1 Indiana School Nurse Effectiveness Rubric This rubric combines many of the professional requirements of a professional School Nurse with those of the RISE Model. It is a working revision of the School Nurse Evaluation Rubric available on the Learning Connection website within the IDOE-Developing New Indiana Evaluations community. It was designed to provide another option as schools develop or refine effectiveness rubrics and evaluation materials for Indiana Student Services Personnel. For questions or comments, contact Fleck Education Services at or DOMAIN 1: PURPOSEFUL PLANNING School nurse uses student data to plan and set goals for the school s Health Services Program. Competencies Highly Effective (4) Effective (3) Improvement Necessary (2) Ineffective (1) 1.1 Utilize Data to Plan - Involves and collaborates with parents and the community as well as health care providers, school safety specialist and staff - in developing individualized educational and health care plans for the classroom Nurse uses data to: - Consistently compare and contrast clinical findings with normal and abnormal variations, child development and cultural diversity to develop an effective Health Services Program plan of action/care - Develop plan with input from administration, health care providers, school safety specialist and staff Nurse rarely uses data to develop a Health Services Program plan of action/care. Nurse has not developed a Health Services Program plan of action/care. 1.2 Set Ambitious and Measurable Goals -Set goals in collaboration with administration, school safety specialist, staff and community members. Nurse develops annual goals that: - Are measurable and include benchmarks to help monitor progress and inform interventions throughout the year - Create a healing environment through alignment of health and education needs of, vision and mission of the school, and the school improvement plan Nurse develops annual goals that: - Are measurable The goals may not: - Include benchmarks to help monitor progress and inform interventions throughout the year OR incorporate the vision and mission of the school and the school improvement plan Nurse rarely or never develops goals OR goals are developed, but are extremely general and not helpful for planning purposes

2 1.3 Develop or Maintain School Health Services Program At Level 4, a teacher fulfills the criteria Nurse develops or maintains a School Health Services Program that: - Effectively applies current knowledge of the epidemiology of infectious and communicable diseases - Includes an emergency care plan and first aid measures and an immunization program -Incorporates emergency care and first aid measures in the school safety plan and communicates plan to all school personnel -Involves nurse as an active member of the crisis team Nurse develops or maintains a School Health Services Program that: - Effectively applies current knowledge of the epidemiology of infectious and communicable diseases -Include emergency care and first aid measures in the school safety plan -Communicate the school safety plan to all school personnel -Be active on the crisis team Nurse rarely or never plans a school health services program 1.4 Adhere to Professional Practice for Delivery and Planning of Services At Level 4, a teacher fulfills the criteria -Nurse is continually seeking ways to improve nursing care and locates and shares information and resources both within and outside the school for the safe, effective and specialized nursing care of. Nurse adheres to professional practice by: - Consistently and efficiently uses current knowledge of the pathophysiology, signs, symptoms, and treatment of acute and chronic conditions in practice -Maintains well-organized health office -Ensures medications are properly stored, organized and easily found for student administration -Maintains daily schedule of medications and treatments -Respects confidentiality of student/staff health information and records -Consistently follows universal precautions when delivering health services. Nurse adheres to professional practice by: - Using current knowledge of the pathophysiology, signs, symptoms, and treatment of acute and chronic conditions in practice - Maintain a well-organized health office OR -Ensure medications are properly stored, organized and easily found for student administration - Maintains daily schedule of medications and treatments Nurse rarely or never adheres to professional practice or maintains a well-organized health office 1.5 Track Student Data and Assess Program -Assesses impact of school health services program on removing barriers to student achievement Nurse uses data for: -Conducting a formal assessment of the school health services program Nurse occasionally uses data to make an informal assessment of the school health services program Nurse rarely or never uses data to assess progress

3 DOMAIN 2: EFFECTIVE HEALTH CARE Nurse uses assessment to deliver effective health care services to and information to staff and the community. 4 Competency 2.4: Engage Students and Staff in Health Care Nurse is highly effective at engaging and staff in health care Nurse is effective at engaging and staff in health care - Annual training for school staff on health care issues is planned, implemented, supervised and recorded Nurse needs improvement in engaging and staff in health care - Occasional training is planned and implemented for school staff on health care issues Nurse is ineffective at engaging and staff in health care - No training is planned and implemented for school staff on health care issues Competency 2.5: Keep Accurate and Efficient Records of Student Care - Data shows overall improvement in wellness among school staff and - Parents, staff and participate in creation of health care plans Nurse is highly effective at keeping accurate, relevant and efficient health care records - Health records data is used for program management, administrative reporting and school health program evaluation - Individual interactions and group presentations by nurse effectively engage and staff in assuming an active role in promoting wellness, a healthy lifestyle and preventing disease - Information for substitute nurses is available and well organized -Students self-manage for timely appointments, independently take medications and follow designated safe routines and procedures Nurse is effective at keeping accurate, relevant and efficient health care records -Health recordkeeping is systematic, confidential and efficient -Medication administration information is recorded in appropriate and well organized format and signed release and parent permission forms are on file -Immunization records, medication and treatment documentations are timely, complete and aligned with state and district requirements - Individual interactions and group presentations by nurse could more effectively engage and staff in assuming an active role in promoting wellness, a healthy lifestyle and preventing disease - Little information is available for substitute nurse -Students are somewhat dependent on nurse for making appointments, taking medications and following designated safe routines and procedures Nurse needs improvement at keeping accurate, relevant and efficient health care records -Health recordkeeping is somewhat systematic and confidential but could be more efficient -Medication administration information is sometimes recorded and signed release and parent permission forms are on file -Immunization records, medication and treatment documentations are mostly timely, complete and aligned with state and district requirements - Individual interactions and group presentations by nurse do not engage and staff in assuming an active role in promoting wellness, a healthy lifestyle and preventing disease - No information is available for substitute nurse -Students depend on nurse for making appointments, taking medications and following designated safe routines and procedures Nurse is ineffective at keeping accurate, relevant and efficient health care records -Health recordkeeping is not efficient or systematic or confidential and data is rarely if ever used for program management, administrative reporting and school health program evaluation -Medication administration information is poorly recorded or not well organized and/or signed release and parent permission forms are missing -Immunization records, medication and treatment documentations are incomplete

