10 FITA ATHLETE S ANTI-DOPING AGREEMENT Return to the organizing committee the latest registration in Sopron-Ägfalva III. FITA 3-D World Archery Championships, Sopron-Ágfalva, Hungary June, 2007 I, as a member of a Member Association of FITA or participating in a FITA authorized or recognized event, hereby acknowledge and agree as follows: 1. I have received information on the FITA Anti-Doping Rules and had an opportunity to review them. 2. I consent and agree to comply with and be bound by all of the provisions of the FITA Anti-Doping Rules, including but not limited to, all amendments to the Anti-Doping Rules* and all International Standards* incorporated in the Anti-Doping Rules. 3. I consent and agree to the creation of my profile in WADA Doping Control Clearing House (ADAMS) and/or any other authorized National Anti-Doping Organisations (NADOs) similar system under FITA s agreement for the sharing of information, and to entry on my doping control an Therapeutic Use Exemptions related data is such systems. 4. I acknowledge and agree that FITA and its Member Associations have jurisdiction to impose sanctions as provided in the FITA Anti-Doping Rules. 5. I also acknowledge and agree that any dispute arising out of a decision made pursuant to the FITA Anti-Doping Rules, after exhaustion of the process expressly provided for in the FITA AntiDoping Rules, may be appealed exclusively as provided in Article 13 of the FITA Anti-Doping Rules to an appellate body for final and binding arbitration, which in the case of International-Level Athletes is the Court of Arbitration for Sport. 6. 11 FITA PARTICIPATION WAIVER AND COMPETITORS AGREEMENT EVENT: FITA 3-D WORLD ARCHERY CHAMPIONSHIPS Hungary, 6 9 June 2007 This form needs to be signed by all the athletes before they can be accredited at the event. The event detailed in the document heading will be referred to as The Event. As in any sporting event, athletes always run the risk of injury or worse. Archery is no different. Therefore, the following waiver is currently required for participation. In consideration of my involvement in The Event, I acknowledge and agree that: 1. I understand that participation in archery competition may involve risk of bodily injury, including but not limited to paralysis, dismemberment or even death, as well as loss of or damage to property. 2. I fully understand such dangers and knowingly and freely assume all such risk, and 3. I for myself and on behalf of my heirs, assigns and next of kin, hereby release, hold harmless and promise not to sue FITA, their Officers, Officials, Organisers, Agents, Sponsors and/or Employees and/or other representatives ("The FITA Agents"), with respect to any and all such injury, paralysis, dismemberment, death and/or loss or damage arising from my participation in the except that which is resultant of gross negligence and/or wilful or wanton misconduct by FITA and/or FITA agents. 4. I promise to abide by all FITA rules, regulations, decisions and rulings, including without limitation all those relating to doping or use of drugs. I agree to submit all disputes or claims of any nature relating to the Event (including without limitation its conduct, sponsorship and my participation therein), FITA s rules, regulations, rulings and decisions on all subjects to binding arbitration before the Court of Arbitration for Sport under its statutes and regulations. 5. The sponsors of The Event and FITA may use my photograph, image, name or nick name for the promotion of the Event, FITA or archery events. FITA Sponsors or Partners may use my name and likeness to describe any prizes given by Sponsors or Partners without implying any endorsement of their product or service. This use may not include any endorsement by me. I am free to negotiate these endorsements on my own. 6. I will accept any prizes or awards given to me by FITA or the sponsors of The Event for my participation, and I will attend all special award ceremonies at which prizes or awards are presented. 7. If I am invited to attend a press conference by the FITA Press Officer or his representative I will without any delay attend this conference. FITA 3-D World Archery Championships, Sopron-Ágfalva, Hungary 6-9 June, 2007 IMPORTANT INFORMATION Location: The World 3-D Archery Championships 2007 will take place at The Vadon Horse Club Ágfalva, Magyar Street 25, Sopron-Agfalva, Hungary. Entry Fees include transport between the airport/railway station and hotels and return, local transportation between hotel and competition site as well as lunches from Wednesday Saturday (incl.) on the competitionfield. Entryfees: Archers Euro Teams Euro 50.- Officials Euro 60.- Travel: Place of arrival: Sopron is easily reached by plane through the Austrian International Airport Schwechat/Vienna, which has daily connections to major European airports. Vienna airport is located about 60 km away from Sopron City. If you travel by train the nearest railway station is Sopron. Ágfalva is 4 km from Sopron. World Championships: The competition consist of the following categories: Instinctive Bow, Compound Bow, Barebow and Longbow. In each category the individual competition event will be: 2 qualificationrounds followed by the 1 st eliminationround for the first 16 athletes in each category, followed by the 2 nd eliminationround for the first 8 athletes in each category after, followed by the Final Rounds - 4 athletes in each category The team event consists of the: Team elimination round (top eight teams 3 athletes per team after the 2 individual qualification rounds), followed by the Team Finals Rounds for the top four Teams from the Team elimination round If possible we ask you to send passport type photos of all archers and officials together with their names to us, by in good time. There will be a practice field ( Vadon Horse Club ) next to the competition field. The practice field will be open for training purposes to archers two days before competition event. The same field will be used as the warm up area for the competition days. All teams are requested to bring along whit them a CD with their national anthem and 3 National flags (100x200 cm) Doping control will be executed according to the FITA rules.

14 Lunch packets: 2 sandwiches, fruit, drink. There is a restaurant near the competition field where athletes can buy a meal. Electric outlet of 220 V We provide enclosed, some information about visas. However, this information is not binding, so please contact the Hungarian Embassy in your country to check if you need visa. You find all documents and other information also on our website: Please note other following important information You ll find attached a waiver form and anti doping agreement that ALL ATHLETES (or their parents/ guardians for minor athletes) must sign and send to HAA office in Budapest, or bring whit them in order to get their accreditation. Accreditation won t be possible without this form! Upon receipt of your entries, a confirmation for the number and category of athletes registered will be sent to you. The HAA will confirm also your accommodation, travel and/or all related payment issues. For TV media purposes, it would be of great assistance to us if we could receive a biography of each athlete (see attached form) We can receive this information by fax ( ) or (fita3d@misz.hu and misz@misz.hu) or at the latest in Agfalva upon delivery of the archer s accreditation All Member Associations in good standing (see Appendix1 of Book1, article definition) may enter athletes Member Associations wishing to enter athletes in a FITA Championship will return the completed entry forms to the Organizers as follows: A printed copy of the current prohibited list can be obtained from the FITA Office. No later than ninety (90) days before the first day of competition for the Preliminary Entry Form; No later than twenty (20) days before the first day of competition for the Final Entry Forms; The names of the members of a national Field team will be communicated to the Organizing Committee before the start of the Field Team Elimination Round Entry forms submitted after the specified closing date may be refused by the Organizers; however, if the cause of the delay is due to circumstances beyond the control of the Member Association concerned, the Organizers may accept that entry. The Secretary General must be informed at once of this late acceptance and of the reasons for the delay.
