1 Health and Social Care Foundation Degree Learning Agreement The philosophy behind using a Learning Agreement is that of clearly stating the responsibilities of all those involved in the learning process before it takes place. It clarifies expectations on all sides and helps to ensure commitment from the signatories towards achieving the learning objectives The agreement is made between the Foundation Degree Student, Mentor, Assessor, Line Manager, Clinical Practice Facilitator and the Training Provider. The objectives of the Agreements are to: Identify and clarify the roles of the Foundation Degree Student, Mentor, Assessor, Line Manager, Clinical Practice Facilitator and Training Provider. Specify the commitment required by all parties to the achievement of the Foundation Degree. Identify any special needs. The Foundation Degree Students role The Foundation Degree Student must attend and be punctual to all scheduled tutorials/skills placements and study days. Annual leave will not be granted where it coincides with these events unless agreed in advance by the Line Manager and Training Provider. If the Foundation Degree Student is unwell on a study day or tutorial they must inform both the Training Provider as well as their Line Manager. It is expected that the Foundation Degree Student complete all workshops facilitated/provided by the Training provider or practice trainer/site (more than 3 taught days results in withdrawal from programme). Requires attendance for the overall programme within the work environment and study days is a minimum of 80%. Each individual Foundation Degree Student is responsible for their own learning. Each individual Foundation Degree Student is responsible for developing their own portfolio of evidence. The Foundation Degree Student is responsible for keeping up to date their own daily records of learning assessment documentation and organisational documentation. The Foundation Degree Student is responsible for arranging to meet with their assessor, mentors, University Tutors and Clinical Practice Facilitator.

2 The Foundation Degree Student is responsible for keeping their manager abreast of all internal and external learning opportunities that have been identified. The Foundation Degree Student will be respectful, trustworthy, non-judgmental and abide by the organisations confidentiality requirements. The Foundation Degree Student will identify and plan learning time required in liaison with the line manager in charge of the service. The Foundation Degree Student should ensure they use the learning resources in a manner, which is fair and equitable to all trainees. Sickness absence from study days or tutorials will be dealt with in line with the Sickness Absence Policy. If the Foundation Degree Student experience difficulties during their training they may be set specific performance targets in addition to those required for Foundation Degree. This will be addressed through Disciplinary/Capability procedure, as appropriate. The duration of the programme is 24 months. You will remain on your current terms and conditions (same salary, same scale, and same annual leave entitlement) and in addition attend 51 days and an hour a day or pro rata equivalent for clinical practice relevant to the Foundation Degree Student role. Success on the Foundation Degree is dependent on achievement of the required Foundation Degree standard. If the Foundation Degree Student fails to achieve the required Foundation Degree competences they will be dealt with in line with the University s procedures for managing Students who are failing to progress and according to Trust policy. The Assessor Role Support the Foundation Degree Student for at least for one hour a day or pro rata equivalent to facilitate coaching/training/observation. Work with the Foundation Degree Student and undertake performance assessment. Judge whether the Foundation Degree Student is competent to national standards using the evidence presented by the trainee. Liaise with the Training Provider and Line Manager regarding the progress of the Foundation Degree Student and any concerns. Participate in the evaluation of the training initiative within the care setting. Inform the Training Provider if there are any difficulties you are experiencing in your role. Assess evidence to Foundation Degree competencies and relevant standards, ensuring you are correctly completing all relevant documentation whilst performing the required activity.

3 The Mentors Role Support the Foundation Degree Student in their workplace and help them put their learning into practice and remain accessible to trainee. Provide objective and trustworthy support. Highlight learning opportunities obtainable through work tasks and relationships with others. Empower the Foundation Degree Student, especially when faced with difficulties in learning. Foster a self-managed approach to learning. Assist the Foundation Degree Student, assessors and Training Provider to review the programme and develop action plans to facilitate the success of the programme. Provide opportunity and support cultural change for implementation on newly acquired clinical skills. The Line Managers Role Provide the Foundation Degree Student with capacity to ensure appropriate competences and skills can be gained to fulfil requirements for Foundation Degree. To create opportunities where learning can be applied and develop competencies continually in the work place. Includes identifying time within the workplace i.e. allowing at least one hour per day or pro rata equivalent for study time and time for the individual to attend university study days (1 day per week) To roster the Foundation Degree Student and Assessor on the same off duty for sufficient time to undertake workplace assessment. To ensure the Foundation Degree Student and Assessor have sufficient time allocated to undertake assessment. To ensure the Foundation Degree Student and Mentor have sufficient opportunities to meet. To liaise with all parties regarding the Foundation Degree Students progress and assist in resolving any problems that may arise. Undertake and review the Foundation Degree Students appraisal as per Trust Policy. Work closely with all parties to evaluate the training programme.

4 The Clinical Practice Facilitators Role Oversee the coordination of the Foundation Degree Programme. Be responsible for the Foundation Degree Student recruitment process. Monitor the Foundation Degree student progress/achievement. To escalate corporate issues impacting on the success of the Foundation Degree Programme. The Training Providers Responsibility To support the Foundation Degree Students Assessor, Mentor and Line Manager throughout the training programme by providing a co-ordination process for all parties to communicate. Monitor the progress of all parties ensuring equal opportunity and the appeals procedure are appropriately integrated into the programme structure. Monitor the progress of the Foundation Degree Student in the workplace and classroom and agree identified targets with their manager, assessor and mentor for the trainee to achieve. Ensure all required tasks are given and explained. Provide SWBH with regular progress reviews for each candidate The Learning Agreement All parties should sign to say that they are committed to the fulfilment of the contract before the study commences.