5 DOMAIN 3: SCHOOL NURSE LEADERSHIP School Nurse develops and sustains the intense energy and leadership within their school community to ensure the achievement of all. Competencies Highly Effective (4) Effective (3) Improvement Necessary (2) Ineffective (1) 3.1 Contribute to School Culture - Seek out leadership roles - Go above and beyond in dedicating time for and peers outside of class - Contribute ideas and expertise to further the schools' mission and initiatives - Dedicate time efficiently, when needed, to helping and peers outside of class - Serve on the coordinated school health advisory council - Contribute occasional ideas and expertise to further the school's mission and initiatives - Frequently dedicate time to help and peers efficiently outside of class - Serve on the coordinated school health advisory council Nurse rarely or never contributes ideas aimed at improving school efforts. Nurse dedicates little or no time outside of class towards helping and peers. School does not have a health advisory council. 3.2 Collaborate with Peers and Other Professionals - Go above and beyond in seeking out opportunities to collaborate - Coach peers through difficult situations - Take on leadership roles within collaborative groups -Contact various agencies to facilitate referral and utilization of resources for health care, housing and financial issues -Collaborate with health care providers, health departments, other related agencies and school staff in developing individualized educational and health care plans for the classroom - Seek out and participate in regular opportunities to work with and learn from others - Ask for assistance, when needed, and provide assistance to others in need -Make occasional contacts to various agencies to facilitate referral and utilization of resources for health care, housing and financial issues - Collaborate minimally with health care providers, health departments, other related agencies and school staff in developing individualized educational and health care plans for the classroom -Participate in occasional opportunities to work with and learn from others - Ask for assistance when needed Nurse rarely or never participates in opportunities to work with others. Nurse does not make contacts to agencies to facilitate referral and utilization of resources for health care, housing and financial issues and does not collaborate with health care providers, health departments, other related agencies or school staff in developing individualized educational and health care plans for the classroom - Seek to provide teachers with assistance when needed OR - Regularly seek out opportunities to work with others

6 3.3 Seek Professional Skills and Knowledge - Regularly share newly learned knowledge and practices with others - Seek out opportunities to lead professional development sessions or volunteer for committee work at local, state/national meetings - Contribute to the development of other nurses by mentoring - Understand the health priorities of the school - Actively pursue opportunities to improve knowledge and practice - Seek out ways to implement new practice, where applicable - Welcome constructive feedback to improve practices - Attend all mandatory professional development opportunities - Actively pursue optional professional development opportunities - Seek out ways to implement new practices into instruction - Accept constructive feedback well -Understand the health priorities of the school Nurse rarely or never attends professional development opportunities. Nurse shows little or no interest in new ideas, programs, or classes to improve professional practice 3.4 Advocate for Student Success - Make changes and take risks to ensure student success - Display commitment to the health and well-being of all - Attempt to remedy obstacles that restrict student achievement - Advocate for ' individualized needs - Display commitment to the health and well-being of all - Advocate for individual ' needs Nurse rarely or never displays commitment to the health and well-being of. Nurse accepts failure as par for the course and does not advocate for needs. 3.5 Engage Families in Student Learning - Is available to address concerns in a timely and positive manner, when necessary, outside of required outreach events - Is proactive in providing complete information regarding the school health program - Utilizes a variety of communication techniques that includes a home visit with individual families regarding their child s health care needs. - Is proactive and assumes leadership in handling routine, unexpected and emergency situations with parents - Treats parents with respect and empathy - Proactively reach out to parents to communicate and maintain the health and well-being of - Respond promptly to contact from parents - Engage in all forms of parent outreach required by the school - Initiate collaboration with parents, health care providers and staff in developing individualized educational and health care plans for the classroom - Treats parents with respect and empathy - Respond to contact from parents - Engage in all forms of parent outreach required by the school - Proactively reach out to parents to provide information on the school s health program Nurse rarely or never reaches out to parents and/or frequently does not respond to contacts from parents.

7 Core Professionalism Rubric These indicators illustrate the minimum competencies expected in any profession. These are separate from the other sections in the rubric because they have little to do with teaching and learning and more to do with basic employment practice. Counselors are expected to meet these standards. If they do not, it will affect their overall rating negatively.
