1 Sr Workforce Health Specialist / Population Health / Full-time / Santa Rosa Memorial - ( ) The people of St. Joseph Health have worked for 53 years to improve health and quality of life in California's North Bay region, starting in Sonoma County, where the Sisters of St. Joseph of Orange opened the doors of Santa Rosa Memorial Hospital in Today, we continue the mission begun by the Sisters and continued through the St. Joseph Health Ministry of extending the healing ministry of Jesus to those we serve through an integrated spectrum of primary, urgent, acute, outpatient, palliative care and regional referral services. Sonoma County entities aligned with St. Joseph Health include the 278-bed Santa Rosa Memorial Hospital, the region's only Level II trauma center, as well as the 80-bed Petaluma Valley Hospital. Our services also encompass three Urgent Care centers, Hospice of Petaluma, Memorial Hospice and North County Hospice, the Annadel Medical Group, as well as the St. Joseph Home Care Network. We act as a regional referral hub for outlying hospitals, while also providing outpatient behavioral health care, education to promote health and prevent chronic disease, rehabilitation, oral health care, community benefit programs, and more, all fostering health and quality of life throughout the

Job Summary: Under the direction of the Department Supervisor or Manager of Employer and Community Relations, the Sr. Workforce Health Specialist develops relationships with community employers and promotes St Joseph Health (SJH) service lines, the Workforce Health programs, and insurance products in and around Sonoma County (Santa Rosa Memorial Hospital). While coordinating the Workforce Health Program, the Specialist also identifies the needs of community employers to engage them towards SJH services and directs them to Heritage Foundation physicians and services for their employees healthcare needs including preventive health, acute and ancillary care,. Additional duties may include all aspects of event planning, support and execution. All duties performed under minimum supervision. Essential Functions: This position requires approximately 25% of the time spent in the office and u of the time to be spent in the field. Network and build relations with community employers using specific business strategies identified by the Strategic Service team and executive level

3 leadership. Obtain commitment to and/or support of company program(s) and activities (health fairs, health screenings, events, health education). Recognize/identify health risk needs with community employers and promote SJH- Sonoma County including Heritage Foundation physicians to meet those needs. Meet regularly with key team members and department leadership in SJH-Sonoma County, to develop multidisciplinary relationships that will evaluate the success of the program, learn more about services and develop new ideas and programs proactively as needed. Perform organized project management duties and plan daily, weekly and monthly activities to reach established objectives. Keep and complete other administrative work in a timely basis. Communicate progress of employer program with Strategic Service team and SJH-Sonoma County leadership through regular reports. Coordinates, trains and recruits presenters, volunteers and event staff building strong

4 relationships. Business Development: Make cold calls to prospective employers, brokers and community groups representing the Workforce Health Program and/or insurance products. Represents SJH in community-based meetings/functions as a public and/or keynote speaker. Maintain confidentiality of Santa Rosa Memorial Hospital and SJH Sonoma County employer, and employee information within HIPAA standards. Ensure proper handling and distribution of company funds and agreements. Event planning, managing, coordination and execution including wellness events, programs, health fairs, campaigns, workshops, interactive booths, and biometric health screenings. Developing and presenting health education programs. Creates and provides marketing material for events and outreach. Initiate, coordinate (including logistics), meet with, train and lead teams of event staff and volunteers on each project. Ensuring the procurement of on-site volunteers, contractors,

5 etc. to include technical/administrative help with blood pressure, cholesterol, biometric health, general health and fitness screenings all meet Santa Rosa Memorial Hospital standards at each activity and/or event as needed. Minimum Position Qualifications: Associates degree in Business, Marketing, Human Resources, Health or a related field Two (2) years of experience in any of the following combinations professional sales/business development, in healthcare or related field, event coordination, community outreach, health education, health fairs, marketing, and/or community relations. Workforce Wellness or B2B sales preferred Must Current California drivers license. Preferred Position Qualifications: Bachelor's degree preferred in Business, Marketing, Human Resources, Health or a related field. Four (4) years experience in professional sales in healthcare, healthcare

6 admin or related field. Ideal candidate would have Bilingual (English/Spanish) skills, although not required for position. Experience in nursing, EMT, physiology, wellness, fitness or health coaching; or health related field. Wellness Certification with experience developing health education programs. Experience in event planning, community organizing or campaigns. Workforce Wellness or B2B sales experience.
