RN, Disease Manager (Job# ) FT position with benefits, Disease Management, Palm Springs, Ca. The RN, Disease Manager provides nursing services and care coordination to high risk and/or high cost patients with chronic disease. Services are provided telephonically and on-site in the clinic. The Disease Manager provides care coordination, education, counseling and on-demand information to patients who have been enrolled in a condition specific program. This role allows highly skilled Registered Nurses to reach individuals with chronic diseases and influence their healthcare outcomes. The Disease Manager is responsible for coordinating certain operations of the Heart Failure/CAD/COPD Programs, including collaboration of services with the patient, physicians, nursing staff, case managers, social workers, home health infusion services, cardiac rehabilitation and the emergency department to facilitate management of the patient. In addition the position focuses on CAD patients, post Coronary Artery Disease Bypass patients, patients with Stents and hypertension and patients requiring medical management for CAD who are not surgical candidates. Strong Clinical background in Cardiac patient s previous cath lab and/or Cardiac ICU experience desired. Knowledge of Disease Management and Case Management. The Clinical Pharmacist provides clinical, supervisory and educational functions as necessary to support the overall patient care provided by Medication Management Services. Pharmacists provide medication therapy management for patients as defined under medical staff approved protocols. Nurse Practitioner/Physician Assistant (Job# & ) Full Time position, Seniors Only Evaluation Clinic, primary location is Orange County. Training will be conducted in Palm Springs, Ca. The ideal candidate is a professional who enjoys working with seniors and has a good understanding of managed care systems, with the ability to work in a multidisciplinary team environment. The position entails assessing seniors (H&P), during an annual evaluation, for chronic condition and seeing to preventative health care needs (Star Measures); following the review of labs, available medical records and screening/testing results that have been accomplished (by support staff) for such matters as dementia, depression, fall risk, safety, PVD and advanced directives. This provider will be responsible for discussion of all with the member as well as documentation/coding and implementation of a plan of care. Requirements: State of California RN License, Furnishing number, Advanced Degree in Nursing/Advance Practice Nurse Certification. Minimum of 1 year experience as a Nurse Practitioner or Physician Assistant preferably working with seniors. Ability to utilize Clinical/Nursing expertise in order to make appropriate decisions in the plan of care and comfortable with Assessment skills. Assist vascular surgeon, supervise staff, with all aspects of this encompassing vascular surgery practice. Academic practice includes performing H&PS, assisting in all spectrum of vein procedures (RFA/laser/sclero tx, etc.) in new procedure room. Interact with vascular lab two RVTs in the 2 new ultrasound rooms, participate in a holistic approach to wound care that includes sharp debridement, vacs/artificial tissue placement in the new 2 bed wound care suite, administer medications including intravenous solutions, other vascular procedures and supervise rotating students. Requirements: Certified Physician Assistant/Certified Nurse Practitioner/R.N. with specialty in surgery (CNS). Base knowledge commensurate to degree.
