agreed or agreed that they were confident that staff had the skills to support them, one did not know about this. - Twenty one strongly agreed or agreed that staff had enough time to carry out the agreed support and care, one said this was not applicable. - Twenty two strongly agreed or agreed that staff treated them with respect. - Twenty two strongly agreed or agreed that they knew the names of staff who provide care and support. Staff we spoke with could tell us about their responsibilities in their work in this service and of the training and supervision arrangements. We saw that core induction training was provided for all new staff and thereafter refresher training gave staff the opportunity to update their practice. This training included adult support and protection, manual handling, patient assisted transport training (for staff assisting service users with transport) and food hygiene. Other training provided assisted staff to meet the assessed needs of service users for example dementia awareness. Staff told us that they were able to access training relevant to their work and to assist in obtaining an SVQ qualification. Supervision was in place but a more structured and formal supervision and appraisal system was in the process of being developed. Staff were also able to discuss their practice, any training needs and to exchange information about page 22 of 29

23 service users at daily meetings and staff meetings with the manager. We saw that staff worked well as a team and were supportive of each other to provide a high standard of care to service users. The National Care Standards - support services Standard 2 Management and staffing arrangements were met at this inspection. Areas for improvement The service should continue to develop staff training opportunities to ensure that staff remain up to date with legislation and best practice guidance. 24 Quality Theme 4: Quality of Management and Leadership Grade awarded for this theme: 4 - Good Statement 4 We use quality assurance systems and processes which involve service users, carers, staff and stakeholders to assess the quality of service we provide Service Strengths The service demonstrated the following strengths in the areas covered by this Quality Statement. We did not look at all aspects of this quality statement but looked at progress of the recommendation made at the last inspection to develop and implement auditing systems. We saw that the strengths identified at the last inspection were still evident in that the ethos of the organisation was to regularly consult with service users continued and quality assurance surveys took place twice a year. We also saw that service user forum meetings and day to day contact with the manager and management committee gave service users the opportunity to discuss any suggestions and views about the service. The manager was aware of her responsibility to inform the Care Inspectorate and Scottish Social Service Council of stipulated events. Appropriate notifications were made promptly to the Care Inspectorate. All staff we met impressed as being committed to developing the service and to provide a high standard of support to service users. page 24 of 29

25 Areas for improvement We concluded that the quality assurance systems need to be fully developed, implemented and improved. This should include how the outcomes and actions are shared with residents, relatives'/carers and staff. This would assist to show that audits resulted in sustained service improvements and the ongoing development of the service. An effective quality assurance system may also assist to evidence the overall high standard of service we saw during the inspection. We have made an amended recommendation about quality assurance systems. (See recommendation 1). Grade 4 - Good Number of requirements - 0 Recommendations Number of recommendations The manager should develop the quality assurance systems to evidence that outcomes of audits and consultations are effective in identifying any deficits and improvements which could be made to the overall service provided. These should include but not be limited to: - Care plans and reviews and associated records. - Staff training, practice and competency. - Safety of the environment. - Meals and dining experience. - Accidents, incidents and complaints. 26 4 What the service has done to meet any requirements we made at our last inspection Previous requirements 1. The Provider must make proper provision for the health, welfare and safety of serviced users. In order to do so, the Provider must ensure that each service user has a care plan. Care plans should reflect the aims, wishes, needs of the individual service user and clearly demonstrate how those needs are to be met. This requirement was made on 19 February 2013 We saw that care plans had been developed for service users and systems were in place for these to be reviewed and updated at regular intervals. Met - Within Timescales 2. The Provider must ensure the health, welfare and safety of service users. In order to do so, the Provider must ensure that there are policies and procedures in place to guide and direct staff in the delivery of personal care. The provider must supply to the Care Inspectorate with up to date information on the progress that has been made on obtaining Disclosure Checks for staff working within the service. The Provider must ensure the health, welfare and safety of service users by ensuring that persons employed in the provision of the care service receive training appropriate to the work they are to perform. In order to this the provider must carry out an analysis of staff training needs and instigate a training plan as a result of the needs analysis. This requirement was made on 19 February 2013 We saw that systems were in place to monitor and record staff training. We saw that mandatory staff training had been undertaken by all staff and any time limited training was within date.
