Some of you will attend the ECCO/ESMO meeting in the RAI conference centre prior to our PanCare meetings. Most hotels are in close proximity of the public transport system in Amsterdam. If your hotel is located somewhere else and you need directions from your hotel to the Central Station, the metro or the venue, please let us know. If you want advice or help with finding a hotel please let us know, and we will see what we can do. For specific PanCareSurFup questions you can contact Elise: elise.kvarnstrom@med.lu.se Amsterdam All you want and need to know about Amsterdam can be found on: and for the Netherlands on: Travelling to Amsterdam Wherever you are travelling from, Amsterdam is easily accessed by road, rail and air. The International Airport is a 20 minute train journey from the city centre and is well serviced by train and taxi services. You can also take a train directly to the Academic Medical Centre (station Holendrecht). The Central Train Station offers excellent rail links to the rest of Europe and is ideally situated in the city centre. If you need specific information, please let us know. Venue The meeting will take place in the Academic Medical Centre (AMC) in Amsterdam, Meibergdreef 9, 1105 AZ Amsterdam, the Netherlands. For specific rooms: see program. We 1

2 will make sure that there are signs guiding you to the locations. Both locations are close to the main entrance and located on the ground floor. The AMC is easily reachable by public transport, either by subway (metro) or train, the train/metrostation Holendrecht is within walking distance. From the Central Station in Amsterdam you take the metro in the direction of Gein (leaves every 10 minutes, and the journey is approx. 20 minutes). Leave the metro at station Holendrecht. Walk in the direction were the metro is going, you will see the AMC on your right hand side. Leave the station and walk to the main entrance of the hospital (see map of the AMC, entrance number 1), enter the hospital and walk straight ahead. On Tuesday afternoon we are the guests of the radiotherapy department (located on B0, room B0-133, after the entrance directly on your left hand side). On Wednesday and Thursday, we have one of the conference rooms on the ground floor: the Costerzaal (located on G0): walk from the entrance straight to the square between the F- and G-building, and go right. The Costerzaal is located just behind the bookshop on your left hand side. We will make sure there are sign posts. 2

3 Transportation You will need to buy tickets for the public transport. There are a few options: 1 Hour disposable card: GVB sells one-hour disposable chipcards on trams and buses. This card is also available at the GVB Tickets & Info kiosk and at Ticket Vending Machines in the metro stations. This card provides you with unlimited travel on all GVB routes regardless of distance. The cards are only valid on GVB vehicles, so it is not possible to transfer to other carriers such as Connexxion or EBS. The 2013 fare price is: The card becomes valid on the first check in. Within this period, there is no limit to the transfers you can make between GVB routes. However, you must still check out (when you disembark) and check in (when you embark) or the card will stop working. After the time period expires, the card can no longer be used. Day tickets The GVB day or multiple day card entitles you to unlimited travel around Amsterdam - day and night - on bus, tram and metro, for the number of hours that best suits your plans. For instance, the day card is valid for 24 hours from first check in. 3

The GVB hour cards are available at the same places as the 1 hour ticket. Anonymous travel card (OV-chipkaart) An anonymous OV-chipkaart has a one-time cost of 7.50 and can be purchased at sales machines at stations, various tobacco specialty shops (such as Primera), at many supermarkets, at some Bruna's, and at GVB Tickets & Info. The card is valid for four to five years. You can load and use a card immediately. Traveling on your e-purse balance is simple. You check in at the beginning of your journey, you check out at the end of your journey. The fare is automatically calculated and debited from your card. You can load a maximum balance of on your card. You can load an e-purse balance at the Ticket Vending Machine at all metro stations in Amsterdam. See the map of metro stations to learn how you can pay at a machine with a cash or debit card and the yellow Add Value Machine in tobacco specialty shops and supermarkets.
